Nephrostomy in cancer patients: to do or not to do?

One hundred patients who have had nephrostomies performed for obstructive uropathy in the presence of extensive carcinoma have been retrospectively analysed. Patients with prostatic and cervical carcinoma had a moderately good long-term survival and quality of life, but those with bladder carcinoma or other primary sites had poor results. Significant factors in the selection of patients for nephrostomy drainage in the presence of pelvic cancer are discussed.

Metastatic tumors of the penis.

The penis is a rare site of metastatic neoplasm in spite of its rich blood supply, abundant vascular space, proximity of the pelvic organs and interconnections with the pelvic and vertebral veins as well as lymphatics. The location of primary tumor has varied widely. The urinary bladder, prostate, rectum and rectosigmoid were common primary sites. In our series the bladder was the primary site in 3 patients, prostate in 2, and lung in 1. In 1 patient the metastatic lesion was secondary to lymphoma and in another to leukemia. The mode of metastasis, symptoms, treatment and prognosis are discussed.

Constipation in childhood.

Most commonly, constipation in the infant is functional, with increased tone of colonic smooth muscle. Hirschsprung's disease is the best known of the infrequent organic causes. Treatment includes reassurance of parents, use of sugars, fluids and bulk and care of fissures. Voluntary stool withholding occurs in older children, who present with a variety of symptoms. Medicines are required to restore the functional capacity of the colon after chronic distortion by stool. This must be combined with psychologic help.
