Rectal manifestations of pellagra.

Twenty (44%) of 45 patients with cutaneous pellagra had diarrhoea. Inflammation was seen on sigmoidoscopy in 42 and was severe in 5 (11%), moderate in 4 (9%) and mild in 33 (73%). Six patients (13%) had perianal excoriation. Rectal biopsies taken in all cases showed extensive ulceration in 1 case, moderate to severe inflammation in 16 (36%) and mild inflammation in 18 (40%). Minimal oedema of the lamina propria was seen in 11 (24%) patients. At 5 to 7 days after starting vitamin treatment in nine cases histological changes had returned to normal in 4, improved in 3 and had not changed in 2. Diarrhoea stopped within a week in all cases.

Hollow visceral myopathy in black South Africans. A report of 14 cases.

Familial (hereditary) hollow visceral myopathy is a rare cause of chronic intestinal pseudo-obstruction. Fourteen black patients with the disease, 6 males and 8 females, are described. The earliest morphological lesion is an isolated smooth-muscle degeneration of the muscularis propria of the bowel, especially the colon. The pathological changes are lysis and disappearance of the muscularis propria of the bowel wall, leading to dilatation and intra-luminal stasis. progressive involvement results in large areas of defective muscularis propria. The occasional occurrence of mesenteric arteries showing medial muscular fibrosis supports the concept of a generalized primary smooth-muscle myopathy.

Effects of very low doses of atropine on basal acid and pepsin secretion, gastrin, and heart rate in normals and DU.

Muscarinic mechanisms in basal acid and pepsin secretion in man were quantitated by graded intravenous doses of atropine (1-16 micrograms/kg). Secretion was dose-responsively inhibited in six healthy controls. For the mean dose response, maximum inhibition (Imax) was 100%, and D50 (dose inhibiting 50%) was 0.31 +/- 0.06 and 0.93 +/- 0.13 micrograms/kg, respectively, for acid and pepsin. In 24 patients with duodenal ulcer (DU), calculated Imax was also 100%, and D50S were 1.2 +/- 0.27 and 1.7 +/- 0.3 micrograms/kg, respectively. The low D50 values and the 100% calculated maximum inhibition indicated that in both groups basal secretion was largely or completely cholinergic dependent. We also found that atropine raised heart rate in controls by 44 +/- 1 beats per min (bpm) (D50 = 6 +/- 1.1 micrograms/kg), while the mean maximum increase in DU was only 23 +/- 2 bpm (P less than 0.01) with (D50 = 5.3 +/- 1.0 micrograms/kg (NS)). In DU atropine increased fasting serum gastrin from 62 to 82 pg/ml (P less than 0.05); the increase in normals from 32 to 38 pg/ml was not significant. Thus, while both normals and DU exhibited the same qualitative responses to muscarinic receptor antagonism by atropine with respect to gastric secretion, gastrin levels, and heart rate, there were quantitative differences in all three parameters.

The endoscopic diagnosis of gastric calcification.

The endoscopic diagnosis of gastric calcification has not been previously reported. We describe the endoscopic and histologic features of gastric calcification in a patient with diffuse interstitial pulmonary calcification of unknown etiology. It is suggested that this investigation may provide a convenient additional method of diagnosis, in a patient with suspected metastatic calcification.

Polyps and colorectal cancer in South African Blacks.

This study reflects the fact that cancer of the large bowel is uncommon in South African Blacks, and that colorectal polyps do not appear to constitute a precursor to most colorectal cancers. Furthermore, it is shown that dietary factors associated with this cancer in Western populations are not evident in the Black population. In the South African White population, however, the disease behaves in a similar way to that observed in Western countries. Other definitive differences found were the absence of multiple synchronous cancers and diverticular disease in the Blacks with colorectal cancer. It is thus postulated that dietary factors are absent, or have not been present for a sufficient length of time to influence the development of polyps or polyp--cancer sequence in this population. It is also possible that the adenoma--carcinoma progression observed in Western countries may not be relevant to the development of all colorectal carcinomas in communities such as those reported here.

Foreign-body ileitis. A case report.

A patient who ingested a soft-drink bottle top and stones, which resulted in a chronic active ileitis, is described. Histological examination revealed a chronic active inflammatory process, probably indicating long-term ingestion of foreign material. The histological features in some respects resembled those of Crohn's disease.

Hiatus hernia in Johannesburg blacks.

With westernization of diet and lifestyle among South African Blacks, particularly urban dwellers, hitherto unknown gastro-intestinal diseases are becoming evident. Previously unreported is hiatus hernia, now a definite disease entity, although still very uncommon. At Baragwanath Hospital, Johannesburg, over 17 months in 1977-1978, 46 patients with hiatus hernia were detected from barium meal examinations on 1392 persons, i.e. 3,3%, or 0,07% of total hospital admissions of adults. The disease affected mainly middle-aged and elderly women. Prevalences of obesity, comparative physical inactivity and hypertension appeared commoner than in the general urban adult population. Just as appendicitis is a marker of westernization in young urban Blacks, hiatus hernia may be regarded as a marker in the middle-aged and elderly.

Cirrhosis of the liver associated with inferior vena-caval obstruction.

Seven patients with inferior vena-caval obstruction associated with cirrhosis of the liver are described. Inferior vena-caval obstruction is suggested when an enlarged azygous vein is seen on chest radiography, or when superficial collateral veins draining upwards are visible on the abdomen and chest. When a short obstruction or 'web' is present, this should be treated surgically. Venous thrombosis below the obstruction may result in occlusion of a long segment of the vena cava. Chronic suprahepatic venous obstruction may cause macronodular cirrhosis and predispose to the development of hepatocellular carcinoma.
