[Diagnosis and surgical management of mediastinal neurogenic tumors].

OBJECTIVE: To study the clinical manifestations, diagnostic methods, surgical management and prognosis of patients with neurogenic tumors of the mediastinum. METHOD: One hundred and ten patients with neurogenic tumors of the mediastinum were analyzed retrospectively. RESULTS: After operation, 2 patients died in hospitalization and 8 experienced such complications as Horner's syndrome or laryngeal recurrent nerve paralysis. In 102 patients with benign tumors, 2 patients had recurrence, and 4 patients with neurofibrosarcoma or malignant neurilemmoma died within 3 years postoperatively. CONCLUSIONS: Most neurogenic tumors of the mediastinum are benign and could be diagnosed by chest X-ray or CT. The clinical manifestations, diagnosis methods, surgical management of the dumbbell tumors differ from others. Minimal invasive surgery and video assist thoracoscopy surgery are of special value in treatment of the selected neurogenic tumors of the mediastinum. Benign neurogenic tumors rarely recur after complete resection, and malignant neurogenic tumors have poor prognosis.

[The trend of injury epidemic in Ningxia].

OBJECTIVE: To study the epidemiological transition of injuries in Ningxia. METHODS: Demographic and mortality data during 1990s in Ningxia were collected and analyzed for temporal trend with linear regression model. RESULTS: During the past decade, mortality of injury raised by 28.22% among the residents in Ningxia and ranked the 3rd place in the causes of deaths. In 1999, the top four leading causes of injury deaths were traffic accident, suicide, drowning and poisoning. Overall injury mortality and mortalities of traffic accident and suicide increased, but mortality of drowning decreased during the past decade. As predicted, suicide will rank the 1st place in the causes of deaths among the residents in Ningxia by 2005. CONCLUSIONS: Injury was one of major risk factors influencing health and death among the residents in Ningxia, with a temporal trend in its incidence. Effective research, prevention and control of traffic accident, suicide, drowning and poisoning should be focused.

[Relationship of expression unbalance of matrix metalloproteinase and tissue inhibitor of metalloproteinase to invasiveness and metastasis in gastric carcinomas].

BACKGROUND & OBJECTIVE: Matrix metalloproteinase-9 (MMPs) and its tissue inhibitor of metalloproteinase-1 (TIMPs) have the important regulatory roles in the processes during tumor invasion and metastasis. Observing the coexpression of matrix metalloproteinase 9(MMP-9) and tissue inhibitor of metalloproteinase 1(TIMP-1) in gastric carcinomas, this study was designed to investigate the relationship of the expression unbalance of MMP-9 and TIMP-1 to the invasiveness, metastasis, and prognosis in gastric carcinomas. METHODS: By using immunohistochemistry, the expression of MMP-9, TIMP-1, and proliferating cell nuclear antigen Ki-67 was detected in 256 gastric cancers, and the patients were followed-up. RESULTS: The expressive degree of MMP-9 in the tumor invasion beyond musculariis propria(70.13%) was higher than that within lamina propria or submucosa(42.50%, P < 0.05) and its expression in tumor was positively associated with tumor cell proliferation index and lymph node metastasis, and its expression in high TNM stage(75.00%) was higher than those in low TNM stage(46.15%, P < 0.05). The relation of the expression of TIMP-1 to the depth of invasion, lymph node metastasis, and TNM stage was negative(Pearson contingency coefficient were 0.1688, 0.3556, and 0.3004, respectively P < 0.05). There were four groups of the unbalance expression of MMP-9 and TIMP-1 in gastric carcinomas. While the frequency of the serosa invasion and lymph node metastasis in the group of the expression of MMP-9 over TIMP-1 was significantly higher than those in the groups of the expression of TIMP-1 over MMP-9 or the balance expression of MMP-9 and TIMP-1. However, prognosis of the patients with the expression of MMP-9 over TIMP-1 was significantly worse than that with expression of TIMP-1 over MMP-9(P < 0.05). CONCLUSION: The expression of MMP-9 was positively related to the tumor invasiveness and metastasis in gastric cancers, however the association between the positive expression of TIMP-1 and the invasiveness and metastasis was negative. The relationship of MMP-9 and TIMP-1 to the invasiveness and metastasis in gastric carcinomas was that MMP-9 promotes and TIMP-1 inhibits tumor metastasis. The patients with MMP-9 positive expression and TIMP-1 negative expression are poorer prognosis.

[Effect of acrosin inhibitor KF-950 on acrosin activity and acrosome of human sperm].

