Promotion of safer sexual behaviour and HIV/STD prevention among adolescent students and army recruits.

Estonia is a country with a low incidence rate of HIV infection, but a high rate of STDs. Drug use is increasing among young people, with an unknown number of users. A considerable number of IVDUs are infected with hepatitis B and C viruses. There are conditions for rapid spread of HIV infection in Estonia. From 1997 to 1999, three prevention projects were carried out with young people by the Estonian Association Anti-AIDS. The idea of preventive strategies was to promote the young people's own decision making about risk behaviour. Interactive learning methods were designed for work with young people. Teachers and peer educators were seen as collaborators in prevention: for them an innovative methodological manual was issued and they were also provided with training sessions. During interactive workshops the project staff learnt from young people background issues such as aspects of their psychosocial and sexual development. From our experience, prevention must be carried out among the young people continuously as they need the development of the motivation and personal insight to establish the goals of personal (and sexual) health.

HIV-infection in Estonia.

Only a few cases of clinical AIDS have been diagnosed in the Baltic countries that previously were part of the Soviet Union. This suggests that the spread of HIV-infection has been much slower than in several other countries belonging to the previous Eastern Europe. To get a more precise picture of the situation, we have analyzed the data from seroepidemiological screening programs that have been conducted in Estonia since June 1987. Large population groups were tested according to a decree by the former soviet All-Union center on AIDS. On several occasions the tests were mandatory and probably had a coverage close to 100%. After more than a million serum samples were tested, 29 HIV-infected persons, 3 women and 26 men have been identified. One of them was HIV-2 positive. Most of them live in Tallinn, the capital city of Estonia. All cases probably have a sexually transmitted infection. In one the infection has proceeded to clinical AIDS. Analysis of the gene sequences of several strains suggests that the strains are closely related and that the number of sources of infection is quite small. The study suggests that the spread of HIV in Estonia is still quite limited. A contributing factor may be the restricted possibilities for travel to the Western countries during the Soviet era.

An electroaerosol device for group vaccination against influenza.

With the aid of an electroaerosol device a group of persons were vaccinated by inhalation of influenza vaccine. In a first trial 76 persons with a low antibody level (1 : 20 and less) were vaccinated against A2/Victoria/1/72 and B/USSR/69: Antibodies were reexamined 30 days later, Interferon at the 3rd and 10th day. This kind of vaccination caused an antibody increase statistically significant. In the course of an influenza epidemic 2,300 persons vaccinated by inhalation were compared with 2,195 not vaccinated. Among the persons vaccinated there were 13.7 +/- 0.7% diseases, among the non vaccinated persons 26.3 +/- 0.9%.

Vaccination by the aerosol method.

After a review on vaccination by inhalation of aerosols the authors describe the group vaccination with the aid of an electroaerosol generator. 8 persons may be vaccinated simultaneously in a very short time. The vaccination by inhalation is without any hazards. The viability of the microorganisms in the aerosol is preserved. Interferon inhalation seems to reduce symptoms of intoxication in the first days after outset of influenza or acute airway infection. The efficiency of vaccination through aerosol inhalations against bacterial infections demands further researchs.
