[Difficulties in recognition and treatment of lethal midline granuloma and otolaryngologic and radiotherapeutic practice].

The authors report on two cases of a "lethal midline granuloma". The first patient was treated by irradiation and chemotherapy. He lives for the last three and a half years in complete remission. The established diabetes did not affect the healing process. In the second case only a palliative irradiation was practicable because of the extreme tumour propagation, but the result was nevertheless good.

Dose reconstruction of x-ray irradiation of the larynx.

The authors have determined the dose distribution in the neck for parallel opposed and three field orthovoltage X-ray irradiation techniques of the larynx. LiF:Mg, Ti rods and pellets combined with film densitometry were used. Absorbed dose measured in the region of vocal chords is about 20% lower than that assessed by assuming full backscatter (perpendicular incidence) and by using standard isodose tables.

In vitro proliferation of normal and leukaemic human leukocytes controlled by an inhibitory endopeptide.

GI-3, an endogenous inhibitory fraction isolated from leukocytes, selectively inhibits the proliferation of granuloid precursor cells in a non-toxic manner. Its active principle was determined as an acidic chlor-tolidine positive decapeptide [ 3 ]. The in vitro effect on normal and acute leukaemic human bone marrow and blood cells was examined. A dose dependent inhibition by GI-3 of 3H-TdR incorporation into myeloid cells of normal bone marrow was found, the sensitivity of human cells being higher than that of rat cells. The proliferation of the target leukaemic bone marrow and blood cells (AML, AMMoL) was also decreased by the endogenous inhibitor in a dose dependent manner in untreated subjects as well as in patients in remission or relapse. The rate of inhibition of leukaemic of well-known cytostatics (adriamycin hydrochloride, dianhydrogalactitol) applied for comparison. Beyond its direct cytostatic effect, GI-3 could be used in the differential diagnosis of blastic leukaemias, complementing the routine cytochemical methods.

New primary malignant tumors in patients treated for carcinoma of the uterine cervix or corpus.

Subsequent primary malignancy was revealed in 1.8 per cent of 2, 689 cases with cervical and in 1.7 per cent of 773 cases with corpus carcinoma. In the cervical carcinoma series the most frequent new primary sites were the skin, breast and recto-sigmoid. Of the cases with cervical and corpus carcinoma 18.8 and 30.7 per cent, respectively, had a family history of malignant disease.

A clinicopathological study of non-Hodgkin's malignant lymphomas in terms of the Kiel classification.

A group of 25 patients with non-Hodgkin's lymphoma was investigated. Patients diagnosed earlier were reclassified according to the Kiel system. A correlation between age distribution and histological malignancy was found. The time between the onset of symptoms and diagnosis was 1 year on an average. The majority of the patients belonged to stage IV. The survival rate was higher in the low-grade malignancy group than in the high-grade group. When assessing the prognosis, the histological classification as well as the clinical staging ought to be considered. The bone marrow was the most frequent extranodal site of involvement in stage IV. Cytopenic changes were almost invariably accompanied by bone marrow infiltration. All the 7 cases analysed for lymphocyte surface markers proved to be B cell type. No significant difference was seen between the results of single agent and combined chemotherapy.

[Radiation therapy of oropharyngeal tumors (author's transl)].

The clinical data as well as a statistical evaluation of the treatment of patients with oropharyngeal tumors, who had undergone a combined treatment by interstitial radium therapy, megavoltage irradiation, and operation, lead to the conclusion that better final results and palliation effects may be obtained by a megavoltage irradiation of the tonsillar region and soft palate than by means of ultraradical surgery. Good results have been observed after interstitial radium therapy or electrosurgical operations of keratoid squamous cell carcinomas. The radical operation with preceding telecobalt irradiation, however, is to be preferred in the case of basioglossal squamous cell carcinomas.

The role of radiotherapy in the treatment of primary tumours of the frontal sinuses.

Three cases of primary frontal sinus carcinoma are described, all three in males. Radiotherapy for these tumours can be applied either combined with surgical treatment pre, or postoperatively using supervoltage radiation, or by itself. If applied alone, it may either be a palliative measure in inoperable cases, or a curative measure in radiosensitive tumours.

Comparative observations in the treatment of acute leukaemia.

The results of treatment of 84 acute leukaemia patients during a 15-year period are reported. Sixty-three of the patients suffered from acute myeloid leukaemia, 14 had blastic crisis of chronic leukaemia and 7 had acute myelomonocytic leukaemia. Administration of prednisolone + purinethol, prednisolone + vincristine, and prednisolone + vincristine + purinethol combinations resulted in partial remission. The best results were achieved with the combination ARA-C + thioguanine + prednisolone, which produced complete remission in 2 out of 8 cases. One patient was refractory to this treatment.
