[The role of the second-look operation in modern therapy of ovarian carcinoma].

The authors evaluate the results of second look operations of ovarian cancer in a group of 34 women. The second look operation was preceded by intraperitoneal and intravenous chemotherapy. This therapeutic protocol contributed to improvement of therapeutic results. In 50% of thus treated women it proved possible to remove the residual tumour and the survival time was prolonged in 75% of women by 1-3 years. The advantages of this therapeutic procedure are according to the authors in particular reduction of the risk of super-radical primary operations and facilitation of the decision whether to terminate treatment or proceed with it.

[New approaches in the treatment of hypotrophic fetuses].

Hypotrophy or intrauterine growth retardation of the foetus associated with placental insufficiency still remains a serious obstetric problem. In the submitted paper the authors compare hypotrophic foetuses treated in 1989 and 1990 at the authors' clinic by conventional methods only (biometry, bed rest, CTG oestriol, glucose and possibly heparin) with a group of hypotrophic foetuses treated in 1991 and 1992 with magnesium and where the therapeutic effect was followed up by Doppler flowmetry. Based on the results, the demand to improve placental insufficiency by pharmacological induction and stimulation of specific placental enzymes is met perfectly by concomitant parenteral and oral magnesium administration to pregnant women.

[Control of labor with prostaglandins after previous cesarean section].

The authors studied the possibilities of vaginal labour management by induction after previous caesarean section. They compared the effectiveness of oxytocin and prostaglandins in uterus action. A total of 48 patients after previous caesarean section were induced, 26 by oxytocin and 22 by prostaglandins. The success of the induction was 30.7% with oxytocin and 95.4% with prostaglandins. Compared were also both the incidence of complications during labour and the effect of the uterotonic used on the fetus. It follows from the results that local application of prostaglandins is more advantageous than overall application and carries practically no risk for the patient in labour.

[Does the spermatozoa survival test correlate with fertilization of oocytes in vitro?].

The spermatozoa survival test (SST)--which is used in our laboratory is considered to be a relatively simple test with a large probability determining the ability of spermatozoa to fertilize oocytes in vitro. The SST reveals the percentage of motile spermatozoa after 20hr. cultivation with an oocyte. 221 sperm samples used for oocyte fertilization were analyzed and divided into four groups: normospermia (123 samples), oligospermia (19), asthenospermia (45) and oligoasthenospermia (34). The results show a significant reduction of the fertilization rate (< 35%) below the range of 40% SST in all groups. Above the value of 40% motile spermatozoa after 20hr. cultivation is the fertilization rate in all groups higher (> or = 50%). The authors recommend the SST as a part of the screening examination before the couple is admitted to an assisted reproduction programme.

[Prophylactic administration of Claforan in patients with premature loss of amniotic fluid].

The authors compared two groups of pregnant women with early loss of amniotic fluid. In the first group Claforan was administered, 3 x 1 g by the i.m. route, and in the second group Ampicillin, 3 x 1 g by the i.m. route. The data were recorded in a special protocol and were divided into three groups: 1. patient before delivery, 2. patient during puerperium, 3. neonate. From the results two conclusions were drawn: 1. The authors did not detect any reasons why antibiotics should not be administered prophylactically in case of premature loss of amniotic fluid. 2. The main differences between the compared groups were found in neonates where after administration of Claforan there was a substantially lower incidence of positive bacterial cultures than after Ampicillin and there was also a lower incidence of RDS II and adnatal infections.

[The status of children and family relations after successful in vitro fertilization and embryo transfer].

Using a retrospective questionnaire method, the authors evaluated the neuropsychologic status of 20 IVF children and the family relationships. The status of the children is favourable and in certain activities (composition of sentences) the children were better than their peers. According to the authors this is due to the increased care of mature parents after the arrival of the long expected child. The authors confirmed on their own material that the birth of an IVF child has a far reaching positive biological, psychological and social impact on both parents and training of their children. If the family environment was influenced by long lasting treatment of sterility, the arrival of an IVF child normalizes the situation.

[Gestrinone in the therapy of sterility due to endometriosis].

Thirty-two infertile women with laparoscopic diagnosis of asymptomatic pelvic endometriosis were treated with gestrinone 2.5 mg twice weekly for 6 months. Subsequently, a second-look laparoscopy was performed in all of them. All of the patients were followed for at least 12 months after the end of treatment, during which time they attempted to conceive. Ten (31.25%) from all 32 treated women became pregnant. The highest incidence of pregnancy was in the stage I (12.5%) and II (12.5%) (rAFS) of endometriosis, a half of them in stage III (6.25%) and no pregnancy was recorded in the stage IV found prior to the beginning of therapy. There was no statistically significant (chi 2-test) relationship between the reduction of laparoscopic findings (defined as a visual extermination or as a decrease in staging of endometriosis) and pregnancy. In 19 (59.4%) from 32 treated women the stage of endometriosis remained unchanged, in 9 (28.3%) it was reduced by one up to three stages and in 4 (12.5%) the endometriosis was visually eliminated. In no case the staging of endometriosis was worsened during therapy. Gestrinone was well tolerated. In no one of patients it was necessary to interrupt the treatment untimely.

[The effect of self-monitoring on perinatal outcome in insulin therapy of diabetic women during pregnancy].

