[Plasma and urine beta-thromboglobulin determination in the detection of thrombocyte hyperactivation in diabetic nephropathies].

Serum creatinine, immunoreactive serum and urine beta-2-microglobulin, plasma and urine thromboglobulin, plasma thromboxane-B2 levels and daily protein excretion were determinated in 61 insulin treated diabetic patients, comparing the different patient groups (complication free, nephropathy without azotaemia and nephropathy with azotaemia) with the control subjects. In the groups of all diabetic patients plasma and urine beta-thromboglobulin and plasma thromboxane-B2 levels were higher that in the controls. There was a positive significant correlation between urine beta-thromboglobulin and beta-2-microglobulin in the group without complication, and between the plasma beta thromboglobulin and beta-2-microglobulin, and plasma beta thromboglobulin and thromboxane levels in the diabetic group with azotaemia. In contradiction to some previous assumptions, the increased level of plasma beta-thromboglobulin reflects a real platelet hyperactivation also in patients with diabetic nephropathy. At the same time urine beta-thromboglobulin also increases. Determination of urine beta-thromboglobulin is more simple with less possibility of methodological error.

Cardiac autonomic neuropathy and QT interval length. A follow-up study in diabetic patients.

For evaluating the clinical significance of QT interval prolongation in diabetics with cardiac autonomic neuropathy (CAN), 53 diabetic patients were followed-up for 5 years or to death and the results of cardiovascular function tests as well as the values of QT intervals were repeatedly determined. At baseline investigation, the QTc intervals were significantly longer in diabetics with definitive (456 +/- 5 ms, mean +/- SEM, n = 17) than those with early (435 +/- 5 ms, n = 13, p less than 0.01) and without (413 +/- 4 ms, n = 23, p less than 0.001) signs of CAN or in controls (414 +/- 5 ms, n = 15, p less than 0.001). Thirteen patients died during the follow-up period (1 without, 2 with early and 10 with definitive signs of CAN) but QTc intervals did not differ significantly between patients with cardiac (456 +/- 9 ms, n = 8) and non-cardiac (459 +/- 15 ms, n = 5) causes of death. At reinvestigation of 40 patients, the severity of CAN worsened in 22 patients, remained unchanged in 15 patients and improved in 3 patients. Accordingly, the mean values of autonomic function tests decreased (beat-to-beat variation from 15 +/- 2 to 9 +/- 1 beats/min, p less than 0.01; 30:15 ratio from 1.19 +/- 0.03 to 1.09 +/- 0.02, p less than 0.01) while QTc interval increased (from 424 +/- 3 to 431 +/- 4 ms, p less than 0.01). It was concluded that CAN carries a poor prognosis in diabetic patients. Nevertheless, QTc interval prolongation could be evaluated as rather an additional sign of CAN than the only explanation for mechanism in the pathogenesis of sudden cardiac death in diabetic patients.

[Correlations between cardiorespiratory reflex impairment and distal neuropathy in diabetics free of clinical symptoms of autonomic neuropathy].

Signs of autonomic cardiac neuropathy and its association with distal symmetrical polyneuropathy were investigated in adult diabetic patients free from clinical symptoms of autonomic neuropathy. Cardiorespiratory reflexes were assessed by non-invasive tests (deep-breathing, Valsalva manoeuvre and lying-to-standing) evaluating parasympathetic function of cardiac innervation. Measurement of motor nerve conduction velocity in both peroneal nerves and neurological physical examination were carried out for assessment of distal somatic neuropathy. Among 64 diabetics, definitive signs of cardiac autonomic neuropathy were found in 28 patients (44%), early signs of cardiac autonomic neuropathy were observed in 19 patients (30%) while no alterations were documented in 17 patients (26%). The values of motor nerve conduction velocity in peroneal nerves (41.8 +/- 0.7 m/s) were significantly (p less than 0.01) lower in patients with definitive cardiac autonomic neuropathy (n = 28) than those (45.8 +/- 1.1 m/s) of patients without any signs of cardiac autonomic neuropathy (n = 17). These latter values were, however, significantly (p less than 0.001) lower than those (53.7 +/- 0.7 m/s) of control subjects (n = 50). Abnormal results of non-invasive tests for autonomic neuropathy, i.e. alterations of cardiorespiratory reflexes indicating parasympathetic impairment in cardiac innervation could be often found in diabetics without clinical signs of autonomic neuropathy. These alterations could be frequently observed in diabetics with distal symmetrical neuropathy as well as in diabetic patients with one or more late specific complications.

[Surgical treatment of pulmonary metastases of colorectal tumors].

