Judicial involvement and conduct problems of fathers of infants born to adolescent mothers.

Data were obtained from retrospective review of 191 patient records to determine the extent and types of conduct problems among fathers of infants born to adolescent mothers. Ninety-eight fathers (51%) reported having committed a legal offense prior to the pregnancy. This rate appears substantially greater than rates of arrests of adolescents in the general population. When paired data for couples were analyzed, there was great similarity for severity of offense. Fathers, however, had committed more severe crimes than their partners. Male offenders came from families of lower socioeconomic status backgrounds and more single-parent families and were less frequently employed or in school (or graduated) than nonoffenders. They also had been more involved with various other problem behaviors, such as a previous pregnancy, drinking behavior, and behavior problems at school. There was a clustering of problem behaviors among offenders suggesting psychosocial maladjustment prior to the pregnancy. Our results suggest an association between fatherhood and delinquency among partners of adolescent mothers. It appears that these fathers form a heterogeneous group. Although some young fathers may be well adjusted, others have significant psychologic problems.

Word associations in old age: evidence for consistency in semantic encoding during adulthood.

Word associations of 80 young and 80 older adults were compared for 113 stimulus words. The proportion of paradigmatic responses varied with the grammatical class of the stimulus word and with the vocabulary level of the subject, but not with age. The same proportion of young and older adults gave the most common responses. Although older adults had a greater number of unique responses, this seems to reflect age differences in vocabulary level, as vocabulary but not age was a good predictor. Within-subject variability was also comparable across age, as on a retest young and older adults gave the same proportion of responses that were identical to those on the original test. Both age groups were more likely to repeat common than uncommon responses on the retest. This, together with analyses of response latency, suggests equivalent use of strategic processes across age. The results indicate that semantic structure and semantic encoding in adults are related to verbal ability, but not to age.

Relapse of acute B viral hepatitis--role of delta agent.

Serologic markers for delta agent were evaluated in 39 patients with acute hepatitis B and clinical relapse within 30 days from the initial episode. Eighteen of the 39 patients (46%) had evidence of acute delta infection. Delta antigenaemia preceded the appearance of antibodies in seven of these 18 patients; delta antigenaemia occurred during the initial episode of illness and the appearance of the antibody coincided with the relapse. Eight of these patients developed severe relapse with fulminant course which resulted in two deaths. This study reveals that delta infection is one of the important causes of severe relapse in cases of acute B viral hepatitis.

Obstructive lung disease after allogeneic bone marrow transplantation.

We report the cases of 3 patients with marked dyspnea and an obstructive ventilation disorder associated with chronic graft-versus-host disease after allogeneic bone marrow transplantation. This disorder was characterized by recurrent pulmonary infections and colonization of the lower respiratory tract by Pseudomonas aeruginosa. Two patients have shown rapidly progressive deterioration with death following due to respiratory failure. Intensive therapy with antibiotics, bronchodilators, high-dose steroids, and azathioprine was not effective in arresting the malignant course of this disorder.

Histologic changes of human hair follicles after electrolysis: a comparison of two methods.

The histologic changes induced by thermolysis of hairs of the scalp and legs were studied by light microscopy. Two techniques were compared on opposite sides: conventional thermolysis (erroneously termed electrolysis) using bare needles and a newly designed precision epilator using an insulated, bulbous-tipped probe. Thermolysis generally did not immediately eliminate the papilla and hair matrix. Thermal injury provoked an inflammatory reaction which eventually destroyed the hair bulb. The insulated probe produced greater damage to the peribulbar tissue below and less necrosis of the perifollicular dermis above, enhancing the likelihood of permanent epilation and reducing the probability of scarring. The infundibulum and associated sebaceous glands regenerated to near-normal architecture. The lower follicle was replaced by a fibrotic streamer--a scar.

Radiation hepatitis following moving strip radiotherapy.

A 71 year-old woman developed acute veno-occlusive disease of the liver after receiving moving strip radiotherapy to the whole abdomen according to the Toronto technique for carcinoma of the ovary. The dose of 22.5 Gy in 10 fractions to the liver is compared with other regimens which have produced this complication, and factors which may have sensitized the liver to irradiation are considered.

Localization of 111In- and 125I-labeled monoclonal antibody in guinea pigs bearing line 10 hepatocarcinoma tumors.

A murine monoclonal antibody (D3) with demonstrated specificity for the guinea pig line 10 hepatocarcinoma (L10) was radiolabeled with either 125I or 111In and used to image dermal tumors in vivo. In one set of experiments, L10 tumors were established middorsally in one group of animals, and the similarly derived, antigenically distinct line 1 tumor was established in another group of animals. In spite of background imaging of liver, kidney, and spleen, L10 tumors were visualized clearly. Incorporation of radiolabel was demonstrated to predominate in the L10 tumor. In a separate set of experiments, L10 and line 1 tumors were established in contralateral thighs in the same animals. L10 tumors were visualized clearly, and tissue uptake of radiolabel was demonstrated to reside predominantly in the L10 tumor.

Autologous bone marrow transplantation in relapsed adult acute leukemia.

From March, 1976 to February, 1979, 28 cases of adult acute leukemia of which 24 were evaluable were treated in irreversible relapse with high dose chemotherapy (piperazinedione) and supra-lethal total body irradiation (TBI) in conjunction with autologous bone marrow transplantation (ABMT). The marrow cells grafted were collected and stored in liquid nitrogen at the time of remission. In 12 patients the marrow cells were fractionated using discontinuous albumin gradients in an attempt to separate normal cells from residual leukemic cells. Twelve patients achieved complete remission (CR); in 9 additional patients signs of engraftment were evident but death occurred before achievement of CR. Seven of 12 AML patients, which were treated with bone marrow transplantation as first treatment of their relapse, achieved CR. Four of 5 patients with ALL, whose bone marrows were collected during first remission, reached CR. The median CR duration was 4+ months and the median survival of the patients reaching CR was 6+ months. Autologous bone marrow transplantation offers a good chance of CR (66%) when marrow is collected during first remission and used as first treatment for AML in third relapse and ALL in second relapse.

Local excision and radiation therapy for early stage breast cancer.

A reasonable alternative to mastectomy for women with early stage breast cancer is wide local excision of the lesion coupled with radical radiation therapy. The recurrence and longevity results with this method have been as good as, if not better, than standard mastectomy after 10 years' clinical experience in a limited number of patients. The physical and psychological impact of this therapy is more acceptable than mastectomy to the woman with breast cancer in its earliest stages.