OBJECTIVES: To study the inhibitory effect of KF-950 on human acrosin and sperm acrosome. METHODS: Human acrosin was extracted and purified with 2% acetic acid, and its residual activity was evaluated by BAEE/ADH assay after treated with different concentrations of KF-950. ABC assay was used to observe the effect of KF-950 on human acrosome with Biotin-PSA as a probe. RESULTS: 1. The activity of normal sperm acrosin was (37.65 +/- 4.47) U/L. 2. The residual activity was inversely related to the concentration of KF-950 (r = -0.998), and had a dose-response curve. The result could be described by Y = 7.57-1.895X. 3. With increase of KF-950 concentration and prolongation of action time, the staining rate of acrosome obviously dropped (P < 0.01). CONCLUSIONS: KF-950 directly inhibits acrosin activity and assumely injures sperm acrosome. It might be a new kind of highly effective inhibitor.

[Expression of DPC4/Smad4, p21wafI, and p16 in human pancreatic cancer].

BACKGROUND & OBJECTIVE: DPC4/Smad4 inactivation was detected in almost half of the pancreatic carcinomas result in the loss of inhibition of the tumor cell proliferation. p21wafI is the downstream target gene of Smad4 while DPC4 and p16 may synergistically play a role in the development of pancreatic carcinoma. By studying the expressions of DPC4/Smad4, p21wafI, and p16. This study was designed to explore the mutual relationship among them and the possible mechanism in human pancreatic carcinoma. METHODS: Immunohistochemistry was used to detect the expression of Smad4, p21wafI, and p16 in fifty-six samples of paraffin embedded human pancreatic cancer tissue, and in situ hybridization, immunohistochemistry, Western blotting technique were used to detect the expression of DPC4/Smad4 in five human pancreatic adenocarcinoma cell lines. RESULTS: The positive rate of Smad4, p21wafI, and p16 in paraffin embedded human pancreatic cancer tissue was 58.93%, 48.21%, and 42.86%, respectively, whereas the positive rate of these proteins in matched normal tissue was 89.29%, 87.5%, and 76.79% respectively. Three out of five pancreatic adenocarcinoma cell lines (P3, P4, and P7) were positive for DPC4/Smad4 with in situ hybridization, immunohistochemistry and Western blotting, while the other two lines were all negative. There is statistically significant difference between cancer and normal tissue (P < 0.05). In pancreatic adenocarcinomas, the expression of Smad4 was related to that of p21wafI (P < 0.05), and so was the expression of Smad4 to that of p16 (P < 0.05). But no correlation was found between p21wafI and p16 (P > 0.05). CONCLUSION: The expression of Smad4, p21wafI, and p16 significantly decreased in pancreatic cancer compared with normal tissue. The decreased expression of the proteins may play an important role in the development of pancreatic cancer.

Induction of apoptosis in mesothelioma cells by antisurvivin oligonucleotides.

Malignant pleural mesothelioma is a rare and aggressive tumor characterized by rapid progression, late metastases, and poor prognosis. In this study, we investigated the expression of survivin, a member of the inhibitors of apoptosis protein gene family, in mesothelioma and an antisense oligonucleotide-based gene therapy for mesothelioma using survivin as a target. Initially, we documented the expression of survivin in human mesothelioma cell lines and fresh tissues using reverse transcription-PCR and Western blot analysis. Our results showed that survivin was overexpressed in 7 of 8 (87.5%) mesothelioma cell lines assayed and in all (12 of 12; 100%) freshly resected mesothelioma tissues analyzed. To investigate the use of survivin as a therapeutic target on mesothelioma, we carried out transfections with antisurvivin oligonucleotides to induce apoptosis in mesothelioma cell lines MS-1 and H28. Results from cellular transfection and subsequent analysis using the flow cytometry demonstrated that antisurvivin oligonucleotides induced significantly greater apoptosis rates in the survivin-positive mesothelioma cell line H28 (42.5%) as compared with the control oligonucleotides (16.2%; P < 0.001). The survivin-negative cell line LRK1A (survivin-/-) did not apoptose with antisense oligonucleotides. Furthermore, time course evaluation by Western blot analysis showed that survivin was inhibited by antisurvivin oligonucleotides within 12 h after transfection. Our results show, for the first time, that survivin, an inhibitors of apoptosis protein family gene member, is highly overexpressed in malignant pleural mesothelioma. Down-regulation of survivin by a targeted antisense oligonucleotide appears to be an effective gene therapy approach to the treatment of mesothelioma.

[Relationship of vascular endothelial growth factor with bile acid in intrahepatic cholestasis of pregnancy].

