Morphometric evaluation of benign prostatic hyperplasia.

Resected prostate tissue from 9 patients with benign prostatic hyperplasia was subjected to a morphometric analysis. The relative volume density of 4 prostatic tissue components was determined: fibrous (47%), muscular (8%), vascular (3%), and glandular (18%). The resolution of the stromal portion of the hyperplastic prostate into 3 subcomponents represents a finer level of resolution than that previously reported. This enhanced precision in the determination of tissue composition lends new insight into the etiology of benign prostatic hyperplasia, and provides a basis for the evaluation of novel approaches to its treatment.

[Well-differentiated thyroid carcinoma].

18 cases of well-differentiated thyroid carcinoma are described, of which 5 were papillary or mixed carcinomas and 3 were follicular. In 12 of 14 in whom radio-isotope scanning was performed, a cold or indeterminate nodule was found. Fine needle aspiration was positive and concordant with the operative histological findings in half the cases examined. In 4 hemithyroidectomy was performed, and in 14 total thyroidectomy. Thyroid remnants were found on scanning in all of the 14. These remnants were ablated with radioactive iodine (131I). An ablative dose of 30 millicurie was insufficient in almost all cases and a second dose was required. 3 cases with regional spread of the tumor were reoperated, and large ablative doses of 131I(150-170 mCi) were given. In 14 cases clinical remission was achieved. Followup included ultrasonography and total body 131I scans.

[Preoperative diagnosis of carcinoid of the terminal ileum in the absence of carcinoid syndrome].

Carcinoid of the small intestine, usually found in the terminal ileum, presents a diagnostic challenge when the classic clinical and laboratory findings are absent. The commonest symptom, marked abdominal pain due to intussusception, may arouse suspicion of carcinoid. The precise preoperative diagnosis in the absence of the classic syndrome is impossible and the only way to diagnose it is by colonoscopic biopsy of the terminal ileum. The case described illustrates such a preoperative diagnosis in a 59-year-old woman with severe abdominal pain, nausea, vomiting and weight loss. X-ray studies aroused suspicion of tumor intussusception as the cause of the intestinal obstruction. Colonoscopic biopsy revealed the presence of a carcinoid tumor. However, there had been no symptoms of the carcinoid syndrome, nor was there increased urinary 5-hydroxy indoleacetic acid. On operation the tumor was found to be disseminated and unresectable, so surgical intervention was limited to palliative ileo-transversostomy.

Nesidioblastosis in adults.

A 75-year-old woman with hyperinsulinemic hypoglycemia who died of acute myocardial infarction with ventricular rupture is reported. The microscopic examination of the pancreas revealed that the cause of hyperinsulinemic hypoglycemia was nesidioblastosis characterized by the presence of enlarged, irregular islets of Langerhans and clusters of islet cells in close proximity to the ducts. To our knowledge, nesidioblastosis in adults has not been previously reported in Israel.

Prenatal sonographic diagnosis of endocardial fibroelastosis secondary to aortic stenosis.

Endocardial fibroelastosis is characterized by a diffuse thickening of the left ventricular endocardium with or without other cardiac anomalies. This entity had been diagnosed prenatally previously (Bovicelli et al., 1984) at a gestational age of 36 weeks. A case of endocardial fibroelastosis due to aortic stenosis accompanied by pericardial effusion, ascites, and hydramnion, diagnosed ultrasonographically and confirmed pathologically at 21 weeks of gestation, is presented.

Keratoacanthoma arising in stasis dermatitis.

A case of keratoacanthoma arising in stasis dermatitis is presented. The eczematous dermatitis was aggravated by episodes of deep tissue inflammation, yet the common course of the tumor was not altered. To our knowledge, this is the first report of a keratoacanthoma arising in stasis dermatitis.

Emergency parotidectomy for expansile cavernous hemangioma.

A rare case of emergency parotidectomy in a boy of 12, necessitated by a sudden, rapid and marked increase in size of a parotid hemangioma is describe. The literature dealing with parotid hemangiomas in adults is reviewed; the number of cases reported is small. The histological differences and the natural history of parotid hemangioma in adults and in infants are described. The therapeutic approach is discussed.
