Medical student attitudes toward the treatment of HIV-infected patients.

The intention to treat HIV-infected persons and correlates of those intentions were examined in a cross-sectional sample of 1st- and 2nd-year medical students (n = 201). An anonymous questionnaire measured knowledge of HIV transmission, intention to provide medical care to HIV-infected persons, comfort in performing a physical examination on patients from subgroups with high HIV prevalence, level of homophobia, professional altruism, and personal perceived risk of HIV infection. Intention to provide medical care was considered as the dependent variable. All variables except perceived personal risk were found to be significantly related to the intention to provide medical care although knowledge showed the weakest relationship (Odds Ratio = 2.14). The presence of professional altruism and the presence of homophobia were associated with a significant increase in the effect of knowledge on increasing the intention to treat. Results of this report support other studies that showed that factors other than knowledge play a role in clinical decision making regarding patients with AIDS.

Acquired Immunodeficiency Syndrome

Juvenile lumbar discs.

One hundred and two patients, ages 10-18 years, were operated upon for herniated lumbar discs. Until age 17, the male-to-female ratio was even. A significant relationship was found between traumatic etiology and the occurrence of free disc rupture. Disc material was found outside the annular ligament in more than half of the patients. Results of surgical excision have been excellent in 95% of cases. Same level recurrence remains below 3%.

Melanotropic peptides: presence in brain of normal and hypophysectomized rats, and subcellularly localized in synaptosomes.

A major and several minor trichloroacetic acid (TCA) soluble, bioassayable melanotropic peptides, as well as bioreactive and immunoreactive alpha-MSH have been found in the hypothalamus, olfactory bulb and cerebral cortex of normal and hypophysectomized rats. By employing subcellular fractionation procedures it was demonstrated that alpha-MSH and the major TCA soluble melanotropic peptide (MMPB) were localized in synaptosomes and were released by hypoosmotic shock. The analysis of MMPB by electrophoretic and chromatographic procedures reveales that it is not ACTH4-10, ACTH1-10, ACTH1-24, NAcACTH1-10 or alpha-MSH. MMPB was found to cross-react with an antiserum specific for the Lys-Pro-Val NH2 sequence in alpha-MSH, indicating that this C-terminal sequence of alpha-MSH may be present in its structure. MMPB was also shown to differ electrophoretically from the two major TCA soluble melanotropic peptides found in the neurointermediate lobe of the pituitary. In view of their synaptosomal localizations MMPB and alpha-MSH may play a role in synaptic function in the nervous system.