OBJECTIVE: To investigate the relationship of vascular endothelial growth factor (VEGF) with bile acid in intrahepatic cholestasis of pregnancy patients (ICP). METHODS: Thirty three ICP patients and 30 normal pregnant women in late pregnancy were studied, maternal serum VEGF concentrations were examined by enzyme linked immunoadsorbent assay (ELISA). Capillaries density in placenta are detected by s-ABC immunohistochemistry, radio-immunity method is used to assay the maternal serum glycocholic acid. RESULTS: Serum VEGF in ICP (2 072 +/- 17) ng/L has negative correlation with glycocholic acid (47.9 +/- 14.2) ng/L, however, positive correlation with capillaries density in placenta (68 +/- 5)/HP; The serum VEGF in normal pregnant woman (2 256 +/- 30) ng/L does not have relationship to glycocholic acid (1.6 +/- 0.7) ng/L but it has positive correlation with capillaries density in placenta (74 +/- 7)/HP. CONCLUSION: Cytotoxicity of high concentration bile acids impairs vascular endothelial cells resulting in lower excretion of VEGF in vascular endothelial cells of ICP, which lead to placental blood capillaries dysplasia and lower permeability, it may be one of reasons of fetal distress and fetal death in uteri in ICP.

[Research on the separation of caffeine and its nine analogues by micellar electrokinetic capillary chromatography].

The separation of caffeine and its nine analogues by micellar electrokinetic capillary chromatography (MECC) using sodium dodecyl sulphate (SDS) as the micellar phase was investigated. The effects of pH and concentration of phosphate buffer solution, SDS micelle concentration, methanol volume fraction, applied voltage and temperature on the separation were studied. It was found that the migration of these compounds was affected by these factors, especially by pH of the solution. The order of elution, as well as the migration time and separation efficiency of these compounds changes with the acidity of the solution, which suggests the disassociation of the hydrogen on the secondary amino group in these compounds. The optimized separation conditions consisted of a running buffer of 20 mmol/L sodium phosphate buffer, at pH 11.0, containing 20 mmol/L SDS, with an applied voltage of 25 kV and a temperature of 25 degrees C. All of the compounds were resolved within 9 min with the detection limits of 0.70 mg/L and the linearity in the range of 1.40 mg/L-45.5 mg/L with the correlation coefficients of 0.998-0.999.

[Cloning and sequencing of fragments associated with cytoplasm male sterility of rice].

Mitochorndrial DNA (mtDNA), nuclear DNA (nDNA), and genomic DNA (gDNA) were individually extracted from Zhenshan 97A, Xieqingzao A and A-23, which were cytoplasmic male sterile (wild abortive, dwarf abortive and BT), and from Zhenshan 97B, Xieqingzao B and BT, which were maintainers lines. Each of them was analyzed with 500 random primers by RAPD. Six fragments specific to male sterile lines were amplified. A fragment, PWH-17, associated with wild and dwarf abortive type cytoplasmic male sterility was analyzed by Southern blotting and sequencing, and also tested by SCAR. It was 1879 bp in length, which contained 6 open reading frames and 8 repeated sequences. BLAST search revealed that partial sequence in PWH-17 showed high similarity to the upstream sequence of tRNA-Asp gene in Elytrigia elongate (GenBank accession number U14335) and wheat (X13243). The similarity ratios were 97% and 100%. It was inferred that the possible position of PWH-17 in rice (wild abortive) could be located between tRNA-Asp and Cox II, which will be discussed further.

Assessment of therapeutic efficacy of chloroquine against falciparum malaria in an outbreak area in Xishuangbanna, Yunnan.

OBJECTIVE: To investigate the failure of treatment with chloroquine in Yunnan in order to help formulate adequate antimalarial drug policy. METHODS: A World Health Organization 28-day in vivo test on therapeutic response for uncomplicated falciparum malaria in area with low or moderate transmission was adopted. Patients of age > or = 6 months old were admitted without limitation in density of parasitaemia and body temperature. Clinical and parasitological observation was conducted for patients on day 0, 1, 2, 3, 4, 7, 14, 21, 28 and 35. RESULTS: Of 62 patients identified as malaria cases infected by Plasmodium falciparum only, Plasmodium vivax only or by both species, 52 cases infected by Plasmodium falciparum only were included in the study. The overall treatment failure rate was 40.7%, with early treatment failure (ETF) rate of 1.8% and late treatment failure rate (LTF) of 38.9%. CONCLUSION: The treatment failure rate was much higher than the rate of 25% recommended by WHO. It is suggested that use of single chloroquine should be stopped in the treatment of falciparum malaria cases in such area. No relationship was found between the failure rate and the density of malaria parasites.
