[Surgical repair of major vascular injuries in the extremities complicated by other injuries: retrospective analysis of 67 cases].

OBJECTIVE: To retrospectively analyze the surgical repair of injured major vessels in the extremities complicated by multiple other injuries. METHODS: According to different time lengths of sustaining ischemia, sites of the major vascular injuries and conditions of other injuries, 91 operations for end-to-end anastomosis, vessel grafting and repair, ligation and shunting respectively were performed on the 67 patients. RESULTS: Except for 2 fatal cases (one due to preoperative brain hernia and hemorrhagic shock, the other to thyroid crisis during operation), all other cases survived the operations. Vascular crisis occurred in 3 cases (2 arterial spasm and 1 venous crisis) 6, 7 and 12 h respectively after the operations, which were relieved by appropriate treatments and surgical explorations. One patient developed pneumonia and acute renal failure with polyuria at postoperative days 7 and 4 respectively, but full recovery was achieved. CONCLUSIONS: For patients with major vascular injuries in the extremity complicated by multiple other injuries, timely revascularization of the extremities should be performed during or immediately after emergency operations of the vital organs. Particular attention should be given to the diagnosis and repair of closed major vascular injuries in such patients, who need to be monitored for blood coagulation during operation and should receive due post-operative anti-coagulation treatment.

[Studies on molecular epidemiology of Y chromosome azoospermia factor microdeletions in Chinese patients with idiopathic azoospermia or severe oligozoospermia].

OBJECTIVE: To determine microdeletion loci and the characteristic of Y chromosome azoospermia factor(AZF) associated with Chinese idiopathic azoospermia or severe oligozoospermia and hence to provide a theoretic basis for gene diagnosis of AZF microdeletion in Chinese infertility men. METHODS: The subjects of this study included 134 cases of azoospermia and 118 severe oligozoospermia, and 210 healthy male controls. With multiplex PCR technique, an analysis of 15 sequence tag sites(STS) in AZFa, AZFb and AZFc microdeletion was performed. RESULTS: No microdeletion was detected in controls whereas 13 STS loci microdeletions existed in cases, including sY84 and sY86 in AZFa; sY121, sY123, sY124, sY127, sY134 and sy133 in AZFb; sY152, sY242, sY254, sY255 and sY157 in AZFc. Five azoospermia patients were involved in AZFa microdeletions, 7 azoospermia and 3 severe oligozoospermia patients in AZFb, and 14 azoospermia and 18 severe oligozoospermia patients in AZFc. The prevalence rates of microdeletion in AZFa, AZFb and AZFc were 2.0%,4.0% and 12.7% respectively. The microdeletion rate showed statistically significant difference between the cases and controls. CONCLUSION: This study revealed an association between the microdeletion of 13 STS loci of AZF region and the idiopathic azoospermia or severe oligozoospermia in Chinese. There is no evidence to identify the genetic polymorphism of the above STS loci. The frequency and distribution characteristic of AZF microdeletion are similar to those of Caucasians. As candidate loci the 13 STS are useful in clinical gene diagnosis for the detection of AZF microdeletion in Chinese idiopathic azoospermia and severe oligozoospermia.

[An experimental study of the effects of profenofos on antioxidase in rabbits].

In order to explore the effects of superoxide dismutase(SOD), catalase(CAT), glutathione-peroxidase (GSH-Px) in rabbits exposed by profenofos and its meaning. 18 rabbits were divided into three groups randomly: A group (high-dose group), B group(low-dose group), C group(control group), each group including 6 rabbits. The activities of SOD, CAT, GSH-Px in plasma, and cholinesterase(ChE) in blood were measured in different exposure period. The results showed that activities of SOD, CAT and GSH-Px increased, ChE decreased markedly compared with those in the same group before experiment and control group (P < 0.05, P < 0.01). The increase of antioxidase activities was earlier than the decease of ChE activity. It suggests that profenofos can result in the increases of the activities of SOD, CAT and GSH-Px, which may be earlier diagnostic index in profenofos poisoning.

[Retrospective analysis of 75 cases of flap transplantations in children].

