[Adjuvant BCG immunotherapy in the management of superficial bladder tumors].

The net benefit of BCG immunotherapeutic prophylactic effect on recurrence of superficial bladder tumours was investigated. The BCG treatment group consisted of 121 stage Ta, T1 patients, while the control group, 49 patients, was treated only with transurethral resection. During 3-year follow-up recurrence rate in the control group was 55.1%, while in the BCG group in the two-and-a-half-year follow-up it was 23.9%. The yearly repeated 6-week cycles resulted in decrease of recurrence rate to 11.8%. The recurrence indexes were 2.2 and 0.6 in the control group and in the repeated treatment group respectively. A progression rate of 18.8% was recorded in the control group and 4.1% in the treatment group. Finally both clinical and investigative results were summarised and the necessity of repeated immunotherapeutic BCG courses were emphasized.

Evaluation of criteria contributions for the classification of systemic lupus erythematosus.

The American Rheumatism Association (ACR) preliminary and revised criteria for classification of systemic lupus erythematosus (SLE) were evaluated for sensitivity and specificity in a population of 100 patients with SLE and 100 patients with other rheumatic diseases. Bayes' theorem was applied for evaluation of ARA criteria for the classification of SLE and a scoring system was developed which allows simple determination of the probability of SLE. The evaluation revealed considerable differences in values of the ARA criteria. The serologic tests and discoid lupus appeared to be the most distinctive criteria, while Raynaud's phenomenon, oral and nasal ulcers and arthritis were of little value. Comparison of SLE and control patients presenting the same number of criteria revealed that patients with SLE exhibit more distinctive criteria. This finding emphasizes the need for a quantitative evaluation of classification criteria.

Reconstituted basement membrane (Matrigel) promotes the survival and influences the growth of murine tumors.

The effects of reconstituted basement membrane (Matrigel) on in vivo survival and growth of several murine tumors were studied. Survival of tumor cells was enhanced in all experiments which resulted in increased incidence and/or in increased tumor mass. While basement membrane enhanced the in vivo growth of B16F6 melanoma cells, survival of these mice was prolonged. Basement membrane increased the incidence but reduced the growth of Ehrlich ascites tumor. Walker-256 hypercalcemic breast carcinosarcoma growth was enhanced and glandular-like structures were observed when grown on Matrigel. The results indicate that the enhanced survival of tumor cells in the presence of basement membrane is not unequivocally linked with increased malignancy.

Long-term BCG immune therapy of superficial bladder tumours.

Long-term local BCG treatment of superficial bladder tumours is described and the 5-year results are reviewed. Complications of major significance in the course of immune therapy did not occur, loss due to death was not recorded. PPD skin test failed to furnish extra information regarding the biologic behaviour of the tumour. Annual repeats of the therapy promise better results than one single 6-week course.

Anti-tumoral and anti-inflammatory effects of biological stains.

The biological stains, methylene blue and its metabolite azure B, were evaluated as anti-tumor and anti-inflammatory agents. Azur B, administered in drinking water to tumor-bearing mice, inhibited the growth of transplanted tumors and the growth of primary tumors induced by methylcholanthrene. Inhibition of growth of primary tumors was observed only in female mice. Azure B also reduced the wet weight of carrageenin-induced granulomas in rats. Azure B, given intravenously to BCG-sensitized mice 15 minutes prior to challenge with lipopolysaccharide, decreased TNF production (to 10% of control values) and prevented death from endotoxic shock. Methylene blue decreased TNF production (to 50% of control values) but did not protect the animals from endotoxic shock. Our results suggest that some of the effects previously ascribed to methylene blue are probably mediated via its metabolite, i.e. azure B. Low toxicity and easy administration of the dyes explain their use in clinical settings.

[Early calcipenic osteopathy in the panoramic x-ray image of the maxillary and mandibular alveolar process of young adult alcoholics].

The authors have examined 220 maxillary and mandibular panoramic roentgenograms of 55-55, all 110, between 21-34 years of age alcohol addict males and females with a complete dentitions and intact parodontium. In 19 per cent of patients developed osteoporomalacia, cystic rarefications and multiple pathologic microfractures of the alveolar spongy bone tissues. The calcipenic osteopathia was verified by the routine laboratory disorders of calcium and phosphor metabolisms, besides by the diminution of roentgenmorphometrical index of right 2nd metacarpus and tibia. The osteoporomalacia of alveolar parts preceded calcipenic osteopathia of all bones in 52 per cent of patients. A quantity of 25-50 g alcohol consumed daily would be resulting the calcipenic osteopathia after 3-5 years. Its development may be due to individual praedisposition influenced by nutritional, hormonal, genetic and hepatopancreatic factors.