In 22 cases of the 510 patients underwent surgery and followed up because of colorectal cancer, only pulmonary metastases were found. General problems in the surgery of pulmonary metastases are discussed and the possibilities and the expected results of the surgical therapy of pulmonary metastases in colorectal patients are analysed in details on the base of the data of their 8 patients underwent surgery and according to the literature. In carefully selected cases the surgical therapy of the metastases can be recommended.

[Correlation of thrombocyte reactivity and serum levels of HbA1c, cholesterol and creatinine in diabetes mellitus].

The thrombocyte reactivity and values of HbA1c, serum cholesterine and creatinine have been examined in 121 insulin-treated and 70 not-insulin-treated diabetic patients and in 98 healthy persons. The thrombocyte functions of patients ranged according to the microangiopathic complications and of control groups matched according to age and sex were analysed with comparative statistics. Positive correlation was found in diabetes between the serum creatinine and cholesterine levels and the aggregating agents' (adrenaline, ADP, and collagen) limit concentrations (p less than 0,05-0,001). Close correlation seems to be between the worsening of renal functions and the decrease of thrombocyte sensitivity in diabetes: The hypercholesterinemia observable in nephropathic diabetes did not lead to the hyperaggregability known in familial hypercholesterinemia. Thus it appears likely that the cholesterine-level increase in the serum does not influence directly, but rather by the effects in connection with its origin, differently the thrombocyte reactivity.

Impairment of cardiorespiratory reflexes and its association with distal somatic neuropathy in diabetic patients free from clinical symptoms of autonomic neuropathy.

Signs of autonomic cardiac neuropathy and its association with distal somatic neuropathy were investigated in 36 type 1 and 28 type 2 diabetic patients free from clinical symptoms of autonomic neuropathy. Using bedside tests (deep-breathing, Valsalva manoeuvre and lying-to-standing) definitive cardiac autonomic neuropathy was found in 28 patients (44%), early cardiac autonomic neuropathy was observed in 19 patients (30%) while 17 patients (26%) showed no alterations. The values of motor nerve conduction velocity in peroneal nerves (41.8 +/- 0.7 m/s, mean +/- SEM) were significantly (p less than 0.01) lower in patients with definitive cardiac autonomic neuropathy than those (45.8 +/- 1.1 m/s) of patients without any signs of cardiac autonomic neuropathy. These latter values were, however, significantly (p less than 0.001) lower than those (53.7 +/- 0.7 m/s) of control subjects (n = 50). Signs of early or definitive cardiac autonomic neuropathy were recorded in 31 of 35 diabetic patients with distal somatic neuropathy assessed by measurement of motor nerve conduction velocity in peroneal nerves. It was concluded that abnormal results of noninvasive tests for autonomic neuropathy, i.e. alterations in cardiorespiratory reflexes indicating parasympathetic impairment of cardiac innervation could be often found in diabetic patients without clinical signs of autonomic neuropathy. These alterations could be frequently observed in diabetic patients with distal symmetrical somatic neuropathy.

Pulmonary metastasis of colorectal tumours.

A total of 21 metastases manifesting only in the lungs were observed during the follow-up of 510 colorectal tumour patients. The general aspects of the surgical solution of pulmonary metastases are reviewed and, based on the data of the patients operated by the authors and that of the literature, the possibilities and expected results of the surgical treatment of pulmonary metastases of colorectal tumours patients are discussed in detail. The surgical solution of metastases is suggested in carefully selected cases.

[Necrobiosis lipoidica without diabetes mellitus (diagnostic and therapeutic possibilities)].

The goal of the present study was to follow the clinical behaviour of 6 non diabetic patients (5 females and 1 male, aged 23-68) suffering from necrobiosis lipoidica. Thickening of the basalmembrane of capillaries could be confirmed by electron microscopy, although the histological structure of skin alterations are not different from those observed in diabetes mellitus. Three patients (2 females and one male) showed impaired glucose tolerance, 2 other patients had increased levels of total cholesterol, whereas one patient suffered from both metabolic disturbances. After treatment with ASA (acetylsalicylic acid, 1.0 g/day) and dipyridamole (200 mg/day) for six weeks, the decrease of platelet in vitro aggregation in platelet rich plasma could be observed by stimulation with arachidonic acid, epinephrine, ADP and collagen, respectively. Healing of the exulceration of skin lesion could be detected by the use of the combined treatment of ASA and dipyridamole in 4 cases.

[Differences in platelet aggregation in various microangiopathic complications of diabetes mellitus].

