[MRT assessment of metabolic and thrombolytic therapy effects on postinfarction left ventricular remodeling].

AIM: To examine effects of trimetasidine on morphofunctional indices of the left ventricle (LV) in myocardial infarction (MI) patients on combined treatment. MATERIALS AND METHODS: Seventy five patients with primary macrofocal MI were randomized into 2 groups. Patients of group 1 (n = 38) received a combination of bisoprolol (beta-blocker) with enalapril (ACE inhibitor) in individual doses under control of blood pressure and blood creatinine level. Group 2 (n = 37) patients received the same combination of drugs and, in addition, trimetasidine in a dose 70 mg/day from postmyocardial day 7-10 for 6 months. Two subgroups from the groups were given systemic thrombolytic therapy (STLT) with streptokinase. MRT and cine-MRT of the heart were made for measurement of LV morphofunctional parameters. RESULTS: Low-field MRT of the heart in MI patients treated with adjuvant STLT (1500000 U within 6 hours after MI onset) and trimetasidine (preductal MB) in a dose 70 mg/day from the disease day 7-10 registered a significant inhibition of pathological LV postinfarction remodeling: a decrease of body surface indexed LV end diastolic volume by 10.3%, systolic volume--by 15.4%, LV myocardial tension--by 14.0%, sphericity index--by 7.1%; an increase in the index of relative wall thickness by 5.3%, cardiac index--by 9.2% compared to the group treated without trimetasidine. CONCLUSION: 6-month therapy with trimetasidine of MI patients leads to a significant regress of morphofunctional changes accompanying LV remodeling. Pathological LV postinfarction remodeling inhibits significantly in MI patients combined treatment of whom included STLT (streptokinase in a dose 1500,000 U within 6 hours after acute MI onset and trimetasidine in a dose 70 mg/day on postmyocardial infarction day 7-10).

[Trimetazidine effects on postinfarction remodeling of the left ventricle].

AIM: To study effects of combined therapy including trimetazidine on morphofunctional indices of the left ventricle (LV). MATERIAL AND METHODS: Seventy five patients with primary macrofocal myocardial infarction (MI) were randomized into two groups: 38 patients of group 1 received combination of bisoprolol, a beta-adrenoblocker, and analapril, an ACE inhibitor, in individually adjusted dose under control of blood pressure (BP) and blood creatinine; 37 patients of group 2 received the same combination and trimetazidine in a dose 70 mg/day for 6 months beginning on postinfarction day 7-10. Each group contained a subgroup treated with systemic thrombolytic streptokinase at admission. MR imaging (MRI) and cine-MRI assessed morphofunctional L V parameters. RESULTS: A 6-month therapy with trimetazine resulted in a significant regress of the morphofunctional alterations associated with LV remodeling in postmyocardial infarction patients. CONCLUSION: Combined treatment including systemic thrombolysis with streptokinase within MI hours 0-6, trimetasidine on MI day 7-10 plus 6 months significantly inhibits pathological postinfarction LV remodeling.

[Dynamics of lipid peroxidation and antioxidant defense in response to dalteparin therapy in acute coronary syndrome].

The aim of the study was examination of lipid peroxidation (LPO) and antioxidant system (AOS) in patients with acute coronary syndrome (ACS); evaluation of the effect of dalteparin therapy on LPO and ACS. Fifty one patients (mean age 57.5 +/- 1.5 years) were randomized into two groups: group 1 (n = 31) received low-molecular heparin dalteparin, group 2 (n = 17) was given non-fractionated heparin. The course of the anticoagulant therapy lasted 8 days. LPO and antioxidant defense in plasm and blood red cells were investigated on day 1 and 9. ACS patients appeared to have elevated LPO products production both in plasm and blood red cells, low plasm antioxidant activity (AOA). Dalteparin therapy lowered content of malonic dialdehyde (MD) in red cells and plasm, AOA enhanced. Heparin activity was lower: MDA in plasm decreased in a lesser degree while MDA in red cells continued to rise in lowering AOA in plasm and inhibition of catalase.

[Clinical and prognostic significance of the degree of delayed ventricular function and vegetative regulation of cardiac rhythm in myocardial infarction patients].