OBJECTIVE: To investigate the therapeutic effect of flap transplantation in repairing soft tissue defects of children. METHODS: From January 1997 to May 2002, 75 cases of different soft tissue defects (52 males and 23 females, with the age of 3-14 years) were repaired by axial and non-axial flaps transfer, and axial flaps transplantation by microvascular anastomosis. The flaps area ranged from 3 cm x 5 cm to 15 cm x 42 cm. Emergency operation was performed in 26 cases and secondary operation in 49 cases (infective wound such as osteomyelitis and plate extra-exposed of fracture). The defect regions included the forearm, back of the hand, thumb, index finger, leg and foot. The types of flap graft and application range included 39 cases of axial flaps transfer or transplantation (27 cases of along- or contra-transfer of transplantation and 12 cases of microvascular anastomosis). The non-axial flaps transfer were designed along- or contra-transfer near the wound area in 36 cases. The ratio of length to width was 2.5:1-3.5:1 in 27 cases, and larger than 3.5:1 in 9 cases. Adequate anesthesia method should be chosen according to the characteristics of children, non-traumatic operating during surgery and postoperative supervision and nursing of flaps should also be paid enough attention. RESULTS: After operation, blood circulation crisis occurred in 2 cases (1 case of artery failure and 1 case of vein failure). The flaps survived in 37 cases and partially survived in 1 case and necrosed in 1 case. The survival rate was 96.2%. The postoperative follow-up period was 3 to 60 months, the blood supply, elasticity and texture of flaps were good. The effect of repair was satisfactory. CONCLUSION: Different types of transplantation of blood-supply of flaps may repair the different types of soft tissue defects in children. Free flap transplantation is safe and beneficial in children, different defects of soft tissue were repaired by axial and non-axial flaps transfer, axial flaps transplantation by microvascular anastomosis. Non-traumatic operating and postoperative supervision and nursing of flaps should also be paid enough attention.

[Characterization of novel alleles and new mutation sites of porcine MHC class II DQA].

The primers, DQAp161 and DQAp443, were designed based on the homologous region of SLA-DQA cDNA sequences and HLA-DQA genomic sequences. The 731 bp fragment of SLA-DQA including completed intron 2, the near completed exon 2 and partial exon 3 was obtained by PCR. The nucleotide sequences of the fragment of SLA-DQA were obtained with cloning and direct sequencing. Both nucleotide sequences of exon 2 and amino acid sequences of alpha 1 domain were analyzed in a pedigree. The sequence data were compared with all sequences of SLA-DQA exon 2 in GenBank. Two novel alleles, DQA-SLT 26 and DQA-TC 21-1, were found according to the above analyses. Four amino acid changes were observed among SLA-DQA haplotype c, d and DQA-SLT 26. They were Val-->Ala(60), Lys-->Glu(65), Asp-->Gly(81) and Lys-->Ile(93). Comparing the amino acids sequence of the all SLA-DQA sequences with DQA-TC 21-1 revealed that the His (94) was changed into Tyr.

Study on the correlation of cytokine gene polymorphism with chronic periodontitis.

OBJECTIVE: To study the correlation of gene polymorphism IL-1B +3953 and TNF-A-308 and the genetic susceptibility to moderate-to-advanced adult periodontitis(AP). METHODS: PCR-RFLP technique was used to identify IL-1B +3953 and TNF-A-308 genotype. Multivariate logistic regression model was utilized to assess the relationship of IL-1B +3953 and TNF-A-308 positive genotype to disease status while adjusting for other potential confounders. RESULTS: The frequency of IL-1B +3953 (28.23%) and TNF-A-308 (29.03%) allele 2 positive genotype in patients with moderate-to-advanced AP showed a statistically significant increase compared to the control group frequency (13.95% IL-1B +3953 and 11.05% TNF-A-308), and the difference was of much significance (P < 0.01 in both group). Multivariate logistic regression analysis showed that IL-1B +3953 and TNF-A-308 allele 2 positive genotype may increase the odds of having moderate-to-advanced AP and the IL-1B +3953 (P < 0.05) and TNF-A-308 (P < 0.01) allele 2 positive genotype was of statistical significance. CONCLUSIONS: IL-1B +3953 and TNF-A-308 allele 2 positive genotype have closer correlation with moderate-to-advanced AP.

