Validating the splicing effect of rare variants in the SLC26A4 gene using minigene assay.

BACKGROUND: The SLC26A4 gene is the second most common cause of hereditary hearing loss in human. The aim of this study was to utilize the minigene assay in order to identify pathogenic variants of SLC26A4 associated with enlarged vestibular aqueduct (EVA) and hearing loss (HL) in two patients. METHODS: The patients were subjected to multiplex PCR amplification and next-generation sequencing of common deafness genes (including GJB2, SLC26A4, and MT-RNR1), then bioinformatics analysis was performed on the sequencing data to identify candidate pathogenic variants. Minigene experiments were conducted to determine the potential impact of the variants on splicing. RESULTS: Genetic testing revealed that the first patient carried compound heterozygous variants c.[1149 + 1G > A]; [919-2 A > G] in the SLC26A4 gene, while the second patient carried compound heterozygous variants c.[2089 + 3 A > T]; [919-2 A > G] in the same gene. Minigene experiments demonstrated that both c.1149 + 1G > A and c.2089 + 3 A > T affected mRNA splicing. According to the ACMG guidelines and the recommendations of the ClinGen Hearing Loss Expert Panel for ACMG variant interpretation, these variants were classified as "likely pathogenic". CONCLUSIONS: This study identified the molecular etiology of hearing loss in two patients with EVA and elucidated the impact of rare variants on splicing, thus contributing to the mutational spectrum of pathogenic variants in the SLC26A4 gene.

[Application of the collagen in mastoidectomy].

OBJECTIVE: To evaluate the effect of the collage application in mastoidectomy. METHOD: Forty-one patients (41 ears) with chronic suppurative otitis media (cholesteatoma type) were surgically treated by mastoidectomy, in which 22 cases (22 ears) used collagen and dissociative flap to deal with mastoid cavity but 19 cases (19 ears) without collagen. All patients were followed up for one year to observe the situation after mastoidectomy comparatively. RESULT: The epithelization time of mastoid cavity was 3 to 7 weeks, averaging (4.5 +/- 0.7) weeks in the collagen-applying group and was 5 to 12 weeks, averaging (7.2 +/- 0.8) weeks in control group. The epithelization time was significantly different between these two groups (P < 0.01). CONCLUSION: The collagen can reduce infection and promote epithelization of mastoid cavity after mastoidectomy, which is clinically worthy to be applicated in mastoidectomy.

[Cochlear implantation in patients with otitis media-related lesions].

OBJECTIVE: To present the experiences of cochlear implantation (CI) in patients with otitis media-related lesions. METHOD: Of the 9 patients with otitis media-related lesions studied for cochlear implantation, one was implanted cochlear in second-stage operation after mastoidectomy and received reconstruction of the posterior ear canal wall with cartilage with perichondrium; one with chronic otitis media (perforation) had CI and tympanoplasty with cartilage with perichondrium in one-stage operation; three with silent otitis media had CI and modified mastoidectomy in one-stage operation; three with mastoid dysplasia and middle ear adhesions had CI and tympanic exploration in one-stage operation; one with cholesteatoma localizedin attic had CI and atticotomy and reconstruction with cartilage with perichondrium. RESULT: All devices worked properly. There were no otitis media-related complications. CONCLUSION: Patients with chronic suppurative otitis media must have all disease eradicated and do the utmost to reserve anatomic landmark before CI. Patients with silent otitis media can have CI in one-stage operation, if the drainage of Eustachian tube, attic and antrum is satisfying. Patients with cholesteatoma localizedin attic can have CI in one-stage operation, but it is necessary to remove all lesions and reconstruct attic. The infection maybe an important cause of mastoid dysplasia and serious sensorineural deafness. Implanting electrode though the ear canal and promontony is a good method if the mastoid is sclerotic type and it is too difficult to implant though the facial recess. In order to avoid the retraction of drum and electrode dropping off, we can use cartilage with perichondrium to rebuild the drum, the posterior ear canal wall and the attic.

[Tympanoplasty and mastoidectomy in patients with large perforation of tympanic membrane in pars tensa].

