[The effect of combined rehabilitation on the physical work capacity of patients who have had a myocardial infarct].

125 patients with past myocardial infarction (basic group) have been treated for 5 years by a comprehensive rehabilitation program and their physical fitness was checked against 35 patients also with past myocardial infarction who did not take part in the rehabilitation program (control group). The rehabilitation program included physical exercises, measured walking and psychological rehabilitation. The results show that the physical fitness of the basic group was significantly higher than that of the control group and the patients could sustain physical load over 600 kgf/min. 88.7% of the basic group patients returned to their professional occupation against 71.2% for the control group. Highest mortality was recorded during the first year following the discharge from the hospital. At the end of the 5-year observation period 77.2% of the basic group patients were alive versus 72.8% of the control group and 70.4% of the basic group patients are working vs 50.8% of the control group. The patients who had taken part in the comprehensive physical rehabilitation are with improved physical fitness (statistically proved). The relation between the restored physical fitness and the real resuming of professional activity is proved.

[The effect of obzidan on the tolerance for physical loading of hypertension patients].

The effect of obsidan (propranolol) on the physical load tolerance was examined in 65 hypertensive patients up to 55 years of age treated for one year in outpatient conditions. The patients were classified into two groups: basic group of 45 patients with hypertension in initial II stage and a control group of 22 hypertensive patients. The basic group patients were treated in the course of one year with obsidan in doses of 80 up to 160 mg daily. The control group patients were treated irregularly with other hypotensive drugs. The dynamic indices were examined in rest and after physical load before and after treatment. The long-term treatment with obsidan led to an improvement of the general condition, effective lowering of the arterial pressure in rest and after physical load, increased tolerance to physical load, decreased double product.

[Secondary prevention of the basic risk factors in patients with a history of myocardial infarction].

160 patients under 60 years of age with a past myocardial infarction, proved clinically and electrocardiographically, were followed up for 3 years during which period a secondary prophylaxis of the basic risk factors was carried out. The smokers were 71.9% and 72.2% of them were over 46 years of age. 65.2% of the smokers gave up smoking after being advised by the authors to do so. Positive changes were found concerning the risk factors under observation in the patients--a statistically significant decrease of the arterial pressure and cholesterol serum level and a considerable increase of the tolerance to physical activity in the non-smokers and the smokers who had given up smoking. Reduction of the frequency of stenocardia attacks, extrasystoles, ST-depression as well as a reduction of the risk of another myocardial infarction were also established. The secondary prophylaxis carried out by the authors in their outpatient practice on patients with past myocardial infarction has proved its efficiency and ought to be recommended.

[The psychological rehabilitation of patients having had a myocardial infarct at the polyclinic stage].

The psychological rehabilitation of patients with myocardial infarction was studied in 104 persons: a basic group of 70 patients and a control group of 34 patients. The E. Key's test recommended by the WHO was used. The mean age of the patients in the basic group was 51 years and that of the patients in the control group was 53 years. The employees were more numerous than the physical workers. The results of the psychological test in the basic group showed a statistically significant decrease of the state of anxiety and depression at the end of the rehabilitation period. In this group the active rehabilitation programme led to considerable favourable changes in the psychic state--improvement of the mood and diminishing of the anxiety and depression and contributed to the favourable end of the process of psychological adaptation to the changed by the disease conditions of life. The patients in the control group who did not perform the rehabilitation programme showed a tendency toward improvement of the psychic state.

[Temporary work incapacity in myocardial infarct patients in the area covered by the 23d Polyclinic in Sofia].

For a five year period 70 patients with myocardial infarction were followed up in order establish the length of the temporary incapacity for work. The chronic coronary insufficiency and cardiac failure were taken into consideration and in all patients bicycle ergometric examinations were carried out. The results show that the basic criterion for restored working capacity is the functional state of the myocardium (stage of chronic coronary insufficiency and degree of cardiac failure). The patients are classified into three groups according to the functional classification of the New York Cardiological Association. Of the patients followed up 88.57% fully restored their working capacity, the other 11.43% remained disabled. The mean length of the temporary disablement of the patients who restored their working capacity was 174 days. The use of non-invasive methods for examination of the cardiovascular system in polyclinical conditions is of basic importance for evaluation of patients' working capacity.

