Posterior pole retinal thickness measurements by the retinal thickness analyzer in healthy Chinese subjects.

PURPOSE: To assess retinal thickness at the posterior pole in healthy Chinese subjects with the retinal thickness analyzer (RTA). METHODS: Retinal thicknesses at the posterior pole and fovea were measured by the RTA in 331 eyes of 178 healthy Chinese subjects. Retinal thicknesses as a function of sex, age, refractive errors, and axial length were also evaluated. RESULTS: The average thickness of the foveal area +/- SD was 158.6 +/- 24.8 microm, the average thickness of the perifoveal region (600-2,500 microm from fovea) +/- SD was 174 +/- 25.2 microm, and the average thickness of the posterior pole region (600-6,000 microm from fovea) +/- SD was 171.9 +/- 25.3 microm. There was no significant difference in retinal thicknesses of the foveal, perifoveal, and posterior pole regions in either right or left eyes or as a function of different spherical equivalents. Moreover, there was no significant difference of foveal thickness between males and females. Greater retinal thicknesses of the foveal, perifoveal, and posterior pole regions were associated with age of older than 40 years and axial length of < 24 mm, and greater perifoveal and posterior pole thicknesses were found for females. Furthermore, there were no significant correlations between age, refractive errors, and axial lengths with retinal thicknesses. CONCLUSION: Retinal thicknesses of the posterior pole region differed by age, axial length, and sex, but there was no effect of laterality or different spherical equivalents. The RTA is a fast and noninvasive technology to assess retinal conditions with thickening or thinning in the fovea and posterior pole.

Clinical characteristics and surgical outcomes of pediatric rhegmatogenous retinal detachment in Taiwan.

PURPOSE: To evaluate the clinical features and surgical and visual outcomes of rhegmatogenous retinal detachment (RRD) in the pediatric population. DESIGN: Retrospective, noncomparative, interventional case series. METHODS: A review of patients under 18 years who underwent primary retinal detachment surgery at National Taiwan University Hospital from 1989 to 2003 was conducted. RESULTS: Included in the study were 152 eyes of 146 patients (mean age of 13.1 years). Male patients comprised 69.9% of the sample. Bilateral RRD was present in 4.1%. Etiologies included myopia >4 diopters (37.5%), trauma (32.9%), developmental anomaly (11.8%), previous surgery (5.9%), previous uveitis (3.9%), atopic dermatitis (2.6%), and unknown (5.3%). Macular detachment was found in 73.0%. The most common primary treatment was scleral buckling (61.2%). Single-operation reattachment was accomplished in 58.5% of patients and eventual reattachment in 78.3% of patients occurring in a mean of 1.5 (SD = 0.9) operations. Average postoperative follow-up time was 48.3 months. Visual improvement occurred in 42.8%, remained the same in 32.2%, and worsened in 19.1%. In the logistic regression model, statistically significant risk factors for poor surgical outcome were nonmyopic RRD (P = .026), macular involvement (P = .01), and presence of proliferative vitreoretinopathy (P = .07). CONCLUSIONS: Myopia >4 diopters was the most common etiology in retinal detachment in our sample, followed by trauma. Myopia (> -4.0 diopters) may be more common than previously reported. Most eyes (78.3%) were anatomically reattached after multiple surgeries. Retinal detachment not associated with myopia is a newly identified predictor for poor surgical outcomes.

An ancova approach to normalize microarray data, and its performance to existing methods.

A microarray experiment includes many steps, and each one of them may include systematic variations. To have a sound analysis, the systematic bias must be identified and removed prior to the data being analyzed. Based on the M-A dependency observed by Dudoit et al. (2002), we suggest that, instead of using the lowess normalization, a new normalization method called ANCOVA be used for dealing with genes with replicates. Simulation studies have shown that the performance of the suggested ANCOVA method is superior to any of the available approaches with regards to the Fisher's Z score and concordance rate. We used a microarray data from bladder cancer to illustrate the application of our approach. The edge the ANCOVA method has over the existing normalization approaches is further confirmed through real-time PCR.

Prolonged survival after single lung transplantation for pulmonary hypertension secondary to systemic lupus erythematosus.

Pulmonary hypertension is one of the life-threatening complications of systemic lupus erythematosus, but these patients are often excluded from lung transplantation candidacy due to the nature of underlying multiple system involvement. We report a long-term survival after single lung transplantation in a case of systemic lupus erythematosus with severe pulmonary hypertension. It suggests that single lung transplantation may be considered in such patients, especially in condition of limited donor organ supply.