OBJECTIVE: To present the experiences of tympanoplasty in patients with large perforation of tympanic membrane in pars tensa. METHOD: Of 147 patients who underwent surgery for chronic otitis media with large perforation of tympanic membrane in pars tensa,101 patients without mastoid problem underwent tympanoplasty, 46 patients who had evidence of middle ear, antral/epitympanic, or mastoid disease underwent tympanoplasty with mastoidectomy. RESULT: The tympanic membrane graft take rate for the entire group of 147 patients was 96.6% (142 grafts succeed). The average postoperative pure tone air-bone gap was (16.6 +/- 10.9) dBHL after 3 months of the operation. CONCLUSION: To patients with chronic supperative otitis media and large perforation of tympanic membrane, the CT scan before operation and detailed exploration of tympanum are necessary. The drainage of Eustachian tube, attic and antrum must be enough. The cartilage-perichondrium composite is an ideal material for reconstruction of the drum, the attic and the posterior tympanum.

[Measurement of transient evoked otoacoustic emissions and follow-up of 215 healthy newborns].

OBJECTIVE: To evaluate the characteristics of transient evoked otoacoustic emissions (TEOAE) of healthy newborns and improve hearing screening of newborns. METHOD: To investigate the history of pregnancy and parturition, TEOAE was measured in 425 ears of 215 healthy newborns from 0 to 8 days postpartum and follow-up study was undertaken. RESULT: The general prevalence was 89%. There was some relationship between prevalence and testing time. The average prevalence was 74% in newborns of 0-3 days. The prevalence was 97% in newborns of 4-8 days. The way of parturition, laterality and time of pregnancy over 36 weeks hadn't any significant effect on the prevalence rate. The prevalence of females was higher than males. Quick screen was more sensitive than TEOAE. One ear of hearing loss was diagnosed after follow-up. CONCLUSION: The hearing screening test of newborns should be taken in hospitals after 4 days of birth, the way of parturition had no effect on the cochlear function. The mature time of cochlear hair cells was by the 36th week of pregnancy. Quick screen couldn't take place of TEOAE. Someone who had no TEOAE in one ear or two ears must be followed up and reexamined and exact diagnosis could be made as soon as possible combining with ABR.

[Trans-external ear canal blow oxygen method].

OBJECTIVE: To study a new method for the treatment of the glue ear and the other out and middle ear diseases. METHOD: 28 ears of the glue ear were treatment with blow oxygen method trans-external ear canal after myringotomy with grommet insertion. The results were compared with that of only tube insertion group of 20 ears in same period. This method were also used to treatment the patients with otitis media after mastoidectomy to compared the effect on dry ear time. RESULT: The therapy time of the blow oxygen group for glue ear were 1.20 +/- 0.42 days, and the control group were 3.50 +/- 0.87 days. The blow oxygen group need only 3.92 +/- 0.77 weeks for dry ear after mastoidectomy. And the control group need 5.82 +/- 2.03 weeks. The results are statistic significant between this two groups. CONCLUSION: The blow oxygen method is a good way for the treatment of glue ear, and can short the time for dry ear after mastoidectomy.

[Combined reconstruction of tympanic membrane and posterior wall of external ear canal with cartilage].

OBJECTIVE: To study the reconstruction technique of tympanic membrane and posterior wall of external ear canal with auricular cartilage at same time. METHOD: The tympanic membrane and posterior wall of external ear canal of 43 cases of chronic suppurative otitis media were reconstructed with auricular cartilage after mastoidectomy between 1995-2000. The results were compared with that of without reconstruction of the posterior wall of external canal group of 60 ears in same period. RESULT: The average follow-up period was 32 months, 3 cases were faced in all of 43 cases, the others were healed in one stage. Postoperative period to be a dry ear was significantly shorter in the reconstruction group than in the canal-wall-down and open group (t-test, t = 3.98, < 0.01). The rezidive results is statistic not significant between this two groups. The patients of both groups have had good hearing results. CONCLUSION: The auricular cartilage is a good material for the reconstruction of tympanic membrane and posterior wall of the external auditory canal. It can not only reduce the time of drying ear, improve the hearing results, but also avoid the second operation. The quality of the patients life can be improved.

[Benefit of bilateral cochlear implantation on congenital prelingually deafened Chinese-speaking children].

