Immersive virtual reality-assisted anatomy training improves endotracheal intubation performance in simulation: a randomized controlled trial among Chinese non-anesthesiology residents.

INTRODUCTION: This study aimed to compare immersive virtual reality (IVR)-assisted versus conventional anatomy training for teaching endotracheal intubation (ETI) to novice non-anesthesiology residents enrolled in China's Standardized Residency Training program. METHODS: A total of 90 non-anesthesiology residents without prior ETI experience were randomly assigned to either an IVR group receiving IVR-assisted anatomy training (n&#x2009;=&#x2009;45) or a control group receiving conventional anatomy training (n&#x2009;=&#x2009;45). All participants underwent a standardized teaching protocol. The primary endpoint was residents' ETI performance on a simulator, assessed using both the Global Rating Scale (GRS) and a task-specific checklist. The secondary endpoints included changes in written multiple-choice question (MCQ) scores and residents' evaluations of the course. RESULTS: In practical ETI assessments on a manikin, the IVR group achieved significantly higher scores on the task-specific checklist than the control group (90.34&#x2009;&#xb1;&#x2009;2.89 vs. 87.20&#x2009;&#xb1;&#x2009;3.29; p&#x2009;<&#x2009;0.001), whereas GRS scores were comparable between groups. Both groups showed significant post-training improvement in knowledge scores (p&#x2009;<&#x2009;0.001), with the IVR group showing a greater gain in theoretical knowledge (54.0% vs. 36.3%; p&#x2009;<&#x2009;0.001). Participants in the IVR group also expressed a stronger preference for their training method (80.8%) and reported higher levels of motivation, confidence, and enjoyment (all p&#x2009;<&#x2009;0.05). CONCLUSION: IVR-assisted anatomy training enhances the effectiveness of ETI training for novice non-anesthesiology residents, offering an interactive, engaging, and reproducible approach within China's Standardized Residency Training framework.

Discovery and synthesis of new immunosuppressive alkaloids from the stem of Fissistigma oldhamii (Hemsl.) Merr.

Three new alkaloids (1-3) and twenty-one known compounds were isolated from the stem of Fissistigma oldhamii (Hemsl.) Merr. which was the ruler herb in an approved Traditional Chinese herbal formula used for treatment of rheumatoid arthritis in China and synthesis of one new immunosuppressive alkaloid was achieved. These compounds, including the crude extracts of this herb, exhibited strong activities in the inhibition of T and B cell proliferation.

Ursolic acid and its derivative inhibit protein tyrosine phosphatase 1B, enhancing insulin receptor phosphorylation and stimulating glucose uptake.

Protein tyrosine phosphatase 1B (PTP1B) is a key element in the negative regulation of the insulin signaling pathway and may play an important role in diabetes and obesity. We identified ursolic acid, a natural pentacyclic triterpenoid that occurs widely in traditional Chinese medicinal herbs, as an inhibitor of PTP1B by screening an extract library of the traditional Chinese medicinal herbs used a diabetes clinic. By modifying urosolic acid, we designed and synthesized a derivative with a K(i) of 283 nM. As competitive inhibitors of PTP1B, ursolic acid and its derivative also inhibit T-cell protein tyrosine phosphatase and src homology phosphatase-2 but not leucocyte antigen-related phosphatase or protein tyrosine phosphatase alpha and epsilon, which are all possibly involved in the insulin pathway. The ursolic acid derivative enhanced insulin receptor phosphorylation in CHO/hIR cells and stimulate glucose uptake in L6 myotubes.

Daturametelins H, I, and J: three new withanolide glycosides from Datura metel L.

Three new withanolide glycosides named daturametelins H-J (1-3), together with two known ones, daturataturin A (4) and 7,27-dihydroxy-1-oxowitha-2,5,24-trienolide (5), were isolated from the MeOH extract of the aerial parts of Datura metel L. (Solanaceae). Their structures were determined mainly by spectroscopic techniques including 2D-NMR (HMBC, HMQC, (1)H,(1)H-COSY, NOESY) and MS experiments. Compounds 1-5 were tested for their antiproliferative activity towards the human colorectal carcinoma (HCT-116) cell line. The nonglycosidic compound 5 exhibited the highest activity of the tested withanolides, with an IC(50) value of 3.2+/-0.2 microM (Table 3).

Nine new dammarane saponins from Gynostemma pentaphyllum.

Nine new dammarane saponins, 1-9, were isolated from the MeOH extract of the aerial part of Gynostemma pentaphyllum, an edible plant of the Cucurbitaceae family, that grows wildly in China. Their structures were elucidated by 1D- and 2D-NMR analysis, as well as by chemical degradation. Aglycons 21,24-cyclopentyldammar-25-ene of 6 and 7, and 21,24-cyclopentyldammarane of 8 and 9, were novel. Compounds 1, 4, and 8 showed moderate antitumor activities against stomach cancer cells SGC-7901 and liver cancer cells BEL-7402.

A zwitterionic alkaloid, containing a rare cyclopentadienyl anion unit, from the stem barks of Daphniphyllum macropodum Miq.

