[Laparoscopic adrenalectomy for neuroblastoma in children].

Since 1992 when it was described the first laparoscopic adrenalectomy in adults, few references can be found about this technique in children in Literature, and less the number of them that makes reference to neuroblastoma. For that reason we think it is interesting to describe two recent patients operated on by laparoscopy of neuroblastoma, the first in the left side and the second in the right with very good results; being the most interesting fact a 48-hours period of postsurgical stay. As conclusion we think that it is an useful technique in Pediatric Surgery, it gives a very good exposure of the retroperitoneal area, it gives small scars that laparotomy, and it reduces the hospital stay, only we object that must pass more time to be sure that it is a good technique for malignancy.

[Non-palpable testicle: joint review of pediatric endo-surgical group and its management by laparoscopy].

We present the joint serie of 5 Pediatric Centers with 256 patients diagnosed of cryptorchidism with nonpalpable testis treated by laparoscopy. We have found that Ultrasound examination is not very useful. This entity affects more to the left side 60%. In near half of the cases 46% we have not found any testicle and in 54% it was carried out an orquidopexy by different methods. In 27% we made an orquidopexy by Fowler-Stephens technique with 86% of good results. We think that laparoscopy is the best way of diagnosis and treatment of this patients.

[ Pulmonary inflammatory pseudotumor in a child].

The Inflammatory Pseudotumor is a no neoplastic process characterised by an irregular growth of inflammatory cells. A case of 10 years old male is described, he was asymptomatic, diagnosed by a casual chest XR, with a spheric mass in the inferior lobe of the right lung. An excisional biopsy was done with the result of "Inflammatory Pseudotumor". The Inflammatory Pseudotumor is a rare process that can be found in different regions of the human body, with most frequent localization, in the lung. The image tests are not specific for the diagnosis that only can be done by biopsy. The recommended treatment is the complete resection.

[Nine years of experience with laparoscopic appendectomy in children].

We review the clinical report of 110 patients that were operated on by a laparoscopic appendectomy in our hospital since January 1992 until december 2000. In 66 patients the reason was an acute appendicitis, and recurrent abdominal pain in 44. The age of them was between 4 and 19 year old with a mean of 10.8; there were 44 males (39%) and 66 females (61%). The maximum weight was 70 kg and the minimum 15, with a mean of 41. In 23% of them vomiting was present in the postoperative period. The hospital stay was 2 or 3 days in 73% of the patients. In acute appendicitis patients 66.1% were with acute inflammation, in 29% complicated and 4.8% negative appendicitis. Of recurrent abdominal pain patients in 50% we found pathological alterations. Finally we had complications in 13% of cases. As conclusion we achieve a reduction in hospital stay, the patients and parents appreciate it, this approach allow a better exploration of abdominal cavity and in those patients with recurrent abdominal pain we obtained a clinical improve.

[Laparoscopy in pediatric surgery].

The aim of this paper is to show our experience in Laparoscopic Surgery in the pediatric age. We make a review of 37 patients, in whom we carried out a laparoscopy. In 20 (54%) the laparoscopy was therapeutic: 15 appendectomies, 1 cholecystectomy, 1 ventriculo-peritoneal catheter extraction, 1 ovarian cyst, 1 bilateral oophorectomy and 1 adhesiolysis. In others 17 (46%) was diagnostic: 15 hepatic biopsies, 1 intussusception and 1 abdominal mass biopsy. Of 15 appendectomies, 8 were for acute appendicitis and 7 for chronic process. The age have ranged from 1 month to 18 years. The operative time from 2 h 30 min to 35 min. We had not complications and the average hospital stay was of 2 days. We can say that this is an useful technique in pediatric surgery.

[The histopathological diagnosis of Hirschsprung's disease. Our experience over 18 years].

Having had several cases of difficult interpretation from the anatomopathological point of view, we make a review of all intestinal biopsies carried out in 54 patients for diagnosis of Hirschsprung's disease and above all of the suction rectal biopsies. The result has been seven cases with false diagnosis that represents 22 per 100 of the series. We analyse the possible causes that can lead to false positives and negatives results with the suction rectal biopsy, like the height where the biopsy has been taken, the age of the patients (71 per 100 of false results were in children under one month) and finally other diagnosis like hyperganglionisme.

[Meckel's diverticulum in children. A series of cases over 16 years].

The authors review 45 cases of patients with Meckel's Diverticulum, 28 were symtomatics and 17 asymtomatics. DM symtomatics were more frequent in males 3:1 and usually the diagnosis was made between 0 and 4 years of age. 85% apared with rectal bleeding, diverticulitis and intestinal obstruction. Near all cases with rectal bleeding and diverticulitis were associated with heterotopic mucosa (90%). The best method of diagnosis is the Tc-99 pertechnetate scan although it has been used in small number of cases because of the emergency. The results have been goods except one death, two intestinal obstructions and two wound infections.

[Carcinoma of the thyroid in childhood].

Authors present two cases of thyroid carcinoma in female patients ages 6 and 7 years with a two month evolution before diagnosis. Thyroid scintiscans showed a "cold nodule" in both. One was a papillary carcinoma and the other a mixed papillary-follicular carcinoma. The long term resulted were good. These neoplasms usually appear as asymptomatic thyroid nodules with normal functional status. Treatment must be medical and surgical. The most frequent type is the papillary carcinoma and the prognosis at 5 years is very good.
