Quantitative morphodynamics of endothelial cells within confluent cultures in response to fluid shear stress.

To evaluate shear stress-induced effects on cultured cells we have extended the mechanical setup of a multichannel in vitro rheological system and developed software allowing entire processing control and image data analysis. The values of cell motility, degree of orientation (alignment), and cell elongation were correlated as a function of time (morphodynamics). Collective and individual endothelial cells within confluent cultures displayed a shear stress-dependent characteristic phase behavior of the following time course: resting conditions (phase I), change of motility (phase II), onset of alignment (phase III), and finally cell elongation (phase IV). Especially cell motility was characterized by a randomized zigzag movement around mean trajectories (fluctuations) together with mean cell locomotion. Onset of shear stress caused a down-regulation of fluctuations of 30% within <10 min and simultaneously increased locomotion velocities preferring the flow direction (phase II). After a lag period of 10 to 20 min cells orientated in the direction of flow (phase III) without significant cell elongation, which finally occurs within hours (phase IV). These data provide first evidence that cells within confluent endothelial monolayers respond to shear stress with a characteristic phase behavior.

Idiopathic coronary artery dissection: a rare in vivo diagnosis.

A 42-year-old white female, who had an acute myocardial infarction, developed postinfarction angina and underwent coronary angiography seven weeks after the event. Coronary angiogram demonstrated a primary dissection of the left anterior descending artery. To our knowledge this is the sixth such report of a patient with primary dissection of a coronary artery demonstrated by angiography and the fourth such patient reported to achieve and maintain asymptomatic status without operative intervention.

[A simple electronic delay line that facilitates the incomplete display of spontaneous bioelectric signals on an oscilloscope].

A simple electronic delay line is described which is appropriate to delay bioelectric events like action potentials of neurons up to 100 ms. Thus, the device allows to use untriggered potentials as trigger signals and thereafter to display the delayed events including possible prepotentials in toto on an oscilloscope. The delay line mainly consists on charge coupled devices (CCD) in which samples of potentials to be delayed are transported like in "bucket-chain". Thereby the delay time at a given number of CCD elements only depends on the frequency of a clock which drives the chain. As long as action potentials and other bioelectric events have to be delayed up to ca. 0,1 s this device may be used as an alternative to transient recorders or other apparatus which are far more expensive than the presented delay line.

The serum digoxin test and digoxin toxicity: a Bayesian approach to decision making.

The clinician may often be uncertain about the presence of digoxin toxicity. This uncertainty is particularly important when the clinician must make initial therapeutic decisions about continuing or discontinuing digoxin. We describe a method that helps to clarify the role of the serum digoxin test in decreasing the uncertainty surrounding the diagnosis and treatment of toxicity. The relation between the test and toxicity was first determined in our patient population. An approach to the interpretation of the test based on the likelihood ratio was then developed by combining our data with selected data from the literature. The relation between the pretest risk of toxicity (the estimated risk of toxicity in the population under investigation before the test result is known) and the predictive value of the test was established. This relation was also used to analyze the importance of the degree of elevation of the test. The appropriate threshold probability for institution of treatment of toxicity was then determined by an interview technique. The test was able to make the patient's probability of toxicity cross the threshold probability for treatment of toxicity for an intermediate range of pretest risk. Our analysis suggests that the serum digoxin test may have a critical effect on therapeutic decisions and can be best considered as contributing to the spectrum of risk.

Transmural myocardial infarction during pregnancy.

Two young pregnant women with no known risk factors had a transmural myocardial infarction while they were lying in the supine position. Coronary arteriograms 3 1/2 and 4 months later, respectively, were normal. Coronary arterial spasm related to renin release from the transiently ischemic chorion is the proposed cause.