[Study on polymorphism of human leukocyte antigen-DRB1 allele in patients with endometriosis].

OBJECTIVE: To study the possibility of a correlation between occurrence of endometriosis (EM) and polymorphism of human leukocyte antigen (HLA) DRB1 allele. METHODS: Polymorphism of HLA-DRB1 allele were detected by polymerase chain reaction- sequence specific primers (PCR-SSP) testing 40 surgically proven EM patients and 50 normal control. RESULTS: At HLA-DRB1 15 allele, positive frequency of EM group was significant higher as compared with those of the normal control group [45.0% vs 26.0%, P < 0.05, relative risk (RR) = 2.1]. Other HLA-DRB1 allele positive frequency of EM group and control group had no significant difference. But for Polymorphism of DRB1 * 15 allele, positive frequency of EM group and control group had no significant difference [1501 : 35.0% vs 20.0%; 1502 : 10.0% vs 6.0%; P > 0.05]. CONCLUSIONS: The occurrence of EM may be associated with the presence of HLA-DRB1 allele, but hasn't associated about polymorphism of DRB1* 15 allele. HLA-DRB1 * 15 maybe one of some factors for the pathogenesis of endometriosis.

Involvement of endothelin-1 in acute ischaemic arrhythmias in cats and rats.

This study aims to investigate the role of endothelin-1 (ET-1) in the genesis of acute ischaemic arrhythmias. In anaesthetized cats and rats receiving continuous monitor of electrocardiogram and arterial blood pressure, the ischaemic arrhythmias during 60-min myocardial ischaemia elicited by the occlusion of the left anterior descending coronary artery (LAD) were analysed. To prevent the putative arrhythmic effects of endogenous ET-1, ET(A) receptor antagonist BQ610 (1.5-6.0 nmol/kg) was intracoronary injected just before LAD occlusion in cats, and preproET-1 mRNA antisense oligodeoxynucleotide (AS-ODN; 30-90 nmol/kg) was intravenously injected 2 h before LAD occlusion in rats. The results showed that BQ610 dose-dependently decreased the incidences of ventricular tachycardia (VT) and ventricular fibrillation (VF), and the numbers of ventricular ectopic beats (VEBs). At the dose of 6.0 nmol/kg, the incidence of VT decreased significantly from 33.3% in normal saline (NS) control group to zero (P<0.01), and total VEBs decreased significantly from 831+/-162 to 158+/-51 (P<0.05). In rats receiving ET-1 AS-ODN, plasma ET-1 decreased significantly after 2 h, and remained stable at 30 min of LAD occlusion. However, in rats receiving the control, NS or sense ODN, plasma ET-1 remained unchanged after administration, but increased significantly during LAD occlusion. The ischaemic arrhythmias were dose-dependently suppressed in the presence of ET-1 AS-ODN. At the dose of 90 nmol/kg, the incidence of VT decreased significantly from 100% in both the control groups to 30%. The numbers of single VEBs, consecutive VEBs, VT and total VEBs were also significantly decreased, from 60+/-15 in NS group to 19+/-12, 11+/-3 to 2+/-2, 155+/-41 to 11+/-11, and 239+/-49 to 35+/-25 respectively. In the present cat and rat models of coronary artery occlusion, antagonism of either ET(A) receptors or endogenous ET-1 synthesis prevented the ischaemic arrhythmias, indicating that ET-1 is possibly an important promotive factor in the genesis of acute ischaemic arrhythmias.

Effects of different preproendothelin-1 mRNA anti-sense oligodeoxynucleotides on ischemic arrhythmias in rats.

