[The clinico-morphological validation of the pathogenetic treatment of industrial eye burns].

The paper generalizes and analyses experimental results obtained in 60 eyes of 30 rabbits after induced burn by means of epichlorhydrine that is widely used in chemical industry and produces industrial burns of the eye in 20% of all burns in the town of Sumgait. Clinico-morphologic researches allowed to reveal peculiarities of pathogenesis of severe eye burns produced by epichlorhydrine causing toxic lesions of the iris and to substantiate effectiveness of hordox used in such cases, that has not only antiproteolytic, but also antiinflammatory, cytotoxic action on the burn process.

[Experimental study of cytokines complex inhibitory effect on wound healing after glaucoma filtering operations: histopathological and immunochemical findings].

Experiments on a model of glaucoma filtering operation in 25 rabbits showed that the natural cytokine complex inhibited the operation wound healing in comparison with the control. The complex was prepared by an original method (culturing peripheral blood leukocytes) at Immunology Department of the Russian State Medical University. Histopathological study revealed delayed formation of cicatricial tissue under the effect of cytokines. Decreased proliferation of fibroblasts and their decreased functional activity (decreased synthetic activity of glycosaminoglycanes and collagen) was detected in cicatricial tissue. Immunohistochemical studies showed that in remote periods (after 3-8 months) type IV membrane collagen formed in new cicatricial tissue, while in the control, types III and I collagens predominated in cicatricial tissue. Immunohistochemical structure of the cicatrix in experiment correlated with fine fibrous structure of cicatricial tissue, characterized by high permeability. Possible mechanisms of the inhibitory effect of the natural cytokine complex on the course of operation wound healing after glaucoma-filtering surgery are discussed.

[New modification of autoconjunctival plastic surgery in urgent corneal surgery].

A new modification of plastic repair of deep defects of the cornea was performed: autoconjunctival plasty of the cornea with a flap on a pedicle with its rigid fixation in the corneal bed. The operation was carried out in 30 patients with infectious, trophic, and autoimmune involvement of the cornea, 24 of these with perforation of the cornea. The patients were observed for 1 year after the operation. The conjunctival-corneal complex formed in 24 cases, dislocation of the flap without its necrosis occurred in 2 cases, and necrosis of the flap in 4 cases. This modification of conjunctival plasty of the cornea can serve as the operation of choice in urgent ophthalmic surgery in cases when transplantation material is unavailable.

[Histological studies of carbon felt (carbotextim M) in late periods after its orbital implantation].

The authors performed histological studies of a grown carbon felt implant and its adjacent tissues in late postoperative studies. During planned operations (cavity or stump correction), pieces of a grown implant with its adjacent tissues were taken for examination from 25 patients in whom carbon had been earlier implanted during enucleation and delayed stump plastic repair. Morphological studies indicated that mature connective tissue had grown between the fibers of the implant, that a connective tissue capsule had formed around the whole implant, and that there was no inflammatory reaction in both the implant itself and its adjacent tissues. By taking into account the fact that there is a good tissue basis and vascularity, the authors present the results of additional simulation of an implant in late postoperative periods and those of additional correction of the implant's volume.

[Specific features of the pathogenesis of epichlorohydrin eye burns and their treatment (an experimental clinico-morphological study)].

Specific features in the pathogenesis of ocular burns with a solution of an organic product, epichlorohydrin, are analyzed. Abnormalities in the vascular system anterior portion were revealed, related to the toxic effect of the agent on tissue vessels and to enhanced permeability and involvement of not only the cornea, but (and even more so) of the iris, where necrotic changes and hemorrhages eventuating in iris atrophy were detectable. Contrykal (hordox) therapy promoted a drastic inhibition of leukocytic and lymphocytic migration into burnt tissue, this resulting in a month in the formation of gentle cicatricial tissue with negligible residual inflammatory infiltration in the burn wound. Iris changes still persisted, since the drug appeared to have no effect on this process.

[A new enriched balanced irrigation solution for intraocular surgery (experimental study)].

Experimental trials of a new intraocular irrigation solution, one of the enriched balanced salines, were carried out. A comprehensive morphologic study has revealed the protector characteristics of lactosol plus (lactosol + taurin) used in the management of irrigation injury to the posterior epithelium of the cornea in the course of experimental intrachamber perfusion. The protective properties of lactosol + taurin, a new enriched balanced saline, are explained by the similarity of its chemical composition to that of the intraocular fluid and by the presence of a stable polyfunctional bioprotector, taurin amino acid.

[Mechanism regulating healing of perforating wounds of the cornea by natural cytokine complex].

