Risk factors for infantile hypothermia in early neonatal life.

Hypothermia in infancy is not uncommon among the low socioeconomic population in various parts of the world. It is prevalent in Israel and is associated with severe morbidity and mortality. We tried to identify neonates at risk among the population of the Negev district of Israel. Ninety-one infant hospitalized with infantile hypothermia (IH) during the years 1974 to 1981 were identified. The neonates belonged to two distinct ethnic groups, Bedouins and Jews, and were compared with 120 healthy controls of similar background. Our data show that premature infants and babies with low birth weight born during the cold season to young (inexperienced) mothers of lower socioeconomic strata and who sustained perinatal morbidity are at risk for IH. It is suggested that parents of infants at risk should be approached while the baby is till in the nursery, be advised about the possibility of hypothermia, and institute the appropriate preventive measures.

Sulphur bath and mud pack treatment for rheumatoid arthritis at the Dead Sea area.

Forty patients with classical or definite rheumatoid arthritis in a stage of active disease were treated for two weeks at a spa hotel. The patients were divided into four groups of 10. Group I was treated with daily mud packs, group II with daily hot sulphur baths, group III with a combination of mud packs and hot sulphur baths, and group IV served as a control group. The patients were assessed by a rheumatologist who was blinded to the treatment modalities. Statistically significant improvement for a period of up to three months was observed in the three treatment groups in most of the clinical indices. Improvement in the control group was minor in comparison and not statistically significant. No significant improvement was observed in any of the laboratory variables measured. Except for three mild cases of thermal reaction there were no side effects.

[Multiple cartilaginous exostoses].

Multiple cartilaginous exostosis (MKE) is an autosomally dominant hereditary disease with great individual expression. The disease becomes apparent most frequently at the age of 2-5 years and approximately 80% of the cases have been diagnosed before the age of ten years. The exostoses continue to grow until shortly after puberty. The exostoses may give rise to severe deformation of the skeleton particularly around the knee, ankle and wrist joints. Reduction of joint mobility is frequently observed either on account of the size of the exostoses and their localization or on account of the joint deformity. Treatment of MKE is surgical and corrective surgery with removal of symptom-producing exostoses is the commonest intervention. Osteotomies or introduction of joint-prostheses may prove necessary in cases with the severest joint deformities. The exostoses may undergo malignant transformation, most frequently to chondrosarcomata which grow relatively slowly and metastasize late. Treatment of these must be undertaken in a tumour centre where limb-saving tumour surgery is available in cooperation between the orthopaedic surgeon and the oncologist. Families with MKE should be given patho-genetic advice.

Palindromic rheumatism in Israel--a disease entity? A survey of 34 patients.

Over a period of 10 years 34 patients were diagnosed as suffering from palindromic rheumatism. Eighty-one percent of the patients were of North African origin. This is significantly higher (p = 0.01) than the age-adjusted origin of the general population in the region. Attacks were usually monoarthritic or oligoarthritic in nature. The joint most often involved was the knee. Prophylactic therapy with colchicine was ineffective. Gold salts brought about partial remission in three of six patients. Despite a relatively long average follow-up period of 9.3 years and the finding of a positive rheumatoid factor in 12% of the patients, not one of the patients developed rheumatoid arthritis. In 50% of the patients we detected an unexplained elevation in serum globulins and immunoglobulins. The possible association between this syndrome and Familial Mediterranean Fever is discussed.

Improved knowledge of diabetic patients through education of primary care staff.

A sample of 142 diabetic patients was interviewed twice, in 1984 and 1986, following an educational program administered to primary care staff. Positive changes were recorded regarding several aspects. (a) Process of care - the percentage of untreated patients decreased from 15% to 4%; the proportion of patients on diet only increased from 36% to 41%, while the insulin-treated proportion decreased from 12% to 8%; the percentage of patients treated with oral drugs went up from 36% to 46%. (b) Patient knowledge - an improvement in various aspects, such as diet and exercise. (c) Patient behavior - an improvement in adherence to diet and in compliance with medications.

Apocrine carcinoma of the axillae. Case report.

A case of recurrent apocrine carcinoma of the axilla is presented. Following numerous excisions and exairesis of the regional glands, treatment with Tamoxifen was tried unsuccessfully. Local radiation therapy proved efficient. It is suggested that primary local radiation therapy be used as a supplement to aggressive surgery in the treatment of these tumors.