The effects of four anti-sense oligodeoxynucleotides (AS-ODNs) against rat or human preproendothelin-1 mRNA on ischemic arrhythmias in anesthetized rats were studied. AS-ODN (60 nmol/kg) or control (normal saline; sense-ODN, and scrambled-ODN, 60 nmol/kg) was injected 2 h before acute myocardial ischemia elicited by the occlusion of the left anterior descending coronary artery. Arrhythmias during 60-min ischemia were assessed, and plasma endothelin-1 was determined with an endothelin-1-specific radioimmunoassay system. The results showed that anti-senses against human preproendothelin-1 mRNA were anti-arrhythmic without significant impact on hemodynamics, whereas two against rat preproendothelin-1 mRNA and the three controls failed to be anti-arrhythmic. In human antisense groups, both the incidence of reversible ventricular fibrillation and the mortality were decreased to zero. The incidences of ventricular tachycardia and salvos were significantly decreased from almost 100% in the controls to < or =30% (p < 0.01), the arrhythmia score from an average of approximately 3.6 to 0 and 0.7, respectively (p < 0.01 versus controls), and the total ventricular ectopic beats from an average of 307-338 to < 40 (p < 0.01). The human AS-ODNs led to less plasma endothelin-1, which was associated with suppressed ischemic arrhythmias in this rat model, indicating a contributory role of endothelin-1 in ischemic arrhythmias. Conversely, considering the two- or three-base mismatches between the human AS-ODNs and rat preproendothelin-1 mRNA, and the failure of the rat AS-ODNs in suppressing arrhythmias, the possibility could not be excluded that human endothelin-1 AS-ODNs acted via an undetermined pathway other than endothelin-1.

[Microsurgical repair of syndactyly of the fingers in children].

OBJECTIVE: To evaluate the effectiveness of microsurgical technique for repairing syndactyly of the fingers in children. METHODS: Microsurgical repair of syndactyly of the fingers was performed in 32 children. The skin joining the syndactyly was incised and relaxed under microscope, and dorsal metacarpal flap of comparable size was used to repair the lateral skin defect of the finger and also to reconstruct the finger web. RESULTS: All the flaps survived without scar leaving on the lateral skin of the fingers, and the reconstruction of the finger web was satisfying. CONCLUSION: Microsurgical technique is applicable in the surgical repair of syndactyly of the fingers in children.

[Descriptions and microscopy identification of Citrus grandis var. tomentosa Hort. and Citrus grandis Osbeck].

OBJECTIVE: To identify Citrus grandis var. tomentosa Hort. and Citrus grandis Osbeck. METHODS: Descriptions and microscopy identification. RESULTS: There are nonglandular hair in the stem epidermis, many starch grains in the stem cortex and many prismatic crystals of calcium oxalate in the phloem of C. grandis var. tomentosa Hart. The palisade tissues of the leave are thick. The outer layer palisade tissues contains prismatic crystals. The cortex of leaf vein is thick. The other characters are similar to C. grandis Osbeck. CONCLUSION: Above-mentioned characters can be used to identify C. grandis var. tomentosa Hort. and C. grandis Osbeck.

[Analysis of expansion degree of seeds of Plantaginis and their processed products].

OBJECTIVE: To explore the affection factors to expansion degree of seeds of Plantaginis caused by different processing methods. METHOD: Determination of expansion degree and analysis by gel chromatography. RESULT: The values of expansion degree were decreased and the structures of polysaccharides were changed for the processed products compared with their crude seeds. CONCLUSION: The polysaccharies in seeds of Plantaginis were degraded after toasted without additive and roasted with salt, which further affected the expansion degree.

Functional changes in children, adolescents, and young adults with cerebral palsy.

Children with multiple handicaps, including cerebral palsy (CP), often lose or regress in their functional ability through adolescence and adulthood. The purpose of this study was to examine functional and psychological changes in children, adolescents and young adults with CP. A retrospective chart review and a prospective telephone interview of 30 patients (11-29 years, M = 16.8, S.D. = 4.9) was conducted. Seventy-three percent of the patients were male (n = 22) and 83% (n = 25) had spastic CP. According to the McNemar's test, four significant functional losses were found including crawling (p = 0.03), standing independently (p = 0.05), walking with or without assistance (p = 0.014), and eating by mouth (p = 0.01). Standing function loss was significantly related to walking function loss (p = 0.02). Sixty-three percent (n = 19) of the patients experienced anxiety and 10% (n = 3) reported depression. Results of this study indicate that a validated yearly assessment tool is needed to measure functional and emotional changes in children with CP rather than relying on parent recall. This data may also lead to a review of the current physical therapy national standards.

