[Traumatic lesions of the hand: narcissistic wound, functional and psychic spin, psychological management].

In our unit, the clinical psychological approach is a constant concern of the therapeutic team. Individual conversations allow verbalisation of personal ideas about the accident, the narcissistic wound, the idealisation of surgeons and surgery, guilt feelings, rehabilitation, the necessity for informations, ... for the discussion of these issues has therapeutic effects for the injured person and improves his or her capacities for reinvestment.

[Materno-fetal dengue. Apropos of 5 cases observed during the epidemic in Tahiti (1989)].

During the dengue 3 epidemic which occurred in French Polynesia between September 1989 and March 1990, number of cases of Dengue Haemorrhagic Fever (DHF) were reported among children and infants. Of interest, five cases of neonatal infections were recorded and involved the transmission of the virus from mother to child as the end of the pregnancy. Detailed observations on these cases are presented in this paper. Fever in mother, when delivering or just a few days before, represents the circumstances of the diagnosis. Analysis of the clinical features shows that fever and vasomotor troubles (blotches) were the dominating traits of the disease in the newborns. Also, hepatomegaly was constantly observed. Concerning the biological results, thrombopenia (platelets count: 100,000/mm3) was reported for all cases. According to these data, all cases were classified into DHF cases. Laboratory confirmation was obtained by serological investigations (antidengue IgM immunocapture ELISA and Haemagglutination Inhibition Tests) and/or virus isolation in both mother and child. Thus, virus was recovered in two newborns while IgM were detected in all infants and in four mothers. Severity of the disease was variable. However, out of five cases one fulfills the WHO definition of Dengue Shock Syndrome (DSC). Moreover, the authors discuss about the difficulties encountered to classify the cases according to WHO criteria for the grade classification of DHF.

[Neuromalaria and cytokines].

Levels of TNF alpha, IL-6-, soluble R IL-2, and fibronectin, were evaluated in fifteen patients with cerebral malaria. Relations between cytokines levels and parasitemia were assessed. Concentration of IL-6, and soluble R IL-2, correlated with parasitic density on admission. It was appeared, that IL-6, would be a prognostic factor, as interesting as TNF alpha.

[Treatment of hypercholesterolemic patients of different age with simvastatin: 1-year study].

Nowadays there is no direct evidence that lowering cholesterol may reduce the CHD in the elderly; however, several evidences suggest the potential benefit of a dietetic and/or pharmacologic intervention in healthy young-old hypercholesterolemics. In this study we evaluated the efficacy and tolerability of simvastatin, alone or in combination with cholestyramine, in two groups of patients (group A: age 30-55 years and group B: age 60-75 years) with primary hypercholesterolemia, over a 48-week period. In both groups Simvastatin induced a significant reduction in total and LDL cholesterol (p less than 0.001 both), with a decrease of the total/HDL cholesterol ratio (p less than 0.001). A reduction of the apo-B100 levels was also observed (respectively p less than 0.01 and p less than 0.001) with a significant improvement of the apo-B100/apo-A1 ratio (p less than 0.01). Simvastatin was well tolerated and free of remarkable side effects in both groups.

[Surgical technique aspects in multiorgan transplantation: personal experience].

The Authors report their experience with multiorgan transplantation performed in 3 patients affected by multifocal cancer of the liver, either primitive (2 cases) or secondary (1 case). The rationale for this new approach was the unfeasible single liver transplantation taking into account the extracapsular diffusion of the neoplasia.

[Acute intracranial subdural hematoma of arterial origin after spinal anesthesia].

A case of an acute intracranial subdural haematoma occurring shortly after spinal anaesthesia is reported. A 67 year old poorly controlled hypertensive man, ASA II, underwent removal of a prostatic adenoma under spinal anaesthesia. He complained of postural headache on the third day after surgery. Unresponsive to the usual analgesics, his headache became severe, persistent and non postural on the fifth day. Twenty-four hours later, he suddenly presented with a left hemiplegia and became comatose. Computed axial tomography showed a large left-sided subdural haematoma, lying over the left hemisphere. During the immediate surgical removal, a pulsatile arterial bleeding originating from a small cortical artery was discovered, and stopped. The patient slowly recovered consciousness, but the hemiplegia remained. He finally died six months later of bronchopneumonia. The link between the haematoma and the spinal anaesthetic is not proven; the possible relationship between the two is discussed.

[Transplantation of the pancreas: surgical aspects].

