[Treatment of carotid artery arteriosclerosis 1966 to 1976. Early and late results (author's transl)].

Early and late results of surgery for carotid arteriosclerosis between 1966 and 1976 were reviewed in 154 patients (192 operations). The percentages of excellent and good early results were 96.7% (stage 0), 91.3% (stage I), 46.6 (stage II) and 51.6% (stage III). Overall mortality was 9.1% within the month (3.8% as neurovascular risk). In late results, out of 140 survivors, the percentages of excellent and good results were 69% in stage 0, 59.7% in stage I, 45.5% in stage II and 56.7% in stage III, the mean delay being 40.5 months. There were 3.6% cases of neurological disorders (0.7% corresponding to the side of the operated carotid artery), 7.1% of vertigo and arterial hypertension, 33 (new) deaths (23.6%) occurred after a mean 31.3 months, of which 16 resulted from some cardiovascular trouble. The cumulative survival curves parallel those of the normal population showing the protection yielded by the operation and the decrease in neurovascular risk.

[Spontaneous right primary iliac arteriovenous fistula and an unrecognized aorto-caval fistula located above an aneurysm].

A patient admitted for treatment of an aorto-iliac aneurism presented himself with swelling of the lower extremities and a low grade bruit over the abdomen continuous throughout systole and diastole. Aortography demonstrated the presence of a suspected ilio-caval fistula. Surgical treatment consisted of closure of the fistula and vascular replacement with an appropriate aorto-bifemoral prosthesis. Large statistics report an incidence of aortocaval fistula up to 4% in association with rupturing aortic aneurisms. The grave clinical signs and aortography confirm the diagnosis. Some of those may obstruct themselves spontaneously only to reopen at the time of operation as in the case reported. The greatest peroperatory problems with closure of those fistulae are pulmonary emboli by air and aneurismal contents. In our first case a temporary caval clip was placed as a part in the management of the lesion.

Venous thrombectomies and partial interruption of the vena cava in 125 cases of thrombophlebitis.

In 125 cases surgically treated for thrombophlebitis of the lower limbs, thrombectomy was done in 96, while caval clipping alone was done in 31. Beginning in 1965, technical improvements and systematic control of the 3 major cross points of the lower limb venous system resulted in gradually improved results as regards patency rate and clinical status. When the return flow was increased by arteriovenous fistula, the patency rate and the clinical results were significantly improved. Comparison of a first group of 23 patients submitted to venous thrombectomy alone with 102 patients protected by filters or serrated clips shows a drastic decrease in lethal pulmonary embolism postoperatively. Mortality in the unprotected group was 8.7% while in the protected series it fell to .98%: the latter death was in fact due to forced selection of an operation of second choice. In the protected series, 69 preoperative pulmonary embolisms were recorded in 55 patients. Only one minor postoperative embolism occurred, this originating from an axillary phlebitis. We find caval clipping to be formally indicated whether or not a venous disobstruction procedure is feasible.

[Vascular complications of injuries. Introduction and results of a survey among Belgian surgeons].

We obtained 68 answers to a questionnaire concerning the organization of the treatment of vascular traumas. Seventy percent of these were treated by association of vascular surgeons and traumatologists. Eighty-six percent of the answers recommend to the surgical responsibilities of traumatologists. 49.1% think the vessel repair should be done by vascular specialists. Vascular surgeons or traumatologists accept to transfer the patient or the surgeon in view of the most appropriate treatment. Traumatologists would appreciate learning vascular skills. Great centers possess adequate X-ray equipments for arteriographic diagnosis. Satisfactory conditions for adequate treatment of vascular trauma seem to prevail in Belgium.
