Platelet hyper- and hypoaggregability in different microangiopathic complications of diabetes mellitus.

In vitro platelet aggregometry was performed in 201 patients with diabetes mellitus, and in 106 controls. The complication-free and retinopathic patients showed hyperaggregability to collagen and arachidonic acid, and also to epinephrine and adenosine diphosphate when neuropathy occurred. Patients with nephropathy, both with and without azotemia, had diminished in vitro platelet responses to each of the four stimuli as compared to age- and sex-matched controls. These characteristics were independent of the type of diabetes. It is concluded that diabetic nephropathy is characterized by reduced platelet in vitro reactivity. Further research is necessary to explain in vitro hypoaggregability in contrast to the numerous proofs of in vivo hyperfunction of platelets in diabetes.

A light and electron microscopic histochemical study on the mechanism of DFP-induced acute and subacute myopathy.

The histochemical changes occurring in association with the development of acute and subacute myopathy have been studied in the rat diaphragm 30 min-48 h after a single i.p. injection of 1.82 mg/kg of the irreversible cholinesterase inhibitor organophosphate, diisopropylfluorophosphate (DFP). In addition to a considerable inhibition of the AChE activity of the motor end-plates, accumulation of ionic Ca2+ and an increase in neutral protease activity in the subjunctional sarcoplasm have been demonstrated. A temporal and causal relationship has been established between the histochemical changes and the development of the ultrastructural signs of myopathy.

The diagnostic value of stapedius reflex and stapedius reflex exhaustion in myasthenia gravis.

Authors studies impedance, stapedius reflex thresholds and stapedius muscle exhaustion on 31 ears of 16 MG patients. Investigations were carried out using GSI 33 computer-assisted middle ear analyzator. Stapedius reflex threshold values were increased in 93% of patients. Stapedius exhaustion was observed in 71% of patients. After the administration of the reversible cholinesterase inhibitor Mestinon (60 mg pyridostigmin bromide), reflex threshold decreased and exhaustion occurred in only 50% of cases. Authors review the literature in context with the audiometric diagnostics of MG and also recommend the use of these methods in more complicated ocular and bulbar cases of MG.

5'-nucleotidase positive microglia reaction in the central nervous system.

Following peripheral nerve injury the proliferation of 5'-nucleotidase positive microglial cells has been noticed around projective motoneurons and autonomic preganglionic neurons of the central nervous system. Similar events were observed in the central arborization fields of the primary sensory fibers. Our results may suggest an important role of glial cells in the neuronal plasticity.