In vitro platelet aggregometry with epinephrine, adenosine-diphosphate, collagen and arachidonic acid was performed in 201 patients with diabetes, and in 106 healthy subjects. Those patients who were free of nephropathy showed hyperaggregability to collagen and arachidonic acid, and also to epinephrine and adenosine diphosphate, when neuropathy occurred. Patients with nephropathy, both with and without azotaemia, had diminished platelet responses to each of the four aggregating agents as compared to age- and sex-matched controls. Aggregability was not dependent on type of diabetes. It is concluded that diabetic nephropathy is characterized by decreased in vitro reactivity of platelets. Further researches are necessary to explain in vitro hypoaggregability besides the numerous evidence of in vivo hyperfunction of platelets in diabetes.

[Clinical significance of beta-2-microglobulin in diabetes mellitus].

The beta-2-microglobulin (B2M) level of the serum and urine was determined in 61 diabetics and 15 control patients by enzyme-immunoassay, to asses the value of B2M in clinical diagnosis. In patients with daily protein excretion exceeding 300 mg there was a significant positive correlation between serum creatinine, daily (24-hour) protein excretion, and B2M level of the serum and urine. In patients with less than 300 mg and in those with more than 300 mg daily protein excretion the B2M levels of the serum and urine were significantly higher than in the controls. Serum creatinine level underwent a significant rise only in patients with more than 1000 mg daily protein excretion. Determination of the B2M level is a sensitive method in the diagnosis of diabetic nephropathy. Simultaneous measurement of B2M level in the serum and urine detects nephropathy at an early stage and thus it may be of value in the prevention of the disease.

Acetylcholinesterase in the alimentary canal of fish: light and electron microscopic detection, quantitative distribution in different segments of the gut.

The acetylcholinesterase activity was measured and histochemically localized in the alimentary tract of 2 fish species, carp (Cyprinus carpio) and tench (Tinca tinca). A comparison was made of the activities in the different gut segments. Light and electron microscopic histochemistry revealed acetylcholinesterase-positive cell bodies along the entire length of the alimentary canal in both species, between the muscular layers. Acetylcholinesterase-positive, cholinergic motor endplates were frequent in the esophagus of both carp and tench, and they were also present in the striated muscular layers of the tench stomach and midgut. The enzyme activity detected by the method of Ellman et al. (1961) was highest (16.3 U/mg protein) in the tench foregut and midgut, while it was at the same lower level (9.5 U/mg protein) in each segment of the carp gut and in the tench hindgut. The morphological findings and the higher acetylcholinesterase activity in the tench foregut and midgut suggest that the enteric striated musculature is endowed with denser cholinergic innervation than the enteric smooth musculature.

[The significance of ketonuria in lactating cows].

In two large sized farms in Hungary and in several small and medium farms in Bavaria the authors studied the development of ketonuria in cows after calving. In two flocks without problems 30 percent of the cows developed ketonuria, whereas the rate was 56 percent in one problematic flock. Milk yield of the cows observed was above 5000 kg per year, their age differing only slightly. Cows with ketonuria revealed an increased enzyme activity of AST and a decreased plasma-glucose concentration in comparison to the ketone-free animals. Also, the ketonuria cows showed higher amounts of free fatty acids in plasma and lower amounts of total cholesterol. Additionally, these animals more often revealed reproductive disorders. The rate of culling and emergency slaughter was also increased, whereas their pregnancy rate was decreased.

[Personal experience with the Milwaukee orthosis in the treatment of idiopathic scoliosis].

The authors evaluate the results of treatment of idiopathic scoliosis by a Milwaukee brace in 192 patients with treatment initiated and terminated in 1975 to 1985. In the group of 116 juvenile scolioses at the end of the investigation 26 had improved, 53 did not improve and 37 deteriorated. In the group of adolescent scoliosis 18 improved, 44 did not improve and 11 deteriorated. In the group of improved patients the number of persons declines with the time interval after completed treatment. With the increasing severity of the initial curve the effect of treatment by a brace diminishes. The percentage of non-cooperating patients is substantially higher in the group of not improved and deteriorated cases. In 26 selected patients the authors investigated the lordosis-promoting effect of the brace on the thoracic spine. Because of progression of the deformity the authors operated 12 patients with juvenile and one patients with adolescent scoliosis.

5'-Nucleotidase in postnatal ontogeny of rat cerebellum: a marker for migrating nerve cells?

