[Diagnosis and treatment of liver echinococcosis complicated by cysto-biliary fistula].

890 patients with echinococcosis of the liver were operated during 1976-1992. Complicated forms of the disease were li detected in 222 (24.9%) patients, among them 141 (15.8%) patients with cysto-biliary fistulas. Ultrasonography examination and endoscopic retrograde cholangiography were regarded as the most informative methods of preoperative diagnosis of cysto-biliary fistulas. Three-stage management was worked out. The usage of laser, plasmatic scalpel and ultrasound of low frequency substantially contributed to improvement of the results of treatment of these patients.

[Combined use of peritoneo-atrial shunting and endoscopic sclerotherapy of esophageal varicose veins in treating liver cirrhosis with portal hypertension and resistant ascites].

The authors in 39 patients with liver cirrhosis and intractable ascites performed peritoneoatrial shunting (PAS), in 16--PAS in combination with prophylactic endoscopic sclerotherapy (ES) of gastric varices (GV). Prophylactic use of ES before PAS permitted to reduce more than 3-fold the incidence of postoperative bleeding from EV.

[Endoscopy in the evaluation of rare forms of gastric ulcers].

The endoscopic examination made in 49309 patients with long standing "gastritis" history has revealed 1476 patients (2.99%) with ulcers of gastric localization. The diagnosis of ulcer disease of the stomach had been previously established only in 17.5% of them. The analysis performed has revealed atypical rare forms of gastric ulcers: by their localization (proximal, ring of the pylorus or the prepyloric portion), by the size (giant), by the amount (multiple), by typological features (associated or secondary forms). Detailed characteristic of the ulcers mentioned is given.

[Ligatures of gastrointestinal anastomoses and their role in postgastrectomy diseases].

The clinico-endoscopic investigations have shown the ligation fistulas observed in 2,3% of patients following operations on the stomach to give the clinical picture characteristic of the operated stomach gastrites. Sometimes they are complicated by hemorrhages. All the ligations must be ablated in one or several steps. It results in recovery of the patients and disappearance of characteristic clinical symptoms.

[Electrocoagulation in the arrest of gastrointestinal hemorrhage].

The curative endoscopy with diathermocoagulation was performed in 100 patients with acute gastro-intestinal bleedings. Peptic ulcers were the cause of the bleedings in most patients (64). The complete arrest of hemorrhage was achieved in 75 patients. Recurrent bleedings at different terms were noted in 25 patients. Patients with critical degree of blood loss (in repeated bleedings) must be subjected to urgent operations without loosing time for a repeated attempt to stop the bleeding. Urgent operations were made on 19 patients.

[Bleeding gastric ulcer].

Under study were 113 patients with hemorrhagic ulcer of the stomach. It was established that gastric ulcers were the causes of hemorrhage from the upper parts of the gastrointestinal tract in 9,3% of cases. Hemorrhagic ulcers of the stomach were the cause of hemorrhages of ulcerous etiology in 22% of cases. Predisposition of gastric ulcers to profuse hemorrhage, high percentage of their degeneration makes such radical surgical intervention as stomach resection very expedient.