[Physical rehabilitation of patients who have had a myocardial infarct--the polyclinic stage].

70 patients who had suffered myocardial infarction were treated for a period of two months by a rehabilitation program including domestic activity, remedial gymnastics and prescribed walking. The program was carried out in polyclinical and domestic conditions. The patients were classified into functional groups according to the classification of the New-York Cardiological Association. At the end of the rehabilitation treatment the number of patients in functional class II sharply increased, a slight increase was noted also in functional class I, but the number of patients in functional class III significantly decreased. The number of patients able to walk 5 km or more increased, too. Of the 70 patients treated by this rehabilitation program 62 (88.58%) fully restored their working capacity and the functional state of their cardiovascular system was classified as I or II functional class. The remaining 8 patients (11.42%) still needed polyclinical treatment. The authors' experience supports the need of a rehabilitation program for patients who had suffered myocardial infarction which has to be carried out in polyclinical conditions and will be direct continuation of the rehabilitation program carried out in the hospital or in the specialized sanatoria.

[Electrocardiographic changes in the pathological climacteric].

The authors share their experience in the importance of climacteric cardiopathy among the dyshormonal cardiopathies under out-patient department conditions. The observation is based on 20 cases of women, aged from 42 to 49. The subjective complaints of the patients were followed as well as the objective findings, some biochemical indices, arterial pressure and the electrocardiographic image within the course of two years. The results from the electrocardiographic records are of particular importance, giving the changes in T-wave of precordial leads, the necessity of differentiation from those in ischemic heart disease and the effectiveness of the treatment applied.

[Dispensary observation and survival of patients with a history of acute myocardial infarct in relation to the risk factors].

The observation was carried out on a group of 138 patients with a past history of acute myocardial infarction for a period of 5 years, after hospital and sanatorial treatment under the conditions of out-patient department. An age group from 30 to 78 years has been cowered with an average age of 59. The patients have been followed up in a clinical and paraclinical aspect according to certain parameters with the adequate drug treatment. The study brought forth the importance of the rhythm and conduction disorders, advanced after the infarction incidence, reinfarction and the following phenomenal of decompensation with post-hospital lethality. The existence of risk factors was confirmed to be with unfavourable effect as regards the remote prognosis of the patient. The importance of the adequate and purposeful follow-up of the patients, with a past history of acute myocardial infarction, is stressed upon as well as its significance for the increase of the percentage of survival during the post-hospital period.

[Peroral contraceptive agents and arterial hypertension].

The authors' task was to follow up the effect of the peroral contraceptives upon arterial pressure. The study was carried out on 94 females, aged from 20 to 40, distributed in age groups of five years with approximately equal number. Their studies reveal a comparatively early development of hypertonic reaction after the initiation of the medication in a rather high percentage of the females--14.89. The elevation of arterial pressure is in a direct correlation with the age and lipid metabolism. The changes in arterial pressure are mainly on account of the systolic pressure and in direct correlation with the medication, since the values of arterial pressure were normalized after the discontinuation of the contraceptives. Follow up care is recommended to those females with a frequent checking of their health state.

[Outpatient polyclinic rehabilitation of patients who have had a myocardial infarct].

Rehabilitation of patients after myocardial infarction has been organized at polyclinical conditions. Forty four patients, aged from 30 to 56, with a duration of the infarction--3-4 months, in a good somatic state and no complications at present, were rehabilitated. The rehabilitation programme was conducted daily for one month with respective tests of loading. As a result, 52,3 per cent of the total number of patients, have completely restored their capacity for work and--back to their previous labour activity, 34.I per cent were transferred to facilitated labour conditions and only 13,6 per cent were permanently disabled. The last are with rhythm disorders and residual angina pectosis. The rehabilitation period lasted from 6 to 12 months since the onset of infarction. Our data confirm the adequacy and necessity of proper rehabilitation measures under polyclinical conditions.
