[Identification of slow anterograde conduction of excitation in patients with paroxysmal reciprocal atrioventricular nodal tachycardia].

Basing on results of examination of a cohort of patients with typical paroxysmal reciprocal atrioventricular tachycardia a group of patients with continuous curve of anterograde conduction was distinguished. The use of incremental doses of adenosine triphosphate (ATP) during transesophageal atrial pacing allowed in some cases to identify slow conduction through atrioventricular node and to initiate paroxysmal nodal tachycardia. The dose of ATP causing conduction block over fast pathway depended on the duration of effective refractory period.

[Electrophysiological criteria for efficacy of different antiarrhythmics classes in patients with paroxysmal reciprocal atrioventricular nodal tachycardia].

Clinical and electrophysiological studies of 510 patients with different paroxysmal reciprocal atrioventricular nodal tachycardias identified criteria for resistance of paroxysmal nodal tachycardia. Examination of changes in major electrophysiological parameters due to different antiarrhythmics established the specific features of their electrophysiological action and thereof criteria for the efficacy of antiarrhythmic therapy.

[The prevalence of arterial hypertension among the organized and unorganized male population based on cardiological screening data].

Cardiological screening for hypertension has been carried out in two population samples of males. The ordered samples consisted of workers and employees (a total of 1383 subjects) of two institutions. Of them workers made up 64.3%. A random male population comprised 2481 subjects. Of them 1188 (47.88%) were employees. The incidence of hypertension appeared much higher in the random population. It was specific for age and occupation. Hypertension occurred more frequently among workers than employees in both populations. The occurrence of hypertension was associated with body mass excess and age.

[The prevalence of cardiovascular diseases among the workers and employees of organized collectives based on cardiological screening data].

Altogether 90.2 thousand workers of 33 enterprises of the cities and villages were examined. Of these, men (mean age 41.1 +/- 1.1 years) accounted for 39.2%. Use was made of an automated system for mass cardiological screening and dispensary observation made over time of the workers and employees of the organized bodies by computer EC, CM and IBM PC XT/AT. Different cardiological diseases were revealed in 38.4% of the examined. Out of the total number of the patients, 44.5% were diagnosed to have arterial hypertension (AH) whereas 18.6% coronary heart disease (CHD). CHD was widely prevalent in the working middle-aged and elderly men and women. Angina pectoris of effort appeared to be most frequently occurring form of CHD, accounting for 70.4% of the total number of CHD patients. Myocardial infarction was discovered in 0.9% of men and 0.3% of women. AH was identified in 40.8% of CHD patients, whereas 38% demonstrated borderline AH. Therefore, associated CHD and elevated blood pressure was recorded in 78.8% of the patients. A relationship was established between the prevalence and incidence of cardiovascular diseases and sex and age.

[Cardiological service: essential components and functions].

A system of cardiological service consisting of five essential components is described: profile of diseases (the object of care and the subject of distributing labour between physicians); qualification of a cardiologist; continuity of care; qualification of a therapist; management. Four main groups of typical targets for cardiological service units are identified. For the purpose of evaluating the performance of the unit of this medical service in dynamics it is suggested to consider it as consisting of three elements (therapist-patient-cardiologist), and the quality of work of each physician as consisting of four interrelated blocks of operations: to assess the health status and diagnose the disease, to make a diagnosis and register it officially, to choose and prescribe treatment, to determine the place for further treatment and follow-up. The data provided could be used for promoting the structure of cardiological service and its components and for choosing the unified method for their further investigation.

[Criteria for a pathogenetic approach to antiarrhythmic therapy in paroxysmal supraventricular reciprocal tachycardia].

An electrophysiological study using programmed transesophageal atrial pacing was performed in 106 patients with abnormalities in the myocardial conduction system, of whom 102 suffered from paroxysmal supraventricular reciprocal tachycardia and/or paroxysmal atrial fibrillation. The electrophysiological properties of the myocardial conduction system were demonstrated to be labile. The factors determining the frequency and length of palpitations were outlined. The conditions for an onset of supraventricular reciprocal tachycardiac paroxysm and for existence of a reentrant chain were found to be minimal in the effective refractory period of the accessory pathway, which was less or equal to the effective refractory period of the atrioventricular junction. An approach to antiarrhythmic therapy was proposed, which was based on modifying the ratio of the effective refractory periods of normal and abnormal excitation pathways.

[Effectiveness of 1 of the specialized devices for mass cardiological screening of the population].

The efficacy of a specialized device for automatic ECG analysis was appraised on the grounds of examination of 1,499 healthy and sick persons. The method makes it possible to detect 74,2% of patients with diseases of the cardiovascular system. Patients with ischemic heart disease were detected with the greatest reliability (over 90%). A high reproduction of the results on homogeneous contingents of healthy and sick persons is shown, which is evidence that the use of specialized devices for automatic ECG analysis should be recommended for the cardiological screening of the population.

[Algorhythm for automatic analysis of electrocardiograms in a special apparatus].

An algorhythm for an automatic analysis of the ECG by using a special device is proposed for the purpose of identifying patients suffering from diseases of the cardiovascular system. The algorhythm is based on the utilization of simple electrocardiographic indications measured with sufficient accuracy on a real time scale (PQ, QRS, T1 configuration, amplitude of the R, S, T waves). With the algorhythm analysis of the ECG taken in patients with primary rheumatic heart disease, complicated mitral lesion, hypertensive disease, myocardialinfarction the ECG changes helped identify 70 per cent of the patients. The algorhythm proved highly effective in recognizing patients with ischemic heart disease. A correlation between the stages of the affection and the frequency of detected pathological ECG was established. The results of automatic evaluation were contrasted against the data subsequent to an ECG analysis made by the physician, both of these being in good accord.