A new Daphniphyllum alkaloid daphnicyclidin L (1), containing a rare cyclopentadienyl anion, which is stabilized as a zwitterion by an internal iminium counterion, was isolated from the stem barks of Daphniphyllum macropodum Miq., together with four known alkaloids: daphnicyclidin D (2), daphnicyclidin H (3), deoxyyuzurimine (4), and yuzurimine (5). The structures were elucidated on the basis of spectroscopic data. The configuration of 1 was elucidated by single-crystal X-ray diffraction crystallography.

Cassane diterpene-lactones from the seed of Caesalpinia minax HANCE.

Five new neocaesalpins, named neocaesalpin J-N (1-5, resp.), along with the known neocaesalpin A and eight known furanditerpenoids, namely epsilon-caesalpin, 7-acetoxy-epsilon-caesalpin, 14-deoxy-epsilon-caesalpin, caesalmins D-F, and bonducellins C and D, were isolated from the seeds of Caesalpinia minax HANCE. Their structures were determined on the basis of spectroscopic analyses.

Sexually transmitted infections among pregnant women attending an antenatal clinic in Fuzhou, China.

OBJECTIVE: To determine the prevalence of sexually transmitted infections (STIs) among women attending an antenatal clinic in urban China and to show whether reported symptoms and findings on clinical examination predicted STIs in this population. STUDY DESIGN: Cross-sectional descriptive study was conducted among 504 pregnant women attending an antenatal clinic in Fuzhou, China. Structured questionnaires were used to collect demographic and behavioral information, and clinical and gynecologic examinations were performed to detect clinical signs of STIs. Blood samples, vaginal swabs, and cervical swabs were collected, respectively, to test for antibodies to syphilis, culture Trichomonas vaginalis (T. vaginalis), and perform PCR to detect Chlamydia trachomatis (C. trachomatis) and Neisseria gonorrhoeae (N. gonorrhoeae). RESULTS: C trachomatis was detected in 51 (10.1%), N gonorrhoeae in 4 (0.8%), T vaginalis in 16 (3.2%), and syphilis in 1 (0.2%) of the 504 pregnant women. Fifty-two (73%) of 71 women with any STI were asymptomatic. Multiple logistic regression analysis indicated that age RMB 2000 yuan (OR=3.57) were significant independent risk factors for chlamydial infection (P<0.05). The reported symptom of vaginal discharge or the clinical sign of either vaginal or cervical discharge poorly predicted infection with C trachomatis, N gonorrhoeae, or T vaginalis, with a positive predictive value <25% for each STI. CONCLUSION: A substantial prevalence of STIs, including a large proportion of asymptomatic infections, was found among pregnant women in the study area. These results support a strategy of screening pregnant women for bacterial STIs (followed by treatment of infections), which could be integrated into routine pregnancy care in China.

PTP1B inhibitors from Ardisia japonica.

In bioassay-directed isolation from the whole plant of Ardisia japonica, sixteen known compounds: chrysophanol (1), physcion (2), oleanolic acid (3), euscaphic acid (4), tormentic acid (5), quercetin (6), quercitrin (7), myricitrin (8), kaempferol 3-O-alpha-L-rhamnopyranoside (9), cyclamiretin A 3-O-alpha--rhamnopyranosyl(1-->4)-beta-D-glucopyranosyl(1-->2)-[beta-D-glucopyranosyl(1-->4)]-alpha-L-arabinopyranoside (10), (7E)-9-hydroxymegastigma-4, 7-dien-3-on-9-O-beta-D-glucopyranoside (11), bergenin (12), norbergenin (13), rutin (14), kaempferol 3,7-O-alpha-L-dirhamnopyranoside (15), (-)-epigallacatechin 3-O-gallate (16) were obtained. Compounds 1-5, 9, 11 and 14-16 have not been reported previously from this plant. Among these isolates, 2, 3, 6 and 12 showed moderate bioactivity against PTP1B in vitro with IC50 values of 121.50, 23.90, 28.12 and 157 microM, respectively.

Furostanoside from Asparagus filicinus.

A new furostanoside, named aspafilioside D (1), together with the five known compounds officinalisnin-II (2), Asp-IV' (3), (+)-4'-O-methyl-nyasol (4), (+)-nyasol (5) and tormentic acid (6), have been isolated from the roots of Asparagus filicinus. The structure of 1 has been elucidated on the basis of spectral data.

Sulfonated xanthones from Hypericum sampsonii.

Xanthones, 1,3-dihydroxy-5-methoxyxanthone-4-sulfonate and 1,3-dihydroxy-5-O-beta-D-glycopyranosylxanthone-4-sulfonate, together with nine known compounds were obtained from H. sampsonii. This is the first report of sulfonated xanthonoids. Furthermore, compounds 1 and 2 exhibited significant cytotoxicity against the P388 cancer cell line.

Total synthesis and cytotoxicity evaluation of syrinenin-4-O-farnesylether and its analogues.

First synthesis of natural product, syrinenin-4-O-farnesylether (1), was carried out via two different paths. Four of its derivatives (9-12) were also prepared. Cytotoxicity screening of the selected compounds were performed on six tumour cell lines. Compound 12 exhibited prominent IC50 values of 1.9 microM and 0.8 microM on CNE and PC-3 cells, respectively.
