Combination radiotherapy and chemotherapy in murine bladder cancer.

A murine transitional cell carcinoma tumor model has been used to evaluate the combined use of radiotherapy and three chemotherapeutic agents, doxorubicin hydrochloride (Adriamycin), cyclophosphamide, and cis-diamminedichloroplatinum. The preliminary radiation therapy evaluation demonstrated that the tumor is radiosensitive. The combined use of cyclophosphamide and radiation therapy was the most effective regimen, especially when the drug was started prior to radiotherapy. Cis-diamminedichloroplatinum and radiation were a lethal combination, but a synergistic effect was produced when the drug was given after completion of radiotherapy. Doxorubicin hydrochloride and radiotherapy may be synergistic, but more evaluation is necessary.

Nondelay in vesicovaginal fistula repair.

Current attitudes toward closure of vesicovaginal fistulas have engendered a period of delay of three to six months prior to surgical closure. We believe that such delay is unwarranted, and describe our experiences with the closure of these fistulas as soon as the diagnosis is made. We consider our success to be good and therefore advocate a more direct approach to these injuries.

Childhood urethral trauma.

Urethral trauma in childhood due to external violence other than vehicular accident is discussed. The need for careful, planned management is highlighted by a satisfactory outcome in each of 4 cases, with no incontinence and no persistent urethral stricture. Temporary suprapubic cystostomy in 2 of these children caused no difficulty in the ultimate repair and permitted a more deliberate approach to these diminutive strictures in the pediatric age group.

Urothelial neoplasia: what constitutes adequate follow-up?

The need for constant vigilance in the follow-up care of patients with transitional cell carcinoma is illustrated by the case reports of 12 patients whose tumors recurred after tumor-free periods of eleven years and longer. Emphasis is placed on monitoring, which includes pyelographic study, cytologic study, and cystoscopic examinations for as long as the patient survives.

Ileal loop in the treatment of radiation-treated pelvic malignancies: a comparative review.

Critical analysis of ureteroileal diversion in the treatment of pelvic tumors in which prior radiation therapy was administered has prompted us to review our experience with ileal loops after patients have been irradiated. Between 1970 and 1975, 136 adults underwent ureteroileal bypass. Of these patients 36 had preoperative irradiation for various pelvic malignancies, 44 had malignancies but no irradiation and 56 were operated upon for neurogenic bladders. Major morbidity in the irradiated group included wound infections (33 per cent), ureteroileal urinary leaks (19.4 per cent) and paralytic ileus (13.9 per cent). The average postoperative hospital stay for this group was 8 to 11 days longer than that of the other 2 groups. In contrast, however, the overall incidence of reoperations for various complications in the irradiated group was only 11 per cent, compared to 25 per cent in the group not given preoperative radiation and 30.4 per cent of those operated upon for neurogenic bladders. Most significantly, none of the patients in the irradiated group required re-exploration and surgical correction of ureteroileal urinary fistulae. Comparison of these results to those reported in the literature is favorable. The ileal loop remains a simple and viable operative technique in patients requiring urinary diversion for pelvic malignancies previously irradiated.

Synergistic gangrene of the scrotum and penis secondary to colorectal disease.

We recently encountered 5 patients with synergistic gangrene of the genitalia secondary to colorectal disease. This number of cases suggests that there may be an increasing incidence of this entity that has been reported only several times in the past. Pseudomonas was one of the organisms cultured in all cases. Because of the high mortality rate we advocate aggressive therapy, consisting of early and repeated radical débridement into normal tissues and appropriate management of the colorectal source of contamination. Our experience indicates that the lower gastrointestinal tract should be considered as a possible cause of infection in all cases of synergistic gangrene of the scrotum and penis.

Microsurgical anastomosis of vas deferens: an experimental study in the rat.

A patency rate of more than 90 per cent was achieved after 52 vasovasostomies. Satisfactory results were shown in both internally stented and nonstented anastomoses. A higher incidence of foreign body reaction was associated with silk sutures. Sperm granulomas and reduced patency were noted when superficial sutures were used indicating that a leakproof anastomosis is essential.

A comparison of the healing processes in uremic and parallel-fed rats.

The effect of nutrition on the healing capacity of uremic rats was investigated. Wound tensile strenth and the amount of collagen formation in polyvinyl sponges implanted subcutaneously were compared in a group of uremic rats and a group of nonuremic rats parallel-fed an identical diet and caloric intake. Wound tensile strength in the parafed rats closely and significantly approximated that in the uremic snimals conpared to the stronger wounds in the controls. A correlation was also noted for collagen accumulation in the uremic and parafed groups. This experiment lends further evidence to support the theory that the mechanism of action for the shown poor healing associated with uremia is based on the poor state of nutrition in the uremic animal.

Epidemiology of cancer of the penis.

World-wide incidences of penile cancer are reviewed and epidemiologic factors including ciecumcision, hygiene, phimosis, smegma, irritation, infection, veneral disease, viruses, environment, race, immune response, trauma, and age are discussed. The lowest rates appear in groups practicing infant circumcision and in areas where standards of sexual hygiene are high.

Is cystectomy a safe procedure in elderly patients with carcinoma of the bladder?

Often total cystectomy is not advocated in elderly patients for the treatment of carcinoma of the bladder. A group of 25 patients more than 70 years old were reviewed to determine if cystectomy is a reasonable therapeutic alternative in elderly individuals. Since there was satisfactory survival and no immediate operative mortalities elderly patients should not be denied this form of therapy on the basis of age alone. A higher incidence of postoperative complications was noted in the patients treated previously with radiation therapy.

Complications of autonomic dysreflexia.

Autonomic dysreflexia can be a life-threatening problem if not promptly recognized and treated. Since the most common cause is bladder distension it is essential that the urologist be familiar with this syndrome. Several patients with serious complications owing to autonomic dysreflexia, including 1 mortality, are presented. The complications of autonomic dysreflexia result from a sudden marked rise in blood pressure, which may be severe enough to rupture single or multiple cerebral blood vessels or lead to a considerable increase in intracranial pressure. The neurophysiology, clinical features and therapeutic measures are discussed.

Adolescent

Initial complications and late results in dismembered pyeloplasty.

A total of 109 dismembered pyeloplasties has been reviewed and examples of initial complications and their management are discussed. These early complications do not preclude excellent long-term results. Over-all followup studies show resolution of pain, hematuria and infection in the vast majority of patients. X-ray results have since, in general, improved or stabilized. No nephrectomies have been done subsequent to the repair of the hydronephrosis.
