12 cases of a second malignant neoplasm in patients with a systemic hematologic disease were for a 10-year-period among the patients of a hematologic clinic. Seven of these cases are reported in detail. The factors which lead to this phenomenon becoming more frequent are discussed.
For a five year period 235 patients with a malignant lymphoma were studied in clinical conditions. In 20 of them (8.9%) the central nervous system was affected: 13 patients showed signs of spinal cord compression, 5 patients were with meningosis and 2 patients had cerebral infiltrations. The meningeal affection was found in patients presenting certain risk factors: young age, packages of lymph nodes localized retroperitoneally, lymphomas with a high degree of malignancy, extranodal localization. The patients with meningosis have a short longevity which implies meningeal prophylaxis in the risk groups. The spinal cord compression as a complication of the lymphoma does not determine the prognosis. The prognosis is related to the spread of the lymphoma. The knowledge of the early manifestations of these complications and the timely treatment lead to a favourable result. A very good therapeutic result can be achieved by the combined treatment--polychemotherapy and intrathecal administration of a cytostatic (cyclophosphamide and/or methotrexate).
A very rare case of myelodysplastic syndrome of long duration transformed into acute leukemia is reported. The syndrome presented with refractory anemia with ring sideroblasts and after eight years of favourable course abruptly turned into acute myelomonoblastic leukemia with a rapid fatal outcome caused by gram-negative sepsis. The disappearance of the ring-sideroblasts from the bone marrow is pointed out as a bad prognostic sign. Treatment of the myelodysplastic syndrome with small doses of cytosine arabinoside is discussed as well as the nonobligatory passing through the stage of a gradual increase of the blast cells before the transformation into acute leukemia.
The authors present their studies on the cytologic and some immunologic features of leukemic cells which determine the specificity of lymphocyte population in the various forms of lymphatic leukemia. 133 patients who were treated in their clinic for the period 1980-1984 were studied. Four clinical forms were differentiated which had been published elsewhere. The morphologic and cytochemical differences of lymphatic leukemia cells in the different variants of the disease are pointed out. Applying the tests for rosette formation, blast transformation and immunofluorescence they have proved immunologic differences in the lymphocyte population of the different forms of the disease.
The study includes 133 patients with chronic lymphatic leukemia treated by the authors for a five year period (1980-1984). Data from the patients' history, status, blood picture, treatment and therapeutic efficacy are taken into consideration. The importance of splenomegaly is emphasized. Four clinical variants are differentiated: I. calm lymphatic leukemia; 2. secondary splenomegalic lymphatic leukemia; 3. primary splenomegalic lymphatic leukemia; 4. pure splenomegalic lymphatic leukemia. The study is supported by cytologic and immunologic investigations. This allows precision of the prognosis and of the therapeutic conduct.

Agenesis of the mesonephros was obtained by preventing its early induction by the Wolffian duct. In the older embryos (14 to 19 days) studied, the ovaries differentiated in the absence of the underlying mesonephros, but were reduced in size. Their different cell populations (somatic and germinal) were however present and showed typical cytological features. Nevertheless, both components of the ovary, the medulla and the cortex, demonstrated lack of harmonious development. The medulla contained much larger lacunae; the cortex was shrunken with less secondary cords. At these stages, the gonocytes are not yet in premeiosis. These results are discussed in relation to the role of the mesonephros in gonadogenesis and in initiation of meiosis.
Blast-transformation reaction of lymphocytes to non-specific stimulation with phytohemaglutinines was studies in 83 patients with rheumatoid arthritis with different activity of the general inflammatory reaction, stage and form of the disease. Lymphocytes react with diminished activity to that stimulation both in the early and late stages of the disease, being an index for the intransience of the conditions for antigen stimulation. The diminished lymphocyte reaction correlates with the clinical-laboratory indices for activity of rheumatoid arthritis. It raises in the course of the medicament tosus effect and reflects the effectiveness of treatment. The cellular immunity takes part in the pathogenesis of rheumatoid arthritis. Blast-transformation test could be used as an aide in the determination of rheumatoid arthritis activity as well as of the cellular immunity state.

Phytohemagglutinin (PHA) that stimulates the lymphocytes to blast transformation in conditions of cellular cultures provokes local skin reaction, when applied intradermally. At the beginning that reaction is manifested with hypermia at the place of the application and later an infiltration is formed. In healthy subjects the infliltration reaches maximal dimensions on the 48th to the 72nd hour post PHA injection. The presence of lymphocytes was established with the cytologic examination of the material from that infilitration at the 24th hour, lymphoblasts and hyperbasophilic blast cells at the 48th and 72nd hour and single reactive lymphocytes were observed at the 96th hour. The infiltration appears later in patients with quiet development of chronic leukosis, only after the 96th hour and is with smaller demensions. Negligible infiltration is observed in malignant leukosis, its progress in time does not differ from that in healthy subjects. However, skin reaction in intraderma injection of PHA gives a certain orientation about the immune organism reactivity and in case of 1 chronic leaukosis--about the form and severity of the disease.
A considerable damage of immunogenesis with involving both humoral and cellular immunity was recently established in certain forms of chronic hepatitis. The investigation of nonspecific cellular immunity of following-up the transformation activity of lymphocytes in vitro in stimulation with PHA provides data for the inhibition of that immunity in chronic agressive hepatitis with manifestations of hypersplenia. In the progress of the disease, the increase or decrease of the transformation activity of lymphocytes correspond to the improvement or deterioration of the disease. After splenectomy transformation activity of lymphocytes is increased parallelly with the favourable effect as regards the basic ailment and the hematological indices.
The author examined the enzymic content of lymphocytes and the changes, which occurred after stimulation with phytohemaglutinins (PHA). There was a difference in the reaction of lymphocytes in healthy persons, in patients with virus diseases and in persons with malignant lympholeucosis.