Ventricular tachyarrhythmias (VT) are responsible for many sudden deaths in postmyocardial infarction (PMI) patients. The aim of the study was to investigate clinical and prognostic significance for such patients of delayed ventricular activity and vegetative regulation of cardiac rhythm. Signal-averaged ECG, mathematical analysis of cardiac rhythm with measurement of variation range, mode, index of regulatory systems tension, 24-h ECG monitoring were made in 90 patients with acute large-focal myocardial infarction (MI) on the disease day 1, 15, 40 and 365. Signal-averaged ECG registered on MI day 15 was found most informative for identifying the risk to develop VT. Combined application of diagnostic techniques brings more information than each of them alone.

[Use of spirometry and bicycle ergometry in the assessment of the stability of mitral valve bioprosthesis].

Muscular and cardiac performance was assessed a spirometry and bicycle ergometry in two groups of patients aged 25-55 with mitral xenobioprostheses functioning normally. 36 patients were examined 1-3 years following surgery (group I), 23 patients in 3.5-8 years. Functional results were found not inferior in group II. Stable parameters were more characteristic for patients having preoperative functional classes I-III.

[10 years' experience of heart valve bioprostheses. Present status and future development].

Long-term results of mitral valve replacement with bioprostheses of three original designs are overviewed. Refinement of the supporting framework design, and of modelling and conservation methodologies has made it possible to improve the survival rates and provided for the absence of specific complications in two groups of patients. Experimental evidence allows for anticipating an increase in the terms of bioprostheses performance.

[Quantitative analysis of hemodynamic disorders in coronary occlusion based on the roentgeno-cinematographic data].

The contrasting time of the coronary arteries in 4 types of atherosclerotic lesions was determined by means of quantitative analysis of the coronarograms. The contrasting time was found to be dependent both on the degree of the stenosis and on the condition of the distal segment of the coronary artery. The condition of the drainage tracts had a more marked effect on the contrasting time. The coronary arteries contrasting time is considered as an index of peripheral resistance in the studied segment.

[Experience in the perfecting of direct myocardial revascularization inpatients with ischemic heart disease].

The first 100 operations for aortocoronary and mammary-coronary shunting in patients with ischemic heart disease are analysed. The operations were carried out on patients aged from 36 to 59 years who had no severe concomitant diseases. Angina pectoris of effort was encountered in 47%, angina of effort and of rest in 33%, and unstable angina in 20% of patients. The operations were performed under conditions of extracorporeal circulation. In 100 patients 184 arteries were shunted (1.8 shunt per one patient). A mammary-coronary shunt was established in 13 patients; the left internal thoracic artery was used in all of them for shunting the anterior interventricular branch. Improvement was recorded in 88.5% of patients after the operation. Thirteen (13.0%) patients died. It is concluded that patients should be chosen with great care for aortocoronary shunting with due regard for the risk factors.

[Characteristics of the epidemic process in pyoseptic infections in a heart surgery hospital].

The authors conducted an epidemiologic analysis of hospital pyoseptic infections on the basis of study of the outcomes of 1,517 operations on the heart and the results of bacteriologic tests. A high frequency of secondary infectious endocarditis in acquired heart diseases was revealed, which allows operations in this group of patients to be related to the category of "conventionally clean" and shows that the proper antiseptic regimen must be organized during the operation without fail. The valves of the heart in 59.3% of patients with acquired heart diseases are affected with the causative agents of hospital pyoseptic infections. The epidemic process is characterized by high intensity of manifestations, dependence on the degree of aggression and invasion of the therapeutic and diagnostic process, the type of the operative intervention, and polymorphism of the clinical manifestations and the etiological structure. The process is of the intermittent dynamic type and is marked by a tendency towards growth of morbidity and mortality with prevalence of an endogenous mechanism of contamination. The main sources and routes of transmission of infection are shown.

[Possibilities of improving the methods of making vascular prostheses using human umbilical veins].

In the experiment, the possibilities for intensification and acceleration of the umbilical vein tissue stabilization, increasing the strength and improving the plastic properties, increasing the resistance of biological grafts to thrombosis due to the use of the improved method of conservation according to the scheme: dynamic stabilization in the glutaraldehyde-enzyme-glutaraldehyde-heparin solution are studied. The developed biological grafts were used in 12 patients with occlusion of the arteries of the femoropopliteal segment. After 18 mos, the grafts are patent in 8 patients.

