[Myosonographic and electromyographic (EMC) evaluation of donor-site morbidity after microsurgical TRAM flap].

Due to improved microsurgical techniques the role of autologous breast reconstruction is expanding. One reason is the decreased donor-site morbidity compared to other techniques. Perforator based flaps (DIEP flaps) seemingly decrease the damage of the abdominal wall due to flap harvesting. However, complication rate in perforator flaps is still considered to be higher than in TRAM flaps. Additionally, operating time is increased due to the meticulous microsurgical dissection of perforators. To evaluate whether donor-site morbidity is caused by harvesting a part of the rectus muscle (as TRAM flaps), 15 patients after unilateral muscle sparing TRAM flap underwent EMG and myosonographic examination of the rectus muscle. EMG is the only technique to assess the muscle functionally, as the electrical activity of the muscle is recorded, and motor unit recruitment can be shown. EMG examination clearly revealed that harvesting of a muscle sparing TRAM flap did not impair the rectus muscle compared to the intact contralateral muscle. However, motor units were enlarged. Myosonography (in contrast to MRI) detected particular damage of isolated muscle fibres indicating nerve-related muscle changes. Taking all findings together, harvesting of a part of the rectus muscle shows a distinct damage to the muscle. The muscle however still acts functionally. This indicates that problems after harvesting a TRAM flap (bulging, hernias etc.) are not due to the partial removal of muscle, but due to the general dissection of the abdominal wall, as seen in other abdominal operations.

[Computer-supported tissue characterization in musculoskeletal ultrasonography].

A new diagnostic system combining conventional gray-scale ultrasonography (US) and computer-assisted texture analysis of sonograms makes it possible to differentiate more easily between specific neuromuscular diseases. The first step involves myosonography with strictly standardized US. In a group of 72 patients with histologically and molecular-genetically confirmed diagnosis 63 patients (88%) were diagnosed by conventional US as having Duchenne's muscular dystrophy, spinal muscular atrophy, hereditary sensomotor neuropathy or inflammatory myopathy. Secondly, in a double blind setting computer-assisted texture analysis was used on the same sample of subjects. Tissue Texture can be characterized by the brightness as well as the micro- and macro-structure of the tissue. The use of these parameters leads to a sensitivity of 77 to 94% and a specificity of 81 to 98%. In conclusion, the combination of both techniques allows us to avoid invasive diagnostic procedures in a substantial group of patients.

[Classification of intracranial hemorrhage in premature infants].

The most common classification of intracranial haemorrhage in premature infants into four degrees of severity is based on the results of CT-scans. However, this classification does not adequately account for some pathophysiological and morphological changes. For this reason, the paediatric section of DEGUM developed a new method of classification. This classification distinguishes more precisely between the bleeding itself and secondary changes, such as posthaemorrhagic ventricular dilation, which were excluded from the revised classification. The new system contains three levels: Grade I: subependymal haemorrhage, grade II: intraventricular haemorrhages taking up < 50% of the ventricular volume, grade III: intraventricular haemorrhages of > 50% of ventricular volume. Areas of increased echo levels within the brain tissue (formerly grade IV) which are caused by haemorrhagic infarction are now taken as a separate entity. The morphological description lists the side and the location of the haemorrhagic infarction as well as its size, which is classified into 'small' (< or = 1 cm in diameter), 'medium' (1 < or = 2 cm) and large (> 2 cm). Bleeding into the basal ganglia, cerebellum and brainstem are separate entities. In post-haemorrhagic ventricular dilation the distinction is made between self-limiting transient dilation and hydrocephalus requiring treatment.

The value of ultrasound for the diagnosis of malignant hyperthermia.

A highly feared complication of anesthesia is malignant hyperthermia (MH). In vivo identification of MH is achieved sonographically. Transducers of 7.5 and 5 MHz were used for transverse cuts of the upper and lower leg. Sonographic changes were found in 22 out of 33 subjects from 12 families affected by MH, including several positive subjects not previously known to be subject to MH. The ultrasound image reveals increased parenchymal echogenicity, with obscuring of intramuscular septum and fascia. Myosonography allows noninvasive screening of family members of subjects with known MH; potentially dangerous challenge testing or biopsy may be reserved for specific indications.

[Facioscapulohumeral neuromuscular syndromes--problems of differential and early diagnosis and genetic counseling].

Among 20 kinships affected by facioscapulohumeral and scapulohumeroperoneal neuromuscular syndromes 6 probands with autosomal dominant transmission and 5 obviously sporadic cases revealed myopathic changes on biopsy and were classified to have facioscapulohumer (operone)al muscle dystrophies. 5 autosomal dominant cases and one sporadic case with neurogenic muscle changes and 3 other non-dystrophic phenocopies emphasize the necessity of muscle biopsy for nosological assignment. Definite correlations between mode of inheritance, clinical and morphological findings could not be found. Pathological ultrasound and EMG findings in parents and children of seemingly sporadic cases suggest to assume autosomal dominant inheritance with incomplete penterance in such cases.

[Kidney sonography in childhood in chronic recurring pyelonephritis without a urinary transport disorder].

201 children with healthy kidneys and 61 patients of the paediatric nephrological dispensary with chronically relapsing pyelonephritis without disturbance of the transport of urine were examined sonographically. With the help of an atrophy score was controlled, whether the summation of unspecific parameters is in connection with the length of the kidney. A correlation of the introduced point system with the size of the kidney could be proved. The atrophy score represented shall facilitate the sonographic control of the course of non-obstructive parenchymatous renal diseases.

[Kidney sonography in glomerulopathies in childhood].

The rapid progressive course of the glomerulonephritis shows an independent sonographic picture. Apart from the massive enlargement of the organ and the increase of the parenchyma-pyelo-index the kidneys show a high echogenicity, wavy marginal contours and unclear inner structures. In chronic glomerulonephritis both enlarged and diminished kidneys are to be found. A differentiation to the nephrotic syndrome is possible only with the help of the partly increased echogenicity. The elimination of the chronic glomerulopathies from the chronic pyelonephritis by means of sonography does not render any difficulties. In the Alport-syndrome the ultrasound diagnostic gives references to the activity or furidity of the disease. With the increase of the renal symptoms the sonographic picture of the kidneys also changes. In the sonogram no peculiarities for the Schönlein-Henoch nephritis and proteinuria are to be expected. For the acute and chronic glomerulonephritis as well as the Alport-syndrome the use of the sonography for the long-term control of the course is recommended.

[Renal sonography in childhood. Normal findings].

In a prospective study 201 children with healthy kidneys were sonographically examined. Apart from the length unspecific parameters in the ultrasound tomogram, such as possibility to delimit the organ, echogenity and differentiation of the fine structure of the kidneys are demonstrated depending on age. The proportion of smoothly delimited kidneys before the 1st year of age was significantly lower than in the following years. With growing age the possibility to delimit mean echocomplex, renal medulla and renal cortex. In the 1st and 2nd trimenon the echo-richness of the kidneys is to be valuated physiologically. The demonstrated dynamics of sonographic signs shall alleviate the age-specific judgement of renal diseases without disturbance of the urine transport.
