[The condition of the proteinase-antiproteinase system in patients with type II diabetes and chronic obstructive pulmonary disease].

Chronic obstructive pulmonary disease (COPD) and type II diabetes are major worldwide health problems. Proteinase-antiproteinase system (PAS) disbalance is known to play a significant role in COPD pathogenesis. There is increasing evidence concerning proteolysis activation as a factor of vascular complications. The subjects of this study were 130 individuals aged 39 to 68 years, divided into three groups. Group I consisted of 25 patients with type II diabetes, group 2--32 patients with type II diabetes and COPD, group 3--33 COPD patients, and group 4--40 apparently healthy people. Investigation of the PAS in these subjects demonstrated an important role of the condition of PAS in the development of COPD and type II diabetes, and a possible role of a hyposecretory mutation of alpha1-proteinase inhibitor gene as a risk factor of both bronchopulmonary and endocrine pathology.

[State of protease inhibitor system in patients with occupational bronchial asthma caused by nonferrous metal aerosols].

The article represents results of studies concerning biochemical features of inflammation in various types of bronchial asthma caused by nonferrous metal aerosols. Also parameters of "proteolysis--antiproteolysis" system were studied on molecular genetic and biochemical levels, with determining serine proteinase, level of protease alpha-1 inhibitor and its hyposecretory deficit variants, levels of individual proteins.

[Role of adenylate cyclase system in pathogenesis of various types of occupational bronchial asthma].

Occupational bronchial asthma with its prevalence amounting to 14% is one of the main entities in occupational morbidity structure. Clinical evidence in recent decades demonstrates changed phenotype of occupational bronchial asthma. Changes are increased number of patients suffering from the severe asthma, higher occurrence of occupational bronchial asthma which pathogenesis is more significantly mediated by nonimmune mechanisms. Prevalence of these types of occupational bronchial asthma approaches 9.7-22%.

[Biochemical and molecular genetic mechanisms underlying occupational respiratory diseases].

The article presents materials on diagnostic value of biochemical and molecular genetic methods assessing proteinase inhibitor system. The author specified criteria to evaluate biologic balance in "proteinase--inhibitors" system. Those criteria could serve as markers for lung inflammatory and destructive processes. Findings also are criteria of industrial aerosols' influence on pathology course and on individual risk of occupational respiratory disease development.

[Genetic polymorphism and occupational diseases: results of 10-years studies].

The paper presents the results of investigations made at the Laboratory of Environmental Genetics, Medical Genetic Research Center, Russian Academy of Medical Sciences, to search for the markers of genetic predisposition or resistance to a number of occupational diseases (asbestosis, occupational fluorosis, silicosis, dust-induced bronchitis, bronchial asthma).

[Human biochemical profile: theoretic and practical aspects of studies and evaluation in occupational medicine].

Biochemical reactions demonstrate not only common principles and characteristics, but also individual features that form "biochemical profile" providing both adaptive adjustability and unique responses in various individuals. Putting the advances of clinical biochemistry and molecular biology into practice expands approaches to study of occupational diseases.

[The role of cardioxane (ICRF-187) in the prevention of cardiotoxicity of anthracyclines in the combined drug therapy of extensive ovarian cancer].

The investigation was concerned with clinical application of cardioxane (dexrazoxan, ICRF-187) which is intended to counteract the cardiotoxic effect of anthracycline drugs. It was tested in 24 courses of combination chemotherapy (CAP) in 48 cases of extended ovarian tumor. The "threshold" total dose of doxorubin (500 mg/m2) which caused persistent cardiomyopathy in such patients as well as cases of relapse was practically never reached due to the absence of therapeutic effect. The total dose of doxorubicin of 120 mg/m2 raised the likelihood of acute cardiac intoxication. With prophylactic administration of cardioxane, clinical signs of acute cardiointoxication were slight; irreversible intoxication was recorded in 0.8%. The drug improved tolerance; it neither increased the overall toxicity of combination chemotherapy nor affected the results.

[The problem of genetic-biochemical basis of individual sensitivity in occupational medicine].

Human sensitivity to various factors of occupational environment (toxic chemicals, aerosols with mostly fibrogenic effects, etc.) depends on individual hereditary features determining adaptation or dysadaptation with subsequent disease. Studies of propensity and resistance in individuals exposed to occupational hazards should cover analysis of genetic basis for polymorphism in certain biochemical systems determining relationships between hazards detoxication and activation, DNA alteration and repair.

[Genetic aspects of silicosis: polymorphic gene distribution frequency].

Electrophoresis and isoelectrofocusing were used to study polymorphism by 7 genetic loci: haptoglobin (Hp), proteinase inhibitor (PI), transferrin (TF), Vitamin D-transporting protein (GC), complement 3 (C3), phosphoglucomutase 1 (PGM1) and glyoxalase (GLO1) in 60 patients with silicosis and in 70 apparently healthy workers of the Dynamo plant. Comparison of the study groups by significant differences in the summary of the genetic information obtained suggests that 5 (Hp, C3, TF, PI, PGM1) of the 7 studied systems showed the hereditary features of silicosis. The gene carriers Hp*2, C3*F, PGM1*2-, PI*M1, TF*C1, TF*C16 TF*D, GC*R due to peculiar biochemical processes appear to have less adaptive potentialities and a greater likelihood of the disease on exposure to industrial factors.

[Changes in the state of monooxygenase system and lipid peroxidation under the effects of carcinogenic dust].

The work was aimed to study relationship of monooxygenase system and lipid peroxidation in experiments and in clinical group. The examinees were workers engaged into graphite ware production and exposed to low fibrogenic dust of coke and graphite with carcinogens (including 3,4-benzpyrene). The experimental data and examination materials prove the carcinogens to alter seriously those systems. Long stimulation of monooxygenase system and activation of lipid peroxidation could result in more intensive carcinogenic effects of polycyclic aromatic hydrocarbons being a component of coal pitch.

[On phenotype and genotype features of serum and erythrocyte proteins in occupation-related bronchopulmonary pathology].

Electrophoresis and isoelectric focusing assessed distribution of phenotypic and gene frequency for 7 highly polymorphic proteins in 163 patients having occupational chronic bronchitis, pneumoconiosis and bronchial asthma. Phenotypic frequency studies, when compared to the reference group, revealed significant differences in the following parameters: proteinase inhibitor, 3rd component of complement, transferrin, serum blood-group specific component, phosphoglucomutase of RBC. Gene frequency studies revealed the significant differences in 3rd component of complement (C3), transferrin (Tf) and phosphoglucomutase of RBC (PGM): the patients demonstrated increased amounts of C3 F, Tf C3 and PGM 2b alleles. The results could be used for individual forecasting the risk of occupational diseases and for choosing the individual prophylactics.