The distribution of the ectoenzyme 5'-nucleotidase was studied in the developing rat cerebellum using enzyme- and immunocytochemical techniques in order to evaluate its appearance in the processes of cellular migration during nervous system development. 5'-Nucleotidase enzymatic activity appeared first in the external granular layer (postnatal day 6). Then, the inner portion of the external granular layer and the developing molecular layer became prominently stained (postnatal day 12). At 21 days the adult pattern was established with a diffuse labeling of the whole molecular layer. Additionally, 5'-nucleotidase enzymatic activity was found around Purkinje cells. At the ultrastructural level migrating nerve cells exhibited an intense surface labeling whereas resting neuroblasts of the external granular layer as well as all post-migratory nerve cells were negative. Immunocytochemistry differed from enzyme histochemistry in that immunoreactivity was found to be selectively associated with glial elements. It is suggested that 5'-nucleotidase may serve as a marker for processes of neuronal migration and neuron-glia interactions during postnatal ontogeny. Underlying mechanisms could be due to an interaction of 5'-nucleotidase with the extracellular matrix molecule, laminin, and cytoskeletal actin involved in locomotion.

Enteric neuromuscular junctions: comparison of ultrastructural features in different phylogenetic groups.

The enteric neuromuscular junctions of snail (Helix pomatia), locust (Locusta migratoria migratorioides), cockroach (Periplaneta americana), carp (Cyprinus carpio) and tench (Tinca tinca) were studied by means of different light and electron microscopic methods. The nitroblue tetrazolium staining revealed that the myenteric plexuses of the above species are composed of nerve cells, a network of varicose nerves and nerve bundles. Instead of highly organized ganglia, single neurons or small groups of 2-4 cells are characteristic of the invertebrates and fish studied. Catecholaminergic fluorescence induced by glyoxylic acid was detected in the muscular layer of the entire alimentary tract in snail and the hindgut of tench. Fluorescent nerves and perikarya were frequent in the snail gut, while only nerves and no perikarya were found in tench. A close contact between enteric muscles and nerves is the most common form of enteric neuromuscular junction in both the smooth (i.e. the molluscan and fish gut) and the striated (i.e. the insect gut) musculature. The striated musculature (i.e. the insect gut, the oesophagus of carp, and the oesophagus, stomach and the midgut of tench) also receives a synaptic input. Cytochemical evidence is provided of the cholinergic character of fish motor endplates. The ultrastructural appearance and vesicle population of certain nerve terminals suggest a universal role of aminergic and peptidergic control in gut motility.

Sex- and age-dependence of platelet aggregation in diabetes mellitus.

Platelet aggregate ratios (PAR) were determined, and threshold concentrations (ED50) of epinephrine, adenosine diphosphate (ADP), and collagen were estimated by platelet aggregometry in 88 IDDM and 52 NIDDM patients without hyperlipidaemia or azotaemia, and in 106 healthy volunteers to revise the question of hyperaggregability in diabetes. ED50-s showed a tendency for negative correlation with age, significant in female but not in male controls. Similar trends were obtained in IDDM and NIDDM females, but were not in IDDM and NIDDM males. The ED50-s of different aggregating agents positively correlated with each other. ED50-s were higher in men than in women in both controls and IDDM patients. Similar but minor differences were observed between women and men in NIDDM. IDDM patients had significantly lower PAR and collagen ED50, and a tendency for epinephrine and ADP to be lower as compared to the sex- and age-matched controls. The differences of PAR were the same, while those of ED50-s were diminished in older NIDDM patients compared to the matched controls. It is concluded, that the previously observed general hyperaggregability in diabetic patients may have partly resulted from sex- and age differences. Threshold concentrations should be compared to sex- and age-matched controls.

Energy-dispersive, bulk specimen X-ray microanalytical measurement of the intracellular Na+/K+ ratio in human laryngeal tumors.

Energy-dispersive X-ray microanalysis was performed on human biopsy materials taken during laryngoscopic intervention in 18 cases. The removed tissue pieces were divided into two parts. One of them was used for pathohistological studies, the other was processed for X-ray microanalysis by the freeze-fracture freeze-drying method. Of the cases investigated 4 proved to be of benign character, whereas the rest contained carcinoma planocellulare keratoides or non-keratoides. Bulk specimen energy-dispersive X-ray microanalysis of 135 cells from the benign tissue samples revealed an average Na+/K+ molar ratio of 0.13 +/- 0.01 (SEM) in the intracellular space, with a regular Gaussian distribution. In the cases of carcinomas 641 cells were measured, the average of the same ratio was 0.67 +/- 0.03 (SEM) due mostly to an increase in the Na+ content. The distribution of data was apparently not normal in the cancerous samples. These observations and some theoretical considerations support the notion that the intracellular Na+/K+ ratio correlates with the proliferative capacity of tissues. The relevance of some recent biochemical results is also discussed in this respect.