The authors compare perinatal results in insulin treated diabetic women who were followed up in the out-patient department of the Second Gynaecological and Obstetric Clinic of the Masaryk University in Brno. In 1980-1984 148 pregnant diabetic women were followed up and in 1985-1991 when self-monitoring has become part of comprehensive treatment of pregnant diabetics in our department, 279 women were followed up. The patients were monitored during pregnancy by a diabetologist and gynaecologist. Other members of the team are a paediatrician, geneticist, psychologist, dietitian, and educational nurse. As to laboratory parameters, short-term and long-term compensation of diabetes were monitored. Systematic preconception preparation of diabetic patients, with an attempt to achieve normal blood sugar levels and genetic examination of both partners from part of comprehensive care. The results achieved in our department revealed a statistically significant decline of the incidence of foetopathies in neonates at the 5% level of significance and a decline of hypoglycaemia in foetuses at the 0.1% level.

[Prenatal preparation--a part of the care of young female diabetic patients].

In the Second Gynaecological and Obstetric Clinic in Brno there has been for more than 15 years a specialized department for treatment of diabetes during pregnancy. The department monitors and treats pregnant diabetics from approximately two thirds of the former South Moravian region, i.e. patients with diabetes assessed before pregnancy as well as those where an impaired carbohydrate metabolism was detected during pregnancy. The authors submit an account of the organization of team work in their out-patient department.

[Results of transvaginal ultrasound screening for endometrial carcinoma in women at risk after menopause].

Vaginal sonography was used as a prebioptic examination for screening of endometrial carcinoma in postmenopausal women in a risk group (diabetes, hypertension, obesity) without a history of haemorrhage. The examination was made in 104 women. In 23 patients the ultrasonic finding was suspect and subsequent abrasion revealed in three instances carcinoma of the endometrium (2.88%). The authors consider vaginal sonography as important prebioptic method of endometrial cancer in the risk group of women after the menopause.

[Delivery of neonates with a birth weight of less than 1500 grams. Perinatal results during a 6-year period and possibilities of affecting them].

The authors evaluated the perinatal mortality and condition on delivery of 167 neonates with a birth weight under 1500 g who were delivered and hospitalized at the 2nd Department of Gynecology and Obstetrics in Brno between Jan. 1, 1985 and Dec. 31, 1990. The perinatal mortality of neonates with a birth weight below 1000 g was 73.1%, of those with a birth weight 1000 g-1500 g it was 25.9%. Early neonatal mortality was not influenced by the patient's age exceeding 30 years, occupation, environment, status, number of interrupted pregnancies, spontaneous abortions and previous spontaneous and induced deliveries, administration of tocolysis, corticoids, the position of the foetus, transport in utero, way of delivery, indication for Caesarean section and sex. Early neonatal mortality was influenced by the mother's age less than 20 years, administration of antibiotics, multiple pregnancy, grade of asphyxia, presence of RDS, necessity of respiratory support, incidence of intracerebral haemorrhage and birth trauma. In the discussion the authors mention possible ways of reducing the perinatal morbidity and early neonatal morbidity in the mentioned range of birth weights.

[Assisted reproduction in the treatment of infertile marriages].

The authors investigated how treatment of infertile marriages is affected by the introduction of assisted reproduction. They compared the five-year period 1978-1982--485 couples with group II (1983-1987)--787 married couples. In group II successively all methods of assisted reproduction were introduced. The authors found that group II (1983-1987) had by 18.3% more deliveries, as compared with the first period. The increase is due to assisted reproduction in 10.6% and due to other therapeutic methods in 7.7%. The increase occurred in all investigated indication groups using methods of assisted reproduction within the range of 6.9-14.4%.

[Development and results of microsurgical sterility operations at the Gynecology and Obstetrics Clinics of the Brno Masaryk University].

Four periods of microsurgery of tubal sterility and present approach to individual operation techniques in respect to assisted reproduction are evaluated. The evaluation concerns 240 operated women, their average was 31.8 years, 68.8 per cent women were operated for the first time. The pregnancy rate arrived at 90 pregnancies (40.8 per cent), 80 women delivered (33.3 per cent), the abortion occurred in 7 cases (2.9 per cent) and ectopic pregnancies occurred in 11 cases (4.6 per cent). By adhesiolysis we achieved 56 per cent pregnancies, by fimbrioplastics 34 per cent pregnancies and by neostomies only 27 per cent pregnancies. Tubal anastomosis (11 operations) and tubal implantations (9 operations) had no significant influence upon evaluation. Deflection from tubal implantations, anastomoses (except refertilizing ones) or neostomies (especially iterative ones) in favour of in-vitro-fertilization is evident.

[Oocyte removal in conjunction with pelvic microsurgery].

The paper evaluates results of gamete transfer into the oviduct or embryos into the uterus in connection with pelvic microsurgery. Between August 1981-December 1989 225 microsurgical operations have been performed in a stimulated cycle. Ovaries have been stimulated by clomiphene-citrate or in combination with human menopausal gonadotropin. The operation was timed 34 hours after application of human chorionic gonadotropin. In a group of 156 women we recovered at least one oocyte. From 77 gamete transfers into the oviduct we achieved 11 pregnancies, i.e. 14.3 per cent. 46 embryo transfers into the uterus resulted in 5 pregnancies, i.e. 10.9 per cent. From the total number of 16 pregnancies we achieved 5 deliveries, 8 clinic abortions and 3 ectopic pregnancies. The connection of sterility microsurgery with oocyte retrieval increases the success rate of operations without another stress of a patient and has also its economic significance.
