Black stains in the mixed dentition: a PCR microbiological study of the etiopathogenic bacteria.

The aim of this work is to emphasize that particular stains on the third cervical of the buccal and lingual surfaces in mixed dentition, called "black stain." Previous research showed the microbiological etiology of this discoloration by chromogen bacterias. Our study shows bacteria spp involved in stains by means of PCR process and electrophoresis gel on the agarose medium. Sample was formed by 100 subject with black stain and 100 control subjects stain-free. A statistical analysis (SPSS 10.0) using X2 was performed in this study. Porphyromonas gingivalis and Prevotella melaninogenica, were not involved in both in black stain subjects and in the control. On the contrary, Actinomyces could be involved in the pigmentation process.

Psoriasis of the dorsal surface of the tongue.

Psoriasis is primarily an inherited inflammatory skin disease, it is characterized by erythemato-squamous lesions that usually involve elbows, knees and the scalp. Oral manifestations are rare in psoriasis, infact, oral psoriasis involves 2% of psoriatic patients and usually it is observed with the onset of cutaneous lesions and progresses with them. Differential diagnosis should be done for Reiter's syndrome, leukoplakia and geographic tongue. The authors describe a case of tongue psoriasis without cutaneous lesions.

Case report: clinical management of hypoplastic amelogenesis imperfecta.

BACKGROUND: The fundamental therapeutic problems related to amelogenesis imperfecta treatment are governed by the need to effect primary prevention interventions, reducing the risk of calculus accumulation and caries. There are also aesthetic and functional rehabilitative needs. Clinical management rehabilitation techniques vary depending on the AI type, but usually require restoration of affected teeth. Where orthodontic problems also exist, these should be corrected prior to the final restorative treatment. CASE REPORT: The use of composite resins in aesthetic restoration of permanent anterior and posterior teeth as affected by hypoplastic AI type is described. This case illustrates a patient who suffered from less attrition than those of the hypomineralized varieties. The therapeutic choice has allowed the correction of vertical dimension without the use of preformed crowns.

Bilateral mandibular cysts associated with cyclosporine use: a case report.

Cyclosporin A (CsA) is used in the treatment of patients undergoing renal transplantation. There are a number of side effects associated with its use. In particular, the gingival overgrowth represents the most important in the oral cavity. The authors present a case of bilateral mandibular cysts in an 8-year-old boy, treated with CsA after renal transplantation. The genesis of the mandibular cysts might be associated with the combined use of CsA and a calcium channel blocker post-transplantation. CsA-induced gingival overgrowth might contribute to cysts by two mechanisms: interference with control mechanisms that regulate the reabsorption of gingival stromal tissue, allowing progressive dental eruption, and an increase in the gingival connective tissue components. Gingival hypertrophy might mechanically obstruct the eruption of the developing tooth.

[3 different methods for evaluating Streptococcus mutans in the saliva].

Three different methods for detection and count of Streptococcus mutans in saliva have been compared with a microbiological method in order to estimate their efficacy and practicality. The study has been carried out on fifty children of six-eleven years old. The first method (stamp method) derives from that of Kohler and Bratthall, improved on the sampling, without saliva stimulation and, hence, more rapid. Furthermore, using a tongue depressor with a shovel-like extremity, we can either restrict the area where the grown colonies must be counted, avoiding any mistake, and stamp the spatula in the centre of the plate, not in the border, as the original method described, pressing it with the same force in every part, so that almost all the saliva collected can be plated on the Streptococcus mutans selective medium. As stated by this method, a tongue depressor is pressed first on the subject's tongue, then on a plate containing the medium. After incubation, the Streptococcus mutans colonies are counted on the plate's area limited by the stamp of the extremity of the spatula. According to Emilson we can also discriminate Streptococcus mutans and Streptococcus sobrinus colonies. The second method is a "strip mutans test" (Dentocult): saliva is stimulated making the subject chew a piece of paraffin and then it is collected passing a plastic spatula for ten times on the child's tongue. The spatula is introduced into a tube containing a liquid medium selective for Streptococcus mutans and then incubated.(ABSTRACT TRUNCATED AT 250 WORDS)

Chronobiological approach to the treatment of essential hypertension: preliminary data.