Abnormalities in bilirubin and liver enzyme levels in adult patients with bacteremia. A prospective study.

Eighty-four patients with bacteremia were surveyed prospectively for biochemical markers of liver damage. Aspartate aminotransferase, alanine aminotransferase, alkaline phosphatase, and bilirubin levels were elevated in 44 (53%), 39 (47%), 45 (54%), and 5 (6%) of the patients on the first determination (2.0 +/- 0.1 days after onset of fever) and in 11 (13%), 17 (20%), 26 (31%), and 1 (1%) on the second determination (5.4 +/- 0.2 days after onset of fever), respectively. The elevation rarely exceeded three times the upper limit of normal. One patient had severe jaundice. An abnormality of at least one of these values was found in 55 patients (65%). There were no differences in site of infection, bacteria isolated, and outcome between patients with and without biochemical abnormalities. We conclude that in adult patients with bacteremia, elevation of liver enzymes and bilirubin is common, usually mild, of short duration, and of no prognostic significance.

Post-traumatic bony regeneration in a toe.

A case of regeneration of the major part of the distal phalanx of the first toe after traumatic avulsion in a 15-year-old boy is reported. It is concluded that, owing to the possibility of bony regeneration, it is advisable to be extremely reticent in removing soft tissue in cases of such lesions. Regeneration of the bone was demonstrated after 1 month.

Evaluation of an intervention program for head lice infestation in school children.

In order to investigate the point prevalence rate of pediculosis capitis (human head lice) among children in the southern region of Israel, 1431 elementary school children (6 to 15 years old), representing rural and urban environments, were examined and characterized by sociodemographic variables. An intervention program was initiated immediately after the first examination, which included "health education" for children and parents and free medicated shampoo (with pediculocides) provided for each child detected as "positive." The intervention program was evaluated by a second examination performed on the same population after an interval of 1 month. Fifty-five percent of the children (793 of 1431) were found to be infested with one of the markers of head lice, with the highest rate in kibbutz children (80%) and the lowest rate (37%) among children who live in an urban neighborhood of high socioeconomic status. Analysis of various characteristics (related to the children screened in this study) revealed that crowding was the main factor contributing to the variation in the rates of infestation. Evaluation of the intervention program revealed a significant success in reducing head lice infestation that was not influenced by variation in socioeconomic status or place of residence.

Faculty development in medical instruction: the cognitive dimension.

This study explores the impact of a teacher-training workshop on the cognitive dimension of medical instruction. The major findings indicate that following the workshop: 1) cognitive versatility in the lessons was observed as a result of a significant increase in the number of various activities on all cognitive levels; 2) there was a significant decrease in duration of activities (as percentage of lesson time) at the knowledge level, and a significant increase at comprehension and application levels; 3) a significant decrease was observed in the length of activities (in seconds) at the knowledge level and a significant increase at the evaluation level; and 4) in spite of significant differences among small, medium and large classes, the effect of training was evident in all classes.

Serum amyloid A (SAA) variations in patients with cancer: correlation with disease activity, stage, primary site, and prognosis.

Serum amyloid A (SAA) was determined in 160 patients with cancer. Active disease was associated with high titre compared with the titre in non-active condition (31.8 v 5.8 micrograms/ml, respectively; p = 0.0002). SAA value showed a direct correlation with the stage of the disease: it was lowest at stages 1 and 2 and highest at the metastatic stage 4 (stage 1 v 4, p = 0.001; stage 2 v 3, p = 0.05). Cancers of the lung and unknown primary site were characterised by highly increased SAA concentration. Initial SAA value had prognostic significance: a value below 10 micrograms/ml correlated with survival advantage, whereas a higher initial value indicated a greater likelihood of a poor outcome (actuarial survival analysis p less than 0.001). When stage was accounted for, initial SAA value had significant prognostic bearing on survival of patients with advanced disease (stages 3 and 4) but not on that of patients with limited disease (stages 1 and 2). Serial testing showed good concordance between changes in SAA titre and clinical course.

Linear and nonlinear models of patient satisfaction with medical care.

The relationship between overall patient satisfaction with medical care and satisfaction with various aspects of health services is explored in a community survey of 225 service consumers. Three alternative models are examined: (i) linear compensatory model; (2) nonlinear conjunctive model; and (3) nonlinear disjunctive model. The results indicate that the nonlinear, noncompensatory models predict overall satisfaction as well as does the linear compensatory model. The pattern of findings is different for men and women. The implications of the findings are discussed.
