[Pelvic fractures of seat belted car occupants].

From November 1974 to June 1978 60 severely injured car-occupants wearing seatbelt were treated at the department of surgery, University of Kiel. Ten of them sustained pelvic fractures. The cause of the injury was a direct pelvic trauma from the crashed car in 6 passengers. The maximum load on the pelvis was exceeded by the seatbelt during the delay of the car only in 4 patients. Contrary to the trauma by the lap-belt with abdominal-, lumbar spine, and pelvic injuries (seatbelt-syndrome by Garrett and Braunstein [5]) we found car-occupants with a threepoint-seatbelt injuries of the thorax, of the head with facial fractures and rarely with cerebral trauma, and of the legs with tarsal fractures. In our cases and in the literature pelvic fractures of car-passengers, who are wearing threepoint-seatbelts, are rare.

[Residual lesions after serial fracture of ribs (author's transl)].

Follow-up examinations at regular intervals to recognize trauma-induced lesions are as essential for persons who have suffered a chest trauma as they are for cases of injury to the limbs. A classification into three groups according to the number of broken ribs relates satisfactorily with thoraco-pulmonary complications and residual damage. Injuries in group 1 leave practically no permanent damage; in group 2 they are apt to be followed by residual respiratory impairment while in group 3 the likelihood of permanent impairment of lung function is very high. Spirometry, roentgenography and fluoroscopy, and lung scanning are of special diagnostic value in assessing late sequelae of multiple rib fractures. Gas analyses are of little value. They would provide more accurate information if performed during exercise under standard conditions.

[Surgical problems of iatrogenic esophagus perforations (author's transl)].

With increase of invasive investigations of the upper digestive tract an increase of complications is also expected. From 1968--1977 eleven patients with iatrogenic esophagus perforation were referred to the department of general surgery, University of Kiel. Symptoms, diagnosis and different treatment, depending on localization and time of perforation and underlying disease are described.

Operative treatment of prolapse of the pars membranacea of the trachea.

A new method of operation for stiffening the tracheal back wall in cases of prolapse of the pars membranacea is reported; it has been applied with success on 22 cases. The advantages of this technically demanding operation starting from the neck over the method using transthoracic access with insertion of bone chips, fascia graft, or plastic prostheses are discussed.

[Free perforation of stomach and duodenum (author's transl)].

206 perforations of stomach or duodenal ulcers were treated by primary resection or simple suture between 1963 and 1972. The lethality of these two methods of treatment was compared with the age of the patients, the duration of symptoms, the length of time between perforation and surgery, and the localization of the perforated ulcer. Our analysis showed that, as a rule, primary resection is preferable, except in old age.
