Conventional and color Doppler sonography in preoperative assessment of ovarian tumors.

OBJECTIVE: To study the vascular patterns of ovarian tumors by color Doppler imaging (CDI) and compare the findings of conventional sonographic studies and CDI with histopathologic findings for the same tumors. METHODS: Fifty nonpregnant women scheduled for elective surgery for ovarian tumors were examined by sonographic scanning and CDI by the same physician. Sonographic morphology scores [SMSs] were used, and the pulsatility index (PI) and resistance index (RI) were calculated after locating vessels in and around the tumors by CDI. RESULTS: It was possible to obtain CDI results in 26 of the 50 women. The sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) were 100%, 71.43%, 73.33%, and 100%, respectively, for SMS. With CDI, the sensitivity, specificity, PPV, and NPV were 100%, 85.72%, 95%, and 100% for PI and 100%, 71.43%, 90.48%, and 100% for RI. RI sensitivity and specificity were identical to those of SMS, i.e., 100% and 71.43%, respectively, but PI specificity was better (85.72%). CONCLUSION: In this study, CDI was definitely better than SMS when color flow could be obtained through the tumors. The overall efficiency in diagnosing the nature of tumors is very high if SMS and CDI are both used in patients having ovarian tumors.

Disseminating myxoid leiomyosarcoma of the uterus.

CASE REPORT: A 60-year-old female, para 1, menopausal for last 12 years was admitted with complaints of distention and pain abdomen for last 2 months. There was no history of postmenopausal bleeding. On abdominal examination there was a vague mass in lower abdomen reaching upto umbilicus. On vaginal examination, uterus was eight weeks size and a vague mass was felt around it through all fornices. On exploratory laparotomy uterus was studded with multiple friable hemorrhagic growths perforating from fundus and filling the peritoneal cavity. TREATMENT: Total abdominal hysterectomy with bilateral salpingo-oophorectomy with debulking of tumour and omentectomy was performed followed by postoperative adjuvant radiotherapy. CONCLUSION: Histopathology report confirmed it to be uterine myxoid leiomyosarcoma which is an extremely rare variant of uterine sarcoma with poor prognosis exhibiting malignant biologic behaviour.

Recurrent aggressive angiomyxoma of vagina--a case report.

Aggressive angiomyxoma is a rare tumour, which presents as a painless expanding mass in the vulvo-vaginal region. It usually occurs in 2nd to 3rd decades of life. It behaves like a low-grade sarcoma with high propensity for local spread and recurrence and can involve vulva, perineum, vagina and urinary bladder. It is difficult to differentiate clinically this tumour from other mesenchymal tumours occurring in this region. Microscopically it must be differentiated from malignant tumours with myxoid change, like liposarcoma and myxoid leiomyosarcoma. Hence, histopathologic examination has a central role in diagnosis of this tumour. We are presenting a case of young woman, who came with history of swelling in vulva and perineum. Imaging studies in the perineal region revealed a large pelvic mass. Fine needle aspiration cytology was inconclusive due to scanty material. Enucleation of mass was attempted in first surgery but complete extirpation could not be performed. The swelling recurred within few weeks after surgery and required irradiation. A second surgery, however, was successful in complete removal of the tumour.

Successful pregnancy outcome after cesarean section in a case of gravid uterus growing in an incisional hernia of the anterior abdominal wall.

We herein report a case of a pregnant woman with previous cesarean section whose uterus herniated in an incisional hernia of the anterior abdominal wall at 34 weeks of pregnancy. Incarceration of the pregnant uterus in an incisional hernia is a rare but serious obstetric situation. Treatment is conservative till term followed by delivery and herniorrhaphy as was done in our case and it was associated with successful outcome.

Progesterone side-chain cleavage by Bacillus sphaericus.

The side-chain of progesterone was cleaved by Bacillus sphaericus to produce two C-19 keto androstene steroids. The structures of these metabolites were androstenedione and 1-dehydroandrostenedione. High concentrations of glucose in the culture medium inhibited conversion of progesterone to these two metabolites.

Better theatre management through intelligent reporting: the TIME (theatre information, management and efficiency) system.

Hospital managers, clinicians or their colleges, and the government departments of health are interested in a variety of information for understanding the performance of the health care system and making informed decisions. Intelligent reporting aims to provide the most relevant and reliable information to major stakeholders to facilitate evidence-based practice. The key element in the reporting system is its ability to identify unsatisfactory practice. The TIME (theatre information, management and efficiency) system developed at the North Queensland Clinical School and James Cook University aims to provide intelligible reports for better theatre management. It reflects an effective amalgamation of surgical expertise and systems management principles.

A study of clinical signs and symptoms of temporomandibular dysfunction in subjects with normal occlusion, untreated, and treated malocclusions.

In the light of the universally growing concern over the suggested relationship between orthodontic treatment and temporomandibular (TM) dysfunction, the purpose of the present investigation was to compare the status of signs and symptoms of TM disorders in three groups of adolescents and young adults. The groups consisted of 30 persons with normal occlusions, 41 with untreated malocclusions, and 31 with treated malocclusions. The clinical status and subjective symptoms of TM dysfunction were recorded according to the principles introduced by Helkimo. The results showed that the normal occlusion group had the maximum number of persons free from any dysfunction, but the differences between the groups in the distribution of persons according to the anamnestic and clinical dysfunction indices were not significant. The only statistically significant finding was the difference in the clinical dysfunction index scores of the persons with normal occlusions and untreated malocclusions. According to anamnesis, the most frequently reported symptoms were related to periods of stress. Among the clinical signs and symptoms, the most commonly occurring were crepitations on palpation and sounds on auscultation of the joints in all the three groups. In conclusion, the absence of substantial differences between the three groups indicates that the role of orthodontic treatment in either precipitation or prevention of TM dysfunction remains questionable.

Quantitative structure-activity relationships. IV. Use of molecular negentropy as structural parameter.

Molecular negentropy, the measure of information content in a molecule, was found to be a useful parameter for quantitation of the structures of molecules eliciting some non-specific biological response. Quantitative correlations were obtained between negentropy of a variety of molecules and their 1. toxic effects on the Madison 517 fungus, 2. narcotic action on arenicola larvae, 3. inhibiting activity on tortoise heart and 4. narcotic effect of frog heart.
