Acute effect of maternal smoking on the maternal and fetal cardiovascular system.

Maternal and fetal cardiovascular dynamics were studied in relation to maternal smoking in 18 healthy nulliparous subjects randomly divided into a smoking (n = 9) and a control group (n = 9) between 34 and 38 weeks of gestation. At the end of the study, data from 7 smokers and 7 controls were available for analysis. A significant rise in maternal heart rate and systolic blood pressure was observed during and following smoking one cigarette. A significant increase in fetal heart rate occurred following smoking, whereas mean blood flow velocity and vessel diameter in the fetal descending aorta as measured by pulsed Doppler and time motion techniques did not demonstrate any significant changes.

Effect of short-term maternal exercise on maternal and fetal cardiovascular dynamics.

Maternal and fetal cardiovascular dynamics were studied immediately after moderate short-term maternal exercise in 28 healthy nulliparous subjects at between 34 and 38 weeks gestation who were randomly assigned to an exercise group (n = 14) or a control group (n = 14). At the end of the study, data from 11 exercise and 12 control subjects were available for analysis. A significant rise in maternal heart rate and systolic and diastolic blood pressure during exercise was observed. Mean blood flow velocity in the fetal descending aorta as measured by pulsed Doppler ultrasound and fetal heart rate did not show any significant changes. These data indicate that there are no cardiovascular signs of fetal stress immediately after moderate short-term maternal exercise.

The time-distance recorder as a means of improving the accuracy of fetal blood flow measurements.

The influence of pulsatile diameter changes on calculation of volume flow has been studied. In vitro studies and an animal study were carried out with a real-time imaging and pulsed Doppler velocity measurement system. For precise pulsatile diameter information a wall motion tracking device was incorporated. Whereas in vitro a high degree of accuracy was found for the measurements of volume flow, this could not be substantiated in the descending aorta of the fetal lamb, in which Doppler volume flow differed between -7.5 and 17% from magnetic volume flow. In a clinical study the relative influence of various diameter approximations on calculated fetal aortic volume flow was assessed in 16 normal third trimester pregnancies. Depending on the selected diameter approximation method it appeared that differences from 19% underestimation to 9% overestimation in calculated volume flow could be obtained when reference was made to volume flow derived from diameter and velocity information.

[Transabdominal choriocentesis for rapid prenatal diagnosis in the 2d trimester: experiences in 147 pregnant patients].

The simple technique of ultrasound guided transabdominal choriocentesis allows rapid karyotyping early in the second trimester. Choriocentesis was carried out in 147 pregnancies at (genetic) risk during the 13th-19th week of gestation. Results of reliable karyotyping were available within 2-4 days. In 146 (99.3%) women a sufficient amount of chorionic villi could be collected with maximal two needle insertions. An abnormal karyotype was found in four pregnancies, which were terminated selectively on parental request. In all pregnancies a follow-up by ultrasound was performed 4 weeks after the procedure, one (0.7%) unexplained fetal death occurred, 143 pregnancies remained uneventful. We concluded that transabdominal choriocentesis is a simple rapid and relatively safe method, which can be useful for rapid karyotyping during the second trimester.

[Significance of maternal age in assessing the risk of abortion following prenatal diagnosis in the 1st trimester of pregnancy].

A total of 546 consecutive pregnancies was monitored following CVS to define the abortion risk. In 42 pregnancies an affected fetus was diagnosed and pregnancy subsequently terminated. In the remaining 504 pregnancies the fetal loss rate until 28 weeks of gestation was 26% (5.1%). Fetal loss rate was not influenced by the quantity of collected chorionic tissue. However in mothers with advanced age (greater than or equal to 36 years) the fetal loss rate before 16 weeks is related to the number of catheter insertions and increases from 3% after one insertion to 12.5% after three. It is concluded that maternal age is an additional risk to CVS if more than one catheter insertion is employed.