The effect on bioperiodic parameters of hypertension elicited by the beta-blocking agent mepindolol, administered as a single dose of 10 mg at 8 a.m. vs 10 mg at 8 p.m., was evaluated in this study. Circadian blood pressure monitoring was performed in age-matched patients with essential hypertension after a two-week wash-out period with placebo and four weeks treatment. Patients were randomly allocated to one of the two treatment groups. Standard blood chemistry and electrocardiogram were performed at the beginning and end of the treatment period. Statistical analysis of blood pressure circadian rhythm characteristics (mesor, amplitude, acrophase) showed a significant decrease of mesor of systolic and diastolic blood pressure in both groups. Values of the mesor were not significantly different between the two groups. Interestingly, changes of acrophase and amplitude induced by the drug were different in regard to the time mepindolol was administered. Side effects were generally mild.

Contemporaneous administration of digitalis and dilazep to subjects with heart failure of ischemic etiology.

The objective of this trial was to determine whether the digitalis-dilazep combination interferes with the inotropic effects of digitalis or produces significant cardiovascular modifications. Twenty patients suffering from heart failure of ischemic or ischemic plus valvular etiologies, undergoing digitalis treatment for at least 3 weeks, were administered dilazep (300 mg/day). Systolic, diastolic blood pressures and heart rate, supine and standing, QS2I, LVETI, PEPI, PEP/LVET, triple product, and percentage diastole were recorded. The controls were undertaken at entry, after the digitalis + dilazep association, and after 1 month of digitalis alone when dilazep was ceased. None of the parameters assessed were modified as a result of dilazep treatment. Since systolic time intervals and the PEP/LVET ratio remained unaltered and equal to the values recorded during digitalis alone, it is concluded that dilazep does not interfere with the inotropic action of digitalis.

Plasma and tissue distribution of adriamycin in patients with pelvic cancer.

The levels of adriamycin in plasma, ascitic fluid and normal and neoplastic tissues sampled during surgery of 3 patients with advanced pelvic cancer were measured by fluorimetry. The highest content of fluorescent compounds was found in tumoral masses in necrotic or scarcely viable tissue; viable and invasive tumor areas scored fluorescence levels comparable with normal adnexa. Ascitic fluid contained levels of fluorescence comparable to the last observed phase of plasma levels. Adipose and cutaneous tissue scored the lowest concentrations.

Pharmacokinetic approach to in vitro testing of ovarian cancer cell sensitivity.

Cell populations obtained from ovarian cancer specimens were seeded in primary culture and morphologically identified as cancer cells. Methotrexate, cytosine arabinoside, 5-fluorouracil, antinomycin D, melphalan, and adriamycin were added to the culture medium at different concentrations and for various periods of time. The results are discussed in relation to the pharmacokinetic availability of drugs in the plasma compartment of patients treated by different therapuetic regimens. Totally inactive drugs can be identified by comparing plasma levels with active concentrations while for drugs active in vitro at concentrations in the range of pharmacokinetic levels, the percentage of responders among patients might be explained by the intrinsic variability of cancer cells.

Plasma levels of cyclophosphamide in patients under polychemotherapeutic regimens.

Plasma levels of non metabolized CPA were measured in two groups of lung cancer patients receiving polychemotherapy. There was extreme variability in the data obtained during the first hour after CPA administration. After this period of time a wide range of T 1/2 was found particularly in one of the two groups of patients. "Border-line" patients (with T 1/2 at the upper or lower limits of the range found) are discussed as regards the possible influence of ancillary treatment.

Early childhood caries. Oral health survey in kindergartens of the 19th district in Rome.

AIM: The aim of this cross-sectional survey was to determine the prevalence of early childhood caries (ECC) in a pediatric sample randomly chosen in public kindergartens or the 19th district in Rome, according to the WHO guidelines and to access the dental and orthodontic status of the affected children. METHODS: A total of 546 children aged 3-to 5-years were examined. Clinical examination had been executed from January 1999 to June 2000 by 4 calibrated operators; WHO clinical assessment form and anamnestic questionnaires for parents were performed. RESULTS: Children affected by ECC with a dmft score of 6.3 were 8.2% and 93% were ''treatment need'' (TN); low levels of oral health was significantly related with father's low socioeconomical status, both occupational and educational: most of these families were from less industrialized countries; 40% of these kids used to suck sucking-bottles with honey: many others used to drink too much sweet things (milk and sugar, fruit juice etc.); the most frequent orthodontic features in the ECC group were: brachifacial (26.7%), convex profile (70%), 3rd class terminal plane (30%). CONCLUSIONS: The results obtained point out that information, health education and health promotion are lacking throughout this part of roman population. Primary preventive dentistry programs should be therefore organized targeting parents, nursery schools and kindergartens' operators and pediatricians.
