[Possibility of renal vascularization by means of a jejunal flap].

An animal experimental model for the additional vascularisation of the kidney is represented by means of an excluded demucosed small intestine segment. The immediate clinical signification is to be seen in lowering the hypertension in case of pyelonephritic renal shrinkage. Histologic investigations do not show any vascular communication between intestine and kidney and not any ingrowing of intestinal vessels in the kidney. If the improvement of reno-intestinal shunts could be particularly stimulated in case of a primary restricted blood offer must remain unsolved.

[Diffusion in fibrin by simultaneous fibrinolysis].

UNLABELLED: The distribution kinetics of radioactive 51Cr-EDTA between human plasma and a fibrin coagulation dissolving under the influence of different concentrations of streptokinase is measured by scanning the diffusion tubes point by point. RESULTS: 1. The dissolving rate of the coagulum is temporally constant in great concentrations of streptokinase. 2. The faster the coagulation dissolves, the smaller will be the amount of 51Cr-EDTA diffusing into the coagulum. The analysis of these findings and further calculations confirm the assumption made previously that the diffusion of streptokinase or streptokinase-activator complexes into the coagulum seems to play no essential role in the dissolving rate of the coagulum observed clinically and experimentally.

[In vitro measurement of fibrinolysis by urokinase].

The kinetics of the dissolution of fibrin clots caused by urokinase is measured and compared with those tests performed with streptokinase. Under test conditions, urokinase is being de-activated after some hours. streptokinase, on the other hand, will remaIn efficient for many days.

[Blood loss in dialysis in repeatedly used capillary dialysators].

The blood losses increase to the twofold to threefold by capillary dialysators C-DAK 4 and C-DAK 5 are about 6.8 ml and 26 ml, respectively, in the first use. In repeated use of the C-DAK 4 the blood losses by the C-DAK 5, however, are so high that it is not to be advised to use them manifold under our conditions of dialysis. By an increase of the reperfusion quantity from 150 to 250 ml of electrolyte solution for one C-DAK 4 blood losses may considerably be reduced. In a parallel switching of two C-DAK 4 each exemplar should be perfused individually.

[Intensified therapy of newly detected maturity onset diabetes].

In a prospective study 150 newly detected maturity onset-diabetics were randomized in 2 biostatistically comparable groups and underwent a treatment of different intensity. While the patients of the control group were treated according to the routine method used up to now in the dispensary for diabetics, in the intervention group an intensifying of the therapy took place, taking particular into consideration the body weight as well as the carbohydrate and fat metabolism. The decrease of body weight achieved by dietary intensive care proved to be the decisive factor for the tendency towards normalisation of glucose tolerance, hyperlipoproteinaemia and IRI-secretion, which could be registered in the intervention group after 2 years of observation. Following the preceding strong phase of diet, by Biguanides and Clofibrate a further significant improvement of the carbohydrate and fat metabolism could not be achieved. The decisive reserve in the treatment of obese maturity onset-diabetes could be seen in a permanent and continuous reduction of body weight. The results of this treatment depend highly on an intensive education as well as on frequent control of the patients' metabolism and their cooperation.

[Determination of serum vitamin B12 in hematologic diseases].

By means of a test set of the Isocommerz (GDR) determinations of vitamin B12 in the serum were carried out according to the principle of the competitive protein binding. The normal values lie between 200 and 1,000 pg/ml serum. Clearly decreased levels of vitamin B12 are found in the pernicious anaemia, in other megaloblastic anaemias and in disturbances of the resorption after resection of the stomach. Increased values can be stated in the untreated chronic myelosis and in the blast episode. Under Busulphan-therapy a significant decrease of the values of vitamin B12 develops. The method seems to be practically important for the well-timed recognition of deficiency conditions of vitamin B12 in beginning pernicious anaemia, in disturbances of intestinal resorption after resection of the stomach and for the observation of the course of the chronic myelosis.

[In vitro studies and model calculations on the kinetics of fibrinolysis with streptokinase].

A method of measuring the distribution kinetics of radioactive 51Cr-EDTA between human plasma and a fibrinoclot are represented. In the presence of streptokinase the speed of distribution is a measure for the progressing of fibrinolysis. The results of measurement as well as the model calculations on the kinetics of fibrinolysis by streptokinase with physiologically relevant data are compared with those observations on the mechanism of thrombolysis obtained from biochemical findings. These results indicate that at the dissolution speeds of the clots observed clinically or experimentally the diffusion of streptokinase activator complexes into the clot seems to play no important role.

[Studies on the influence of glucose infusion on parameters of hemostasis].

In continuation of previous in vitro experiments the influence of glucose infusions on the following haemostatic functions was investigated: bleeding time, platelet count, platelet aggregation, release reaction, fibrinogen concentration, partial thromboplastin time. Five volunteers with normal metabolism a glucose infusion (100 g) was given for two hrs. Before, 1, 2, 3, 4 and 5 hrs after the beginning of the infusion blood sugar, insulin level and haemostatic parameters were determined. Apart from an increase in the glucose and insulin level, a prolonged bleeding time, increased platelet count, inhibition of platelet aggregation and release reaction occurred. Simultaneously, fibrinogen concentration increased and partial thromboplastin time shortened. The possible causes of these changes in haemostasis during glucose supply are discussed.

[The clinical value of 51 chrome-EDTA clearance].

In routine diagnostics the glomerular filtration rate was in most cases determined by endogenic creatinine clearance. Possibilities of errors in this method are unreliable accumulation of urine, influencibility of the creatinine values by the muscular mass and various exogenic factors. The chromium-51-EDTA-clearance is to be regarded as a more exact method. After single intravenous injection of about 150 muCi Cr-51-EDTA (single-shot-method) the impulse rates in the serum are measured after 60, 100, 140 and 180 min in the bore-hole-crystal. Up to now the number of patients examined is about 300, additionally 6 persons who underwent binephrectomy. Elaboration of a computer programme for the calculation of the clearance values. The best approach is achieved by non-linear regression. There exists a reciprocal connection between serum creatinine and Cr-51-EDTA-clearance. Values lower than 90 ml/min must be regarded as pathological. The Cr-51-EDTA-clearance is particularly valuable in the so-called creatinine-blind region, in which it gives a more reliable information about the beginning reduction of the glomerular filtration rate, as calculation of the correlation and repeated examinations show. The method can be recommended for the clinical routine diagnostics.

[Animal experimental studies on the pharmacokinetics of streptokinase].

By the use of native and radioactively labelled streptokinase its distribution and elimination were determined in rabbits and compared to the course of the fibrinolytic action. Particularities of the pharmacokinetics of the activator of fibrinolysis were studied and related to the therapeutic effect.
