[Morphologic study of aneurysms of the thoracic aorta].

Aneurysm of thoracic aorta was found in 20 from 7175 autopsies (0.25%) of persons deceased at the age of more than 15 years. The aneurysm was localized in 4 cases, diffuse in 14 and combined in two cases. Its pathogenesis was related to medionecrosis in 6 cases, to chronic aortitis in 5 and to isthmic aortic coarctation in two cases. Left ventricle hypertrophy which was found in 17 cases was related to arterial hypertension in 10 cases but only twice to a diffuse aneurysm. The aneurysm was closely connected with the cause of death in 10 patients. In a control study of subsequent 100 autopsies the mean aortic circumference was 75 mm in men and 71 mm in women, 59 mm in patients under the age of 50 and 82 mm over the age of 71 years. Medionecrosis was found in 45 cases, mucodystrophy in 8 and chronic aortitis in two cases.

[Atherosclerotic aneurysms of the abdominal aorta].

Aneurysm of abdominal aorta was found in 58 cases (0.87 per cent) of a group of 6646 postmortems. The group comprised persons over 15, 45 males and 13 females. The highest frequency of aneurysms (36) was at the age 71 to 80. Rupture of the aneurysm occurred in 15 cases and vascular diseases represented a half part of other causes of death. All the aneurysms were atherosclerotic, in one case a combination with mucold dystrophy was found. Evidence of arterial hypertension was in 43 per cent of the analyzed cases. Histology was performed in 24 cases and proved a destruction of elastic structures in tunica media, fibrosis, loss of smooth muscle cells followed by substantial thinning of the vessel wall. Widening of adventicia was caused by increase of collagen fibres. Discussion concerned formal pathogenesis of the lesion.

[Extramedullary nodular plasmocytic lymphoma].

A plasmacytic lymphoma of the cervical nodes in a 71-year-old women spread also to the mediastinal nodes and after two years proved fatal. The authors did not detect a rise of monoclonal serum immunoglobulins nor did they detect their presence in the cytoplasm of tumour cells in the nodes subjected to biopsy. The lymphoma was preceded by a specific granulomatous inflammation of the skin and nodes persisting for four years.

[The eosinophil composition during the manifestations of early syphilis].

Histology of ulcus durum with regional simple lymphadenitis and of condyloma latum was presented. Inflammatory lymphoplasmocytic infiltration included eosinophilic leukocytes in both cases. Syphilitic granulomatous lymphadenitis with conspicuous productive perilymphadenitis has been lasting about 1 month. Treponemas in histological section were found only in condyloma latum.

Rheumatic fever situation in Czechoslovakia with special reference to long-term prognosis for rheumatic carditis.

In a prospective study (1961-1972) the incidence of rheumatic fever (RF), development of rheumatic heart disease (RHD) and penicillin prophylaxis efficacy were investigated in 300 000 children and 70 000 adults. Only patients whose index attack met the modified Jones' criteria were included; all were enrolled in a prophylaxis programme (i. m. benzathine-penicillin). The RF incidence decreased from an initial 61.2 attacks per 100 000 in children and 42.9 in adults to below 10 by 1969 and remained low thereafter. In total, 541 children (209 with carditis) and 176 adults (108 with carditis) were followed up for 5.5 years (average) after an index attack. At final examination, no valvular involvement was detected where previous attacks had been without clinical carditis. Patients studied from time of the primary attack with carditis (211) had a very favorable prognosis: only up to 3.5% had developed a moderate to severe heart lesion by the end of the study. However, of 68 adult patients followed up only from the time of recurrence with carditis and often having had a prolonged rheumatic history: 23.5% had a moderate to severe heart lesion and a further 20.6% had died of RHD by the end of the study. Prophylaxis efficacy calculated as a cumulative attack-recurrence rate per 100 patients and over a 5-year interval was 2.72 in protected as against 64.75 in unprotected children (mostly failing to comply with prophylaxis); in adults, the respective values were 1.3 and 24.92. The prevalence of serious residual RHD may be expected to decrease substantially in the future, starting with the younger age groups.

[Fibrosarcoma of the pancreatic head].

A postmortem examination of a 60 year old female patient proved the presence of fibrosarcoma of the head of pancreas. The tumour first became manifest through an accidentally diagnosed pulmonary metastasis two years before death. 14 weeks before death, the patient had been operated on for a metastasis in the brain. Jaundice lasted 10 weeks.