OBJECTIVE: To evaluate the benefit of bilateral cochlear implants (BCIs) in prelingually deafened children on hearing rehabilitation as well as on speech and language development. METHOD: Two cases of congenital profound deaf children, who received unilateral cochlear implantation (CI, MEDEL C40+) on the age of 2 and 7.5, respectively, were performed secondary CI in the contralateral ear on their age of 5 and 9.5, respectively. One year after the secondary CI, the cochlear implant aided hearing threshold and speech discrimination rate were tested for both ears separately and together. The pronounce/speech distinct rate of the BCIs users were evaluated by their parents, surgeon and audiologist. RESULT: In comparison to unilateral aided ear, the mean hearing threshold at the frequency 250-4,000 Hz of the 2 cases with BCIs decreased by 13 dB and 11 dB, respectively. The speech discrimination rate of the BCIs users increased by 9% and 10%, respectively. The speech recognition in noise was improved. And their pronounce/speech distinct was improved. CONCLUSION: BCIs can provide a significant benefit in hearing, speech understanding, language development and pronounce/speech rehabilitation for prelingually deafened children.

[Expression of p27 protein and cyclinD1 in laryngeal carcinoma].

OBJECTIVE: To find out whether the expression of p27 and cyclin D1 is different in laryngeal squamous cell carcinoma and the edge tissues, and to study the relationship between the expression of p27 and cyclin D1 with the clinic character in patients with laryngeal squamous cell carcinoma. METHOD: An immunohistochemistry method was used to study the expression of p27 protein and cyclinD1 in 38 laryngeal squamous cell carcinoma and the edge tissues. RESULT: The expression of p27 protein and cyclin D1 was significantly different between the laryngeal carcinoma and the edge tissue. Low expression of p27 and over expression of cyclinD1 was associated with the liability of lymph node metastasis and the poor prognosis for patient with laryngeal squamous cell carcinoma. An interesting observation was that the inverse expression between p27 and cyclinD1 might play an important role in laryngeal squamous cell carcinoma. CONCLUSION: p27 protein and cyclinD1 in laryngeal carcinoma is apparently a significance predictor of survival.

[Radiological study of temporal bone in children with profound deafness before cochlear implant: CT vs magnetic resonance imaging].

OBJECTIVE: To evaluate the clinical application of magnetic resonance imaging (MRI) and CT in children with profound deafness before cochlear implant and their impact on surgical decision. METHODS: The pre-operative CT and MRI data from 71 children with bilateral profound deafness planned for cochlear implant were studied. RESULTS: 12 patients showed enlarged vestibular aqueduct (16.9%); 14 patients had Mondini malformation (19.7%); 3 patients demonstrated enlarged internal auditory meatus (4.2%); and 2 patients were suspected to have fistulae (2.8%) on both CT and MRI examinations. In comparison between MRI and CT, unilateral cochlear fibrosis was detected in 5 patients(7%) by MRI, but none by CT scans. Bilateral dehiscent facial nerve was detected in 1 patient(1.4%) by CT scan, while MRI was normal. CONCLUSION: CT and MRI are essential for the pre-operative planning of cochlear implant, especially in patients with enlarged vestibular aqueduct, Mondini malformation, enlarged internal auditory meatus and suspected fistulae. These two imaging modalities are complementary to each other in cases with cochlear fibrosis and facial nerve dehiscence.

[Pathological studies of vocal polyps and vocal nodes].

OBJECTIVE: In order to investigate the pathological differences and similarities of vocal nodes and vocal polyps. METHOD: 144 samples of vocal nodes and vocal polyps were examined by an experienced pathologist. RESULT: There were no significant statistical difference in short-term vocal nodes and vocal polyps. In long-term vocal nodes and vocal polyps, vocal nodes had thickened basal membrane, and abnormal keratinization of epithelium, vocal polyps had thin or discrete thickening of basement membrane, more pronounced edema, dilated vessels or growth of new blood vessels. CONCLUSION: It proved that vocal nodes and polyps are the result of "microtraumas" occurring at various phases of wound repair, thus accounting for the homogeneity and heterogeneity of histological and clinical manifestations.