Based on their personal as well as international experiences, the authors describe different surgical techniques employed in pancreatic allograft related to different complications rate and patient and graft outcome. In fact, the surgical technique represents an only partially solved problem, because of the high percentage of complications dealing with the different approaches. Even though several procedures concerning either the site of implantation either the treatment of the exocrine function have been so far used, the results up to now obtained are the main limiting factor for a large clinical application of pancreatic transplantation.

[Spinal anesthesia with bupivacaine for surgery of the hip in the elderly].

In 60 elderly patients, spinal anaesthesia for orthopaedic hip surgery was induced randomly with 15 mg bupivacaine 0.375% without glucose (Group I), 2.5% glucose (Group II) or 7.5% glucose (Group III), in 4 ml. The injection was made in the lateral position, and the patients turned supine immediately after. The onset, extent and duration of sensory and motor blockade, the cardiovascular effects and the quality of anaesthesia were evaluated. The hyperbaric solutions produced a greater cephalad spread of analgesia (T8,9 and T9,8 respectively) than the glucose-free solution (T10,5). The mean duration of analgesia at the L2 level with the isobaric solution was significantly greater: 187 min versus 171 min and 150 min with the hyperbaric solutions. All the patients had complete motor blockade of the lower limb. The mean duration of the motor block was significantly shorter for the 2.5% and 7.5% glucose solutions (137 and 125 min respectively for Bromage's degree 3) versus 170 min for the glucose-free solution. Although the pre-anaesthetic arterial pressures in the three groups did not differ significantly, the systolic, mean and diastolic arterial pressures decreased only by about 20 to 25% in all groups. It was suggested to take into account the more rapid infusion of lactated Ringer's solution (20 ml X kg-1) in Group III. Anaesthesia was satisfactory in 95% of patients in Group I and Group II, and 90% in Group III. Glucose-free bupivacaine produced a long-lasting blockade suitable for hip surgery of long duration.

[Doppler ultrasound and strain gauge plethymography in the diagnosis of the post-phlebitic syndrome].

Certain non-invasive techniques like Doppler CW and strain gauge plethysmography are highly effective ways of evaluating the natural history of postphlebitic syndrome. 40 patients of both sexes, average age 51.5 +/- 6.9 with a history of deep venous thrombosis (28 documented phlebographically) were subjected to Doppler tests with assessment of venous blood pressure and bilateral strain gauge plethysmography. 34 of the 40 suspected PPS cases were confirmed, though not all cases were at the same stage. Persistent deep venous thrombosis was found in five of the extremities and the last one examined revealed a primary varicose syndrome. A comparison of the Doppler and phlebography results showed both to be highly sensitive techniques (100% accuracy). When the contralateral limbs were examined, the Doppler technique revealed 7 cases of PPS and 21 primary varices. In contrast strain gauge plethysmography identified all 28 cases of increased venous capacitance as primary varices, thus confirming the inability of this technique to distinguish between the various varicose conditions. Assuming the presence of a vascular diagnosis laboratory where both techniques are available, strain gauge plethysmography is recommended as the examination of choice. This technique is simple and fast to perform and can provide extensive information whether at rest (filling and emptying volumes and times; venous tone and distensibility, venous blood pressure at rest) or in movement (venous pressure when standing, muscular pumping index). Hence plethysmography can reveal any canalisation present even in the earliest stages though it cannot pinpoint the precise site of the deep obstruction. The longer, more complex Doppler CW procedure should be reserved for secondary investigations. This technique is preferable to plethysmography when a more accurate assessment of the degree, site and extension of the venous recanalisation is needed. Doppler CW also provides information on any valvar sequelae since it records the direction of the blood flow in the presence of a substitution syndrome (increased venous flow in the surface vessels). Finally if used in a rational manner the two techniques can be combined to eliminate contrast medium techniques, which would only be adopted as a preoperative measure.

[ADP-induced platelet aggregation in vascular diseases of the elderly].

A study of 60 over-65's, of whom 30 were suffering from coronary disease 15 had previous but now stable cerebrovascular insufficiency and 15 were suffering from circulation insufficiency in the lower extremities, showed that ADP-induced platelet aggregability was significantly increased by comparison with a group of 15 subjects of similar age rigidly selected as "normal". The aggregation rate was more enhanced than other parameters. In patients with coronary insufficiency and in those with cerebral vasculopathy, other aggregability parameters were significantly changed (minimum aggregating concentration). It is concluded that platelet aggregation, which is often increased in physiological ageing, is altered to an even greater extent in the vasculopathic elderly.
