Nineteen patients with invasive cervical cancer were treated with intraoperative radiation and most of the patients subsequently received conventional external radiation therapy and intracavitary applications. The technique, resulting complications, and survival of the patients are discussed.
Interferons are being tested in the treatment of a wide range of malignancies. Clinical experience has indicated that interferons may have relatively few side effects compared to other commonly used antineoplastic agents. In addition, they have shown activity as radiation response modifiers in various experimental systems. Thus, as natural biological compounds, interferons may prove to be superior to other radiation response modifiers now on clinical trial in radiotherapy. In this paper radiation-interferon interactions are reviewed and the evaluation of the usefulness of interferons as an adjuvant in radiotherapy is discussed.
A major contraindication of radiodiagnostic procedures is pregnancy. Approximately 1% of all pregnant women are given abdominal x-rays during the first trimester of pregnancy. Evaluation of radiation exposure should involve consideration of the types of examinations performed and when performed, as well as radiation dose and risk estimation. This information is then weighed against other possible risks of the pregnancy as well as personal factors. In the authors' experiences, radiation exposures usually result in doses to the embryo of less than 5 cGy (rad); the resulting radiation risks are usually small compared with other risks of pregnancy. Procedures to minimize diagnostic x-ray exposure of the fetus are also discussed.
The appearance of soft tissue concentrations of technitium labeled phosphate compounds has been observed in a variety of pathological conditions. The mechanism and pathophysiology of this phenomenon remain unclear.Seven new patients diagnosed with lung cancer received (99m)Tc diphosphonate bone scans during the period of January 1975 to November 1975. Three of the seven subjects showed a significant accumulation of isotope in the region of the chest infiltrate at the time of diagnosis. Upon repeat technitium bone scans six months post radiation, two patients showed a sharp reduction in the soft tissue activity previously observed. The material is presented and a possible explanation of this phenomenon is offered.
Intraoperative electron beam irradiation is being used for the therapeutic and prophylactic treatment of the paraaortic nodes in cancer of the uterine cervix. A Varian 18 meV accelerator is employed at an electron energy setting of 6--18 meV. Special aluminum and lucite collimators allow visualization of treatment area and keep the uninvolved small bowel out of the treatment beam. A one-time dose of 2000 rads is delivered in 4 minutes. The procedure presents no special surgical problems, is well tolerated, and appears as a promising supplement in cases where abdominal exploration for surgical staging of cervical carcinoma is carried out.