[Evaluation of the function of mitral valve prosthesis by non-invasive methods].

The study was undertaken to examine the correlation between the parameters of central hemodynamics and mitral valve prosthesis functions which were obtained by invasive techniques, as well as these of physical working capacity, hemodynamics, and blood by noninvasive methods at rest and during bicycle ergometry in the graded exercise test. An examination was made of 24 patients every 0.5-8 years (mean 3.2 years) after replacement of mitral valves by xenobioprostheses. "Oxygen pulse" and cardiac index defined during exercise highly correlated with prosthetic mid-diastolic pressure gradients. A moderate correlation was found between dry erythrocytic mass and total pulmonary blood flow resistance. The findings may be useful for the early diagnosis of prosthetic dysfunctions.

[Ways to prevent the calcification of heart valve bioprostheses].

The influence of the method of preservation and application of medicinal preparation xydiphone on the process of calcification of xenograft bioprostheses in a classic model of subcutaneous implantation of bioprosthesis cusps in rats has been studied. It is estimated that fermentative processing does not influence the intensity of calcification, while the immobilization of heparin to tissue decreases the amount of calcium in implants 4.5 times. Combination of heparin immobilization with peroral administration of xydiphone results in summing up of the effects, decreasing the amount of calcium in experimental set of cusps up to the level of nonimplanted ones.

[Morphology of heterologous aortic bioprostheses intended for clinical use].

Results of an experiment on the preservation of bioprostheses prepared from porcine aortal valves are presented. It is described how the structure of the cusps is affected by autolytic processes, by exposure to various concentrations of glutaraldehyde, and by sequential exposure to glutaraldehyde and papain solutions. Xenogeneic aortal complexes must be processed within the first 24 hours after collection since later on they undergo autolysis whereby the fibroblasts are destroyed and collagen fibrils become damaged. Low glutaraldehyde concentrations preserve the structure of these well enough, although concentrations below 0.5% were found to make the ground substance edematous and cause structural damage to the fibroblasts. Papain treatment without glutaraldehyde pretreatment leads to profound changes in the fibrous structures and in the cells, whereas glutaraldehyde fixation followed by papain treatment assures adequate preservation.

[Hemodynamic results of replacing the mitral valve with an aortic xenobioprosthesis].

Results of an analysis of clinico-hemodynamic data obtained in 66 patients with bioprosthesis of the mitral valve are presented. The data of intraoperative examination as well as of catheterization of heart cavities within the period from 6 to 62 months after operation show the xenoaortic bioprosthesis to give an adequate correction of the mitral valve defect, their implantation was followed by a substantial improvement of the patients's state.

[X-ray evaluation of the results of mitral valve bioprostheses].

An analysis of results of investigations performed in 66 patients has shown that roentgenometric examinations facilitate obtaining an objective estimation of the dynamics of postoperative period in patients with bioprostheses of the mitral valve. After performing such operations many roentgenological signs of the defect undergo involution, the duration of the process being determined by the character of the defect and the initial state of the myocardium. The dynamics of volumetric alterations of the heart were shown to reflect specific functioning of the bioprosthesis.

[Effect of glutaraldehyde preservation on the immunogenic, physicomechanical and functional parameters of aortic valve xenograft bioprostheses].

It was shown that during conservation of heart valve xenograft bioprostheses, with glutaraldehyde (GA) the reagent penetrated the tissue from a 0.625% solution in a greater amount than from 0.25 and 1% solutions. The treatment with a 0.625% solution of GA essentially decreased the sensitizing and anaphylactic activity of the prosthetic tissue and contributed to the tissue inertness in radial immunodiffusion with antiserum of immunized animals. Subsequent treatment of the tissue with a 4% GA promoted a further decrease of immunogenic properties of the xenogeneic tissue. GA conservation increased durability and limited pliability, which led to a mild stenotic effect revealed during prosthesis functioning in vitro. The optimal method of the conservation of the aortal bioprosthesis consists in its 4-week treatment with 0.625% GA and a three-fold change of the solutions followed by storage of the tissue in 4% formaldehyde.