Transient modeling in simulation of hospital operations for emergency response.

Rapid estimates of hospital capacity after an event that may cause a disaster can assist disaster-relief efforts. Due to the dynamics of hospitals, following such an event, it is necessary to accurately model the behavior of the system. A transient modeling approach using simulation and exponential functions is presented, along with its applications in an earthquake situation. The parameters of the exponential model are regressed using outputs from designed simulation experiments. The developed model is capable of representing transient, patient waiting times during a disaster. Most importantly, the modeling approach allows real-time capacity estimation of hospitals of various sizes and capabilities. Further, this research is an analysis of the effects of priority-based routing of patients within the hospital and the effects on patient waiting times determined using various patient mixes. The model guides the patients based on the severity of injuries and queues the patients requiring critical care depending on their remaining survivability time. The model also accounts the impact of prehospital transport time on patient waiting time.

Impact of mineral fouling on hydraulic behavior of permeable reactive barriers.

This paper describes reactive transport simulations conducted to assess the impact of mineral fouling on the hydraulic behavior of continuous-wall permeable reactive barriers (PRBs) employing granular zero-valent iron (ZVI) in carbonate-rich alluvial aquifers. The reactive transport model included a geochemical algorithm for simulating corrosion and mineral precipitation reactions that have been observed in ZVI PRBs. Results of simulations show that porosity and hydraulic conductivity of the ZVI decrease over time and that flows are redistributed throughout the PRB in response to fouling of the pore space. Under typical conditions, only subtle changes occur within the first 10 years (i.e., duration of the current field experience record with PRBs), and the most significant changes do not occur until the PRB has operated for at least 30 years. However, changes can occur sooner (or later) if the rate at which mineral-forming ions are delivered to the PRB is higher (or lower) than that expected under typical conditions (i.e., due to higher/lower flow rate or inflowing ground water that has higher/lower ionic strength). When the PRB is more permeable than the aquifer, the median Darcy flux in the PRB does not change appreciably over time because the aquifer controls the rate of flow through the PRB. However, seepage velocities in the PRB increase, and residence times decrease, due to porosity reductions caused by accumulation of minerals in the pore space. When fouling becomes extensive, bypassing and reductions in flow rate in the PRB occur.

Characterization of 35 novel microsatellite DNA markers from the duck (Anas platyrhynchos) genome and cross-amplification in other birds.

In order to study duck microsatellites, we constructed a library enriched for (CA)n, (CAG)n, (GCC)n and (TTTC)n. A total of 35 pairs of primers from these microsatellites were developed and used to detect polymorphisms in 31 unrelated Peking ducks. Twenty-eight loci were polymorphic and seven loci were monomorphic. A total of 117 alleles were observed from these polymorphic microsatellite markers, which ranged from 2 to 14 with an average of 4.18 per locus. The frequencies of the 117 alleles ranged from 0.02 to 0.98. The highest heterozygosity (0.97) was observed at the CAUD019 microsatellite locus and the lowest heterozygosity (0.04) at the CAUD008 locus, and 11 loci had heterozygosities greater than 0.50 (46.43%). The polymorphism information content (PIC) of 28 loci ranged from 0.04 to 0.88 with an average of 0.42. All the above markers were used to screen the polymorphism in other bird species. Two markers produced specific monomorphic products with the chicken DNA. Fourteen markers generated specific fragments with the goose DNA: 5 were polymorphic and 9 were monomorphic. But no specific product was detected with the peacock DNA. Based on sequence comparisons of the flanking sequence and repeat, we conclude that 2 chicken loci and 14 goose loci were true homologous loci of the duck loci. The microsatellite markers identified and characterized in the present study will contribute to the genetic map, quantitative traits mapping, and phylogenetic analysis in the duck and goose.
