Primiparas with or without oxytocin augmentation: a prospective descriptive study.

AIMS: The aim of this study was to determine during which phase of delivery augmentation is started when used and to establish any correlation that might exist between the oxytocin infusion and the evaluations by primiparas themselves of their labour pain, strength of contractions and fatigue. In addition, we wanted to determine any differences in duration between labour with and labour without augmentation. We finally wanted to measure the incidence of instrumental deliveries, perineal trauma and neonatal outcome among the augmented vs. the non-augmented groups. BACKGROUND: The most commonly diagnosed complication in primiparas is ineffective contractions or protracted labour, otherwise known as dystocia, which literally means arrested or prolonged labour. Different treatments have been tried during the active phase of labour as well as the second stage of labour. The most common treatment today is amniotomy, often used in combination with an intravenous oxytocin infusion. MATERIALS AND METHODS: The study was conducted at the hospital in Ostersund in central Sweden from August 1998 to September 1999. Consecutive primiparas giving birth at full-term were selected to the study. The inclusion criteria were an uncomplicated pregnancy and a spontaneous single delivery with head presentation. The total number of participants was 164. RESULTS: The results showed that 50 of 164 primiparas needed no augmentation, while 88 were augmented during the active phase and 26 during the second stage of labour. The duration of the active phase [median (md) 4 hours 45 minutes, 6 hours 49 minutes and 6 hours 20 minutes respectively for the different groups, P = 0.03], the time between full dilation of the cervix and the start of the second stage (md 20 minutes, 30 minutes and 60 minutes respectively, P = 0.012) and the duration of the second stage of labour (md 40 minutes, 44 minutes and 60 minutes respectively, P = 0.04) were significantly longer in the augmented groups. Operative deliveries, too, were more frequent in the augmented groups. There was a significantly higher rate of perineotomies in the group augmented during the second stage of labour. There were, however, no differences in Apgar score <7 at 1 minute. pH in the umbilical cord and the base deficit were higher in the group which were augmented during the second stage of labour (P = 0.02 and P = 0.06 respectively). Women describing their impression of the experience as a whole generally gave it a high rating, but women who were augmented during the second stage of labour gave the experience a significantly lower score (P = 0.01). CONCLUSION: Augmentation is used in unusually prolonged deliveries. We did not find that augmentation involved a higher frequency of perineal trauma, although it was correlated with a higher frequency of operative deliveries. There was no correlation between the oxytocin infusion and the primiparas' descriptions of the strength of contractions, pain and fatigue, although greater use of epidurals was observed in women with augmented labour. The number of nulliparas in this study was too small to analyse the incidence of ruptures in the sphincter or draw conclusions about differences between the groups with regard to Apgar scores or metabolic acidosis. RELEVANCE TO CLINICAL PRACTICE: In clinical practice, good routines concerning oxytocin augmentation are crucial. Interventions as oxytocin augmentation seem to cause harm to mother and child. To evaluate interventions continuously in obstetric care is therefore important.

Lactate determination in vaginal fluids: a new method in the diagnosis of prelabour rupture of membranes.

OBJECTIVES: To assess whether lactate determination in vaginal fluids, 'Lac test', can be used as a diagnostic test for prelabour rupture of membranes (PROM) and to derive the best cutoff value for a positive test. DESIGN: Prospective observational study. SETTING: Labour ward at Söder Hospital in Stockholm, a large clinical service. POPULATION: Women living in the centre of Stockholm, the capital of Sweden. METHODS: We selected 200 cases with a history of suspect PROM for determination of lactate concentrations in vaginal fluid. In 100 of these cases, actim PROM tests were also analysed. MAIN OUTCOME MEASURES: Sensitivity, specificity, positive and negative predictive values, likelihood ratios (LR) and Kappa indices in the identification of visible amniotic fluid at speculum examination. RESULTS: A lactate concentration > or =4.5 mmol/L was found to be the best cutoff value for a positive test. The 'Lac test' had a sensitivity of 86%, specificity 92%, positive and negative predictive values of 92% and 87%, respectively. Corresponding values for the actim PROM test were 60%, 98%, 97% and 71%. LR for a positive 'Lac test' was 10.75 and for a negative test 0.15. Corresponding values for the PROM test were 30.0 and 0.4, respectively. The Kappa index was higher for the 'Lac test' as compared with the actim PROM test (78% and 59%, respectively). CONCLUSION: Lactate determination is a valid test in cases with a history of suspect PROM and may even be a better predictor than the actim PROM test.

Fetal scalp and cord blood lactate.

Lactate measurements have been evaluated in intrapartum fetal surveillance for some 30 years. However, it was not until reliable, handheld, microvolume devices were launched during the 1990s that lactate became a clinical option. Retrospective data suggest that lactate determinations in fetal scalp blood are better than pH for predicting severe neonatal morbidity. A randomised controlled trial comparing these two measurements found lactate to be more favourable in clinical practice in terms of less sampling failure and reduced time from the decision to do a fetal scalp blood sample to the clinician receiving the result. At present, a large multicenter randomised control trial is in progress in Sweden to compare lactate and pH measurement for the clinical management of suspected fetal compromise. Main end-points in the study are metabolic acidemia or pH <7.00 in cord arterial blood at delivery. Results are likely to be available in 2-3 years time.

Effects of a lipid lowering fibrate and hormone replacement therapy on serum lipids and lipoproteins in overweight postmenopausal women with elevated triglycerides.

BACKGROUND: Cardiovascular disease (CVD) is the leading cause of death in women after menopause. In essence major risk factors for CVD are similar in women as for men inclusive of serum lipid perturbations. The effects of estrogens and hormone replacement therapy on lipid metabolism is widely discussed and warrant further evaluation especially when combined with other lipid lowering drugs. STUDY DESIGN: Postmenopausal women were studied by an open randomised study during 9 months. Subjects were recruited from outpatient clinics in a rural area of Sweden. Major inclusive criteria comprised body mass index (BMI) >28, serum triglycerides >1.5 mmol/l. Participants were at least 12 months postmenopause with a concomitant serum Follicle Stimulating Hormone (FSH) above 28 IU/l. After inclusion in the study patients were instructed to adhere to a low-fat and caloric diet for 3 months and after this period randomised into two groups of intervention; a lipidlowering fibrate (LLF) group and one hormone replacement therapy group (HRT). The LLF group was given gemfibrozil 600 mg orally twice daily and HRT group received 2 mg oestradiol in combination with 1 mg norethisterone acetate as a continuous combined therapy once daily. After 3 months, the LLF group added the HRT regimen and patients in the HRT group added gemfibrozil. Hence, all participants received the regimens combined for the last 6 months of the study. RESULTS: Serum s-cholesterol was markedly decreased in both groups during the first 3 months of single treatment (P<0.0001). This decrease reflected a reduction especially of calculated low density lipoprotein (LDL) s-cholesterol (P<0.001). High density lipoprotein (HDL) s-cholesterol was reduced in the HRT group (P<0.005) but increased (P<0.004) in the LLF group. Triglycerides were also decreased by both treatments but more marked in the LLF group (P<0.0001) than in the HRT group (P<0.02). After 9 months the reduction remained in both groups but no additive effects were encountered in any of the groups. CONCLUSION: The effects by gemfibrozil on s-cholesterol and triglycerides levels seem to be superior to continuous combined HRT in overweight women with elevated triglycerides. The combination of the two drugs did not seem to offer any additional benefit concerning the routine serum lipid or lipoprotein profile.
