[Leiomyosarcoma within a bladder diverticulum].
Sarcomas are a rare entity among malignant tumours of the bladder. This paper presents the case of a male patient with leiomyosarcoma within a vesical diverticulum. The approach undertaken was diverticulectomy and, since the tumour was limited to the diverticulum and no metastatic disease was present, no other adjuvant therapy was considered.
[Infantile enuresis: epidemiologic, clinical, urodynamic, and radiologic data of a consecutive series of 100 cases].
The present paper reports the epidemiological, clinical, urodynamic and cystographic data of 100 consecutive patients (mean age 9.59 years) with nocturnal enuresis. These patients were divided into three groups: group I (4 to 7 years old), 23 patients; group II (8 to 11 years old), 51 patients; and group III (12 to 14 years old), 26 patients. Nocturnal enuresis was more frequent in group II (51%). It was more prevalent in males (51%) of groups I and II. Isolated nocturnal enuresis was more frequent (54%) than nocturnal enuresis associated with diurnal symptoms (46%) in all age groups. Urinary infection was observed in 16% and was more prevalent in girls (68.7%). Detrusor instability was the most common urodynamic finding (40%), particularly in group II and girls. Uncoordinated voiding was the second most common urodynamic finding (24%), particularly in group II and girls. Lower urinary tract infection was observed in 1% (males). No abnormal urodynamic data were observed in 19%. Neurogenic bladder was observed in 16%, mainly in group II (31%). Vesicoureteral reflux was detected in 18%, mainly in girls (66.6%) and a postvoiding residual urine in 10%. A diagnostic and prognostic system and a therapeutic strategy based on our data are proposed.
[Quantitative determination of epidermal growth factor urothelial receptor (EGFR) in superficial and invasive bladder carcinoma].
The epidermic growth factor (EGF) is a polypeptide which stimulates tissue proliferation. The mechanism of action takes place through an specific membrane receptor, known as the EGFR. Recent scientific contributions have allowed to know its implication in various tumoral processes (breast, ovary, bladder, etc), so that its expression may be used as a major prognostic factor. The objective of this work is to quantify and analyze the epidermic growth factor receptor (EGFR) in surface and invasive vesical carcinoma. To this end, 43 tissular samples divided in two groups were studied. The groups were: 1) Group 1 or control group, comprising 14 samples of "control" vesico-urothelial tissue, and 2) Group 2, assembling 29 patients with vesical carcinoma (17 surface and 12 infiltrant). All tissue samples underwent a process of homogenization and subsequent determination of membrane EGFR by means of radioimmunoassay (EGFR-Receptor Assay, Vienna Lab, Labordiagnostika GmbH). Our results demonstrate the expression of EGFR in both control and tumoral vesico-urothelial tissue in 100% of cases, with detection of significantly higher concentrations (p) in samples from vesical carcinoma than in those from control specimens (15.24 vs. 5.02 fmol/mg protein) and higher levels in infiltrant rather than in surface vesical carcinomas (18.92 vs. 11.4 fmol/mg protein).
[Determination of serum SCC antigen in transitional cell carcinoma of the bladder].
The SCC antigen expresses in the squamous epithelium during the process of a neoplastic transformation. This paper's objective is to evaluate the biological behaviour of SCC antigen in surface, deep, localized and spread malignant vesical carcinoma. To this end, 100 patients divided in two groups were studied. The first group consisted in 30 healthy subjects strictly selected and the second group comprised 70 patients diagnosed with transitional cell vesical carcinoma. SCC Antigen measurement was made by Radioimmunoassay (RIA, Abbott). Our results suggest that this tumoral antigen has no use as a prognostic factor in patients diagnosed with transitional cells vesical carcinoma, since serum concentrations suffer no change in relation to size, extent and degree of tumoral differentiation.
[True post-micturition urinary incontinence of the male].
Herein we describe 3 cases of male urinary incontinence that had consulted for intense dribbling at the end of voiding. The urodynamic study identified the true postmicturition urinary incontinence in the male, whose origin was ascribed to alterations in the terminal phase of voiding, arising from localized disorders in the posterior or anterior urethra. The major significance of the true postmicturition urinary incontinence in the male lies in the need to distinguish it from other types of urinary incontinence with more clinical importance.
[Renal injury on a pathological kidney. Incidence, management, and results of treatment].
The present study reports our experience in the management of 13 patients with trauma to previously asymptomatic pathological kidney (11.6% of 112 cases of renal trauma treated over the period 1980-1990). The diagnostic difficulties and treatments, which are unlike those of trauma to normal kidney, are discussed and the literature reviewed. Hydronephrosis from ureteropelvic junction stenosis was the preexisting pathology that was frequently diagnosed (41.6%), followed by trauma to kidney with tumor (30.7%). Twelve (92%) patients underwent surgery: 2 (15.3%) were emergency and the rest were elective surgery. One patient with hilar sinus cysts received medical treatment. Conservative surgery was performed in 5 renal units (1 with bilateral involvement) and nephrectomy was performed in 8 patients (65.1%). The kidney with a tumor is more compromised by the renal injury. Furthermore, post trauma contusion/laceration of the tumor may present problems relative to tumor stage.

[The clinical and urodynamic aspects of enteroplasty].
A study was conducted in 24 patients to determine the usefulness of the intestinal segments used in bladder replacement surgery. Continence and voiding were analyzed, particularly the more important functional symptoms of the lower urinary tract and the urodynamic behaviour of the intestinal reservoirs/conduits. The Bricker ileal conduits (8 cases) showed poor accommodation during the filling phase, making its use as a reservoir little advisable. In general, bladder augmentation (9 cases) with intestinal segments were shown to be useful reservoirs, but of limited capacity for voiding. The ileal reservoirs (7 cases) used for bladder substitution had, in general, compensated voiding, but with defects in the filling phase.
