[Temporary hepatic support by extracorporeal baboon liver perfusion in acute hepatic failure (author's transl)].

Eight patients in acute hepatic failure were treated by a total of 41 extracorporeal baboon liver perfusions. Only one has survived to the present. Parameters of good function proved to be oxygen consumption and bile production. Highly elevated aromatic amino acids decreased significantly during one single passage through an extracorporeal perfused baboon liver, as did ammonia while urea increased significantly. The prognosis of these very ill patients can be improved only when the regeneration of their damaged livers starts up again during temporary hepatic support therapy.

[Acute hepatic coma. Experimental study on the predictive value of clinical-chemical findings for the prognosis of acute hepatic coma].

In order to determine the validity of clinical-chemical parameters for the prognosis of hepatic failure, 28 pigs were subjected to liver ischemia for 40--160 minutes duration. The following parameters were studied: GOT, GPT, gamma-GT, LAP, LDH, GlDH, AP and isoenzymes, total bilirubin, potassium, sodium and chloride. In a statistical comparison in the surviving animals, an unexplainable increase in GlDH activity was observed. In the other clinical-chemical parameters none was seen to be of use for the prognosis for either life or death in acute hepatic failure.

A new method for producing a chronic E. coli pyelonephritis in rabbits.

Chronic pyelonephritis, especially that caused by the most common invader E. coli, corresponding satisfactorily to the pathophysiology of the human disease is difficult to reproduce. We have developed a pattern of infection with which we succeeded reliably in producing a chronic E. coli pyelonephritis. This was achieved by inserting a plastic catheter into the renal pelvis. A suspension of E. coli was injected via the catheter into the renal pelvis of 32 rabbits. This pattern of infection takes a chronic progressive course and, in all cases, results in typical macroscopic and histological changes in the kidney. The bacterial excretion in the urine remains unchanged over a period of months. With the experimental procedure described, the pathogenesis and pathophysiology of chronic E. coli pyelonephritis can be clearly studied. Drugs currently used on patients can be tested for their effectiveness in long-term application.

Acute liver failure due to temporary hepatic ischemia in the pig.

The model of temporary complete liver ischemia was investigated in 60 pigs in order to produce a disease which resembles liver failure in man. The interval of time leading to death in every animal differed in a wide range. A number of biochemical alterations was of no value as for the prognosis of the animal under investigation. The ammonium in the peripheral blood is elevated during shunting of portal blood around the liver only. Microscopic examinations demonstrated that the damage produced is limited to the hepatic tissue. The lesions were less when ischemia was produced after a longer interval after the operation. The role of processes of regeneration and reparation of liver tissue is discussed.