Dynamic cavernosography as a means to test coital impotence.

In our days the radiologic examination method of dynamic cavernosography is still in the state of infancy but has already passed the test as an indispensable means to locate the site of a venous leak and to diagnose other pathologic penile conditions. Its field of application covers penile fracture, the extension of induratio penis plastica, the degree of fibrosis in the postpriapic state, the testing of tumour penetration. The method being invasive it is important to apply it in the proper diagnostic order, as soon as clear proof has been furnished that the potency disorder is not due to impaired reflex activity, has not developed on a psychic background and the Doppler test has revealed normal arterial flow conditions.

[Repeated yearly BCG treatment in superficial bladder cancer].

The authors report on experiences of treatment for superficial bladder cancer with BCG have achieved during three years. To achieve a permanent recidive prophylaxis the 6 weeks long course was repeated yearly. During the 37-12 months follow up a tumor recurrence have been observed in 10 of 49 patients (20.4%). There have not been seen any serious side-effects requering the interruption of the course or requering any antituberculotica. On the basis of their results and the torelable side-effects they recommend the local BCG treatment as a favourable possibility in prevention of bladder tumor recurrence.

Current diagnostic and therapeutic methods in the treatment of vesical tumours.

Reviewing the contemporary therapy of vesical tumour, the authors point to the basic principles that govern the daily practice, the precise application of which may go a long way to improving the results. Not ignorable is the experience that one part of these tumours defies discovery by multifocal and cystoscopic examination. The fundamental tenet of tumour surgery to remove the tumour by incision into sound tissue, must be respected. A brief review is given of the main lines that promise headway in dealing with the disease.

Two cases of traumatic denudation of the penile and scrotal skin.

Two cases of denudation injuries of the penis and the scrotum are reviewed. In the first case a total skin erosion of the penis and scrotum has happened. In the second injury, the major part of the skin was lost. The reconstructive operations are shown in a series of photographs. Replacement of the total skin deficiency was performed by placing a split-thickness graft on the penis, while reconstruction of the scrotum was carried out by using a thigh pedicle flap. In the second case partial loss of the skin could be replaced by mobilizing the free skin edges.

Experiences on the local adriamycin treatment of bladder tumours.

Following repeated resections, local cytostatics have recently been increasingly applied for preventing the recurrence of superficial bladder tumours. Fifty-five patients at stages Tis, Ta, Tl were treated intravesically by adriamycin for 16 to 24 months. After the treatment period, the total number of tumour recurrences decreased by more than half. In 18 cases there were no tumour recurrences. The administered cytostatics did not produce any systemic side-effects. Chemocystitis and haematuria were the most frequently associated side-effects. Based on favourable experiences, local adriamycin treatment is looked upon as an effective prevention of recurrences.

Primary ureteral tumours.

The authors report on the clinical picture, diagnostics and therapy of ureteral tumours in 4 patients. The most important symptoms are haematuria and pain. Of the diagnostic possibilities retrograde ureterography is the most significant. Their own surgical solution was ureteronephrectomy with excision of the orifice. Also the conservative therapeutic measures were surveyed. The post-operative control for detecting early recurrences is considered important.

[Experimental treatment of the lesion of the flexor tendon, by means of tendon sheath substitution using a vein and diphasic flexor tendon plasty. A new method for the reconstruction of peritendinous slipping surfaces (author's transl)].

In dog experiments the surgical treatment of flexor tendons is discussed by the authors: the lesions of the tendons have been repaired according to the method of Paneva-Holevitch, with biphasic flexor tendon plasty; and for the substitution of the injured tendon sheath and the peritendinous slipping surfaces freely transplanted, autologous vein has been used. In the preparations removed after 8 weeks both the macroscopic and microscopic examinations showed satisfactory vascularity of the vein used as tendon sheath and of the tendinous tissue, as well as normal tissue structure. Between the vein and the tendon only small, ligamentous adhesions developed. On the basis of the good results obtained in experimental examinations the method is applied by the authors also in men with satisfactory result.
