[Control of breathing in patients with mechanical disorders of the lung].

22 normal volunteers, 57 patients with obstructive changes and 34 with restrictive type changes were studied. The following parameters were analyzed: maximal inspiratory and expiratory pressures, occlusion pressure and components of the breathing pattern. The patients were divided according to the degree of airway resistance and fall of vital capacity. All patients compared with the healthy volunteers demonstrated higher minute ventilation, and a higher neuro-muscular respiratory drive as shown by the VT/Ti index and occlusion pressure. The breathing pattern did not correlate significantly with the different abnormalities of respiratory function in the studied groups. In the patients with obstructive changes a weak correlation was found between the occlusion pressure and airway resistance. In patients with restrictive type changes a weak correlation between occlusion pressure, static and specific compliance was found. The occlusion pressure was a more specific parameter characterizing respiratory drive compared with the VT/Ti index. Disturbances of pulmonary mechanics have a small but significant effect on respiratory control.

[The frequency and course of fibrotic changes in patients with sarcoidosis].

Basing on observations of 960 patients with pulmonary sarcoidosis the authors found fibrotic changes of pulmonary parenchyma in only a few, the incidence being 3.2. Improvement in radiological examination following steroid therapy was seen in 50%, of respiratory function in 30%. Although both examinations do not fully correlate with each other, they are however supplementary with each other, and should be regarded as important criteria of functional involvement and therapy outcome.

Changes in respiratory muscle function after neostigmine injection in patients with myasthenia gravis.

In sixteen patients with myasthenia gravis, the weakness of respiratory muscles, especially of expiratory, caused some disturbances in lung function. Thirty minutes after injection of neostigmine, general improvement of muscle function was accompanied by an increase of strength and endurance of respiratory muscles. The mean value of maximal inspiratory mouth pressure (PImax) increased by 33%, maximal expiratory mouth pressure (PEmax) by 23.5% and maximal voluntary ventilation (MVV) by 21%. As a consequence of these changes amelioration of lung function indices was found, vital capacity increasing on average by 13% and residual volume decreasing by 12.5%. We concluded that patients with generalized myasthenia gravis have disturbances in lung function which may be partially improved by neostigmine injection or other anticholinesterase agents.

Evaluation of mixed venous oxygenation on the basis of arterial oxygen tension in chronic lung diseases.

The mixed venous oxygenation in relation to arterial oxygen tension was evaluated in the control group (30 subjects) and in patients with chronic obstructive lung disease (COLD; 56 subjects) and fibrosing alveolitis (FA; 20 subjects). The mixed venous hypoxia was found in 28.4% of patients with COLD and in 40% of patients with FA. The arterial oxygen tension values below which mixed venous hypoxia can be expected were below 55 mm Hg in normocapnic and below 46 mm Hg in hypercapnic patients. The values of mixed venous oxygen tension correlated significantly with the coefficient of oxygen delivery in the control group (r = 0.84; p less than 0.001) and in all patients taken together (r = 0.69; p less than 0.001). The finding of mixed venous hypoxia, which is an approximate index of tissue oxygenation, might have some clinical implications.

Small airways function in pulmonary sarcoidosis.

Closing volume and maximal expiratory flow-volume curves were performed in a group of 25 patients with pulmonary sarcoidosis in stages I and II and with normal spirographic results. Although the mean values of these tests were mostly within the limits of predicted values, 48% of the patients had an increased slope of the alveolar plateau and 40% had decreased maximal expiratory flow at low lung volumes. Abnormal test results were found even in patients with normal static lung compliance. Our results suggest that functional abnormalities in small airways may occur early in the course of sarcoidosis.
