[Circulating complexes of plasma fibronectin (fibronectin-fibrin) in human diseases].

The study of circulating complexes fibronectin-fibrin in 21 patients with immunocomplex affections revealed their high levels which occurred in inverse relationships with fibronectin concentrations in the blood, but in direct correlation with indicators of inflammation (fibrinogen, cialic acid). It is suggested that the complexes are involved in the development of fibrosis at the sites of chronic inflammation in immune disorders.

[Effect of cryoapheresis on the course of immune complex diseases].

The paper is concerned with an analysis of the results of the method of cryoheparinoprecipitation of plasma proteins in patients with immune complex pathology. The essence of the method is that in plasma cooling cryofibrinogens, cryoglobulins, immune complexes and other components of the complement system aggregate and precipitate. The process is potentiated by adding heparin to the plasma. After removal of cryoproteins the plasma is frozen and returned to a patient during a session of plasmapheresis. The method makes it possible to give up the use of various protein preparations as substitutes. It was applied to 90 patients: 52 females and 38 males aged 15 to 64. The total number of plasmapheresis sessions was 831, an average of 6 sessions per patient; repeated sessions were performed in 20 patients in different time-intervals. Indications for therapy were the absence of a response to routine drug therapeutic methods for a long period of time and steroid-related complications. Cryoapheresis was applied to 18 patients with hemorrhagic vasculitis. Signs of the abdominal, dermal and articular syndrome were eliminated in all the patients; the urinary syndrome was retained in half of the cases necessitating other therapeutic modalities. A CIC level investigated by a method of precipitation with 3.5% polyethylene glycol was not a criterion of assessment of the gravity of diseases: in cases of its increase before therapy initiation there was a clear tendency to a decrease and normalization by the end of treatment. In hemostasis investigation hypercoagulation activation and hyperfibrinogenemia were revealed in most of the patients.(ABSTRACT TRUNCATED AT 250 WORDS)

[Intensive therapy during splenectomy in patients with subleukemic myelosis].

On the basis of analysis of splenectomies in 25 patients in the terminal stage of subleukemic myelosis--myelofibrosis with the aim of prevention and treatment of complications and better endurability of splenectomy the authors have elaborated a complex of measures for intensive therapy such as conducting the operation under protection of donor thrombocytes, limited transfusion of the whole blood, early administration of antibiotics, heparin therapy and disaggregation therapy in postoperative therapy.

[Changes in hemostasis indices after translumbar aortography].

Changes in the hemostatic system attesting to anticoagulation and antiaggregation activity of radiographic contrast substances belonging to the triiodobenzoate class were recorded in 29 patients with atherosclerosis obliterans after translumbal aortography with the use of 76% verografin in a dose of 1-1.5 ml/kg bw. The same results were obtained in experiments with 76% verografin and iodamide 380 administered to donor's plasma in vitro. Anticoagulant action was the most demonstrable in making thrombin and thrombin-heparin time longer. Antiaggregation activity was evidenced by a decrease in collagen aggregation and reduction of the second wave of aggregation of ADP and adrenaline. Administration of calcium chloride to plasma before an aggregating agent prevented aggregation disorders provoked by radiographic contrast agents.
