The Misgav Ladach method for cesarean section: method description.

SUBJECT: A method description is given for the Misgav Ladach method for cesarean section. This is based on the Joel-Cohen incision originally introduced for hysterectomy. METHOD: The incision is a straight transverse incision somewhat higher than the Pfannenstiel incision. The subcutaneous tissue is left undisturbed apart from the midline. The rectus sheath is separated along its fibres. The rectus muscles are separated by pulling. The peritoneum is opened by stretching with index fingers. The uterus is opened with an index finger and the hole enlarged between the index finger of one hand and the thumb on the other. The uterus is closed with a one-layer continuous locking stitch. The visceral and parietal peritoneal layers are left open. The rectus muscle is not stitched. The rectus sheath is stitched with a continuous non-locking stitch. The skin is closed with two or three mattress sutures. The space in between is apposed with non-traumatic forceps for 5 minutes. RESULTS: The basic philosophy is to work in harmony with the body's anatomy and physiology and not against them. The method is restrictive in the use of sharp instruments, preferring manual manipulation. CONCLUSION: The method gives quicker recovery, less use of post-operative antibiotics, antifebrile medicines and analgesics. There is a shorter anesthetic and shorter working time for the operative team. It is suitable for both emergency and planned operations.

Anxiety and defense against anxiety as reflected in percept-genetic formations.

Two percept-genetic methods, the Meta-Contrast Technique (MCT) and the Serial Afterimage Test (AI), were used to study adaptive aspects of perception in a group of 37 subjects suffering from manifest anxiety. The perceptual processes laid bare by these methods reflected not only anxiety but also defensive formations. MCT and AI correlated with each other in these respects and, when combined, correlated with rated symptoms. In accordance with the theory behind percept-genetic methods, anxiety signs appearing late in the test serials were more likely to correspond to manifest symptoms than signs which appeared early. The AI test scores seemed to reflect a difference between anxiety experienced as an inherent quality of life and anxiety as a foreign force struggling for dominance.

Individual factors affecting the improvement of anxiety during a therapeutic period of 1 1/2 to 2 years.

Thirty neurotic outpatients suffering from pronounced anxiety were studied and after a therapeutic period of 1.5 to 2 years. The patients were described by means of rating scales and two perceptgenetic methods, the Meta-Contrast Technique and the Afterimage Experiment. It was possible to associate original status with improvement or non-improvement of manifest anxiety. The most positive prognostic sign was sensitivity to marginal (inner and outer) cues, whereas the most negative signs were pre-psychotic discontinuities, projection, and lack of defence mechanisms.