Effects of a natural complex of cytokines secreted by peripheral blood leukocytes on the healing of perforating wounds of the cornea were studied in 28 rabbits (14 animals in control and 14 in experimental groups). The process of healing of similar cut wounds of the cornea during local application of cytostatics was assessed by morphological and morphometric methods on days 1, 3, 7, 14, and 30 after the injury. Two periods, early and late, were distinguished in the morphogenesis of healing of a cut wound of the cornea under the effect of natural cytokines. The cytokines enhanced neutrophil migration to the wound canal in the early period of morphogenesis, this accelerating fibrin resorption, which was followed by filling of the wound canal with keratoblastic proliferate. In the late period of morphogenesis of wound healing (days 14-30) the functional activity of fibroblasts was increased, this being conducive to activation of collagenization of cicatricial tissue and resorption of the cicatrix as a result of a possible increase of collagenase release under the effect of cytokines. The possible mechanism of regulating effect of cytokines on corneal repair is discussed.

[Experimental and morphological results of implantation of carbon felt in plastic surgery of the locomotor stump and accessory system of the eye].

Carbon felt (carbotextime) was used to form a stump after enucleation and for plastic repair of the accessory system of the eye in 56 rabbits. Sixteen of these animals were controls in whom silicon implants were used. Comparative analysis of clinical follow-up of experimental animals showed good results of using carbon felt in plastic ophthalmic surgery. Use of silicon implants was associated with a more manifest inflammatory reaction, and there were cases of the implant migration and spontaneous rejection. No complications in early or late postoperative periods were observed in the experimental group. Results of morphologic studies of the tissues adjacent to the implant in various periods postoperation are presented in detail.

[Experimental and morphological results of using carbon foam in the formation of the locomotor stump after enucleation].

Syntactic carbon foam implants were used for the formation of the stump after unilateral enucleation in 24 rabbits. Clinical follow-up of the experimental animals demonstrated positive results of using syntactic carbon foam for plastic ophthalmosurgery. No complications, such as migration, infection, denudation, or rejection were observed in the immediate or late periods after surgery. Results of morphologic examinations of tissues adjacent to implants, carried out at different terms, are presented.

[Experimental validation of using ozonized physiological solutions in intraocular infection].

The study was aimed at assessing the efficacy of ozonized normal salines in experimental endophthalmitis. Experiments demonstrated that, as the control group developed grave endophthalmitis and the clinical picture deteriorated by the end of experiment, eventuating in melting of tissues both in the anterior and posterior segments of the eye, in experimental animals, to which ozonized normal saline was administered directly after infection, endophthalmitis either did not develop at all or was less severe. The anterior segment of the eyeball was not involved in the infectious process, and the changes in the vitreous body were not so grave as in the controls, and by the end of experiment the inflammation ceased completely. By the end of the infectious process in the experimental group the majority of tissue structures of the posterior pole of the eyeball, including the retina, were retained. After administration of ozonized normal saline to animals with full-blood endophthalmitis the symptoms of the inflammatory process abated and the disease took a more favorable course than in control. Hence, experiments demonstrated that even a single intravitreal instillation of ozonized solution during an infectious process brings about a high prophylactic and therapeutic effect.

[Experimental therapy of perforating wounds of cornea with adhelon].

The efficacy of Adhelon, a new stimulator of repair regeneration belonging to the group of adhesion factors, was experimentally assessed in the treatment of ocular injuries on a model of perforating untreated wound of the cornea. Clinical and histologic studies in two groups of rabbits persuasively demonstrated the benign effect of adhelon on the course of wound process: the period of inflammatory reaction shortened, infectious complications were prevented, the wound edges closed sooner, epithelialization and reconstruction of newly formed cicatricial tissue was accelerated. Application of adhelon led to the formation of more delicate, compact, and vessel-free cicatrices and accelerated the repair regeneration of the wounded cornea.

[Experimental morphological study of posttraumatic proliferative vitreoretinopathy prevention by cell adhesion peptide Arg-Gly-Asp-Ser].

Proliferative vitreoretinopathy (PVR) was induced in 25 rabbits by injection of autologous adenosine phosphate-activated platelets in the vitreous. Injury to the internal interface retinal membrane as a result of adhesion of aggregated platelets to it, followed by the formation of an epiretinal membrane, underlies the morphogenesis of experimental PVR. Proliferative processes in the retina were paralleled by destructive atrophic changes in it, which might be due to active physiological substances released in the course of aggregation. The development of PVR was inhibited by a synthetic peptide of cell adhesion with Arg-Gly-Asp-Ser amino acid sequence, which was injected simultaneously with autologous activated platelets. Possible receptor mechanisms underlying the phenomenon of inhibition of the proliferative process in the retina of experimental animals are discussed.

[Natural cytokine complex as a drug bioregulator in healing of alkaline burns of cornea (experimental morphological study)].

Clinical morphological features of reparative processes of alkaline burns of the cornea are studied in 24 rabbits. Effects of the natural cytokine complex on this process are validated. Burn process is regarded as an immune inflammation. Morphological and morphometric data demonstrate the mechanism of the cytokine effect on the reparative processes in the cornea, based on the regulation of the inflammatory postburn and proliferative processes. The repair of the cornea after burn in experiment and control is described in detail.
