[Local defense factors in the peritoneal cavity in patients on chronic peritoneal dialysis].

Examination of the risk factors for peritonitis appearance in patients on chronic peritoneal dialysis is on great importance for better understanding of its therapy and prevention. The aim of the study was to confirm the significance of some local defense parameters of peritoneal space and its role in peritonitis incidence. The methodology was: radial immunodifussion (IgG), nephelometry (C3), ingestion of the yeast particles (phagocytosis) and bioassays for cytokine determination. The level of termostable opsonines (IgG) was significantly lower in patients with high peritonitis incidence. Also, TNF, IL6 and IL1 activity was elevated or very high in 42.1%, 57.8% and 47.3% in the same group of patients but almost unmeasurable in patients with low peritonitis incidence. Phagocytosis by peritoneal macrophages was similar in all patients and the level of termolabile opsonines was critically low in the peritoneal fluid of all patients. The presented differences in local defense parameters may partly explain the susceptibility to infection in some patients being on chronic peritoneal dialysis.

[Treatment of peritonitis in patients on chronic peritoneal dialysis with a single dose of vancomycin].

One dose vancomycinum was successful in treatment of peritonitis in patients undergoing chronic peritoneal dialysis if it was used after initial signs of disease immediately. Thirteen episodes of peritonitis treated in that manner were analysed. Microorganisms isolated previously were: Staphylococcus epidermidis in seven. Enterococcus in three, while no organisms were isolated in three cases. Vancomycinum was applied at the same time intravenously 0.5 g and intraperitoneally 0.5 g in the peritoneal solution for the six to eighteen hours. The treatment was successful in ten and failed in three cases with other medical complications: the tunnel infection in two and Tenckhoff catheter obstruction in one patient. The advantage of such treatment is in healing of most patients, it is cheap, the small drug dose diminishes its toxic effects and the duration of the hospital care is only one day. Anyway, it is not recommended in the treatment of patients with catheter obstruction, tunnel infection and other complications.

[Comparative overview of the nutritional status of patients on hemodialysis and peritoneal dialysis].

Malnutrition is often seen in patients on dialysis. Only few methods could be used in evaluation of nutritional state because of metabolic disorders which are present in patients with uraemia. In this article we evaluated nutritional condition of 50 patients which were divided in three groups according to the type of dialysis: I group--24 patients treated with haemodialysis during 6-42 months; II group--12 patients treated with intermittent peritoneal dialysis during 8-36 months, and III group--14 patients treated with continuous ambulatory peritoneal dialysis during 6-34 months. Nutritional state was evaluated with anthropometric and biochemical indicators, such as body weight, triceps skin-fold thickness, mid-arm muscle circumference, serum albumin, transferrin, C3, C4. The results have shown that signs of malnutrition were markedly present in the first year of dialysis in 33 (66%) patients. There was no significant difference in nutritional state between the three observed groups.

[Tuberculosis infection as an etiologic factor in acute renal insufficiency].

The article deals with a retrospective study of patients treated in the Institute over the period from 1980 to 1984 for acute renal insufficiency caused by tuberculous infection, as well as with the results of control examinations of the renal function in these patients from 5-49 months after the onset of the disease. The creatinine clearance under 10 ml/min was registered in 6 of 9 patients on admission. Recovery of the renal function was noted in 8 patients. The ninth patient, with unchanged terminal insufficiency had also pulmonary and extrapulmonary tuberculosis (lymphatic glands and kidneys). The treatment depended on the degree of the damaged renal function (conservative therapy or active treatment-dialysis). The follow-up of these patients revealed an increase of creatinine clearance (84.22 +/- 14.43 ml/min) in comparison to the result on dismissal from hospital (71 +/- 37.79 ml/min).

[Changes in intraocular pressure during dialysis].

We studied 29 patients on dialysis for terminal uremia, of whom one showed a dramatic rise in intraocular pressure (IOP) during dialysis. We examined the blood pressure (BP) and body weight (BW) before and after treatment in each patient and visual acuity by gonioscopic examination and optic disk status. After dialysis the statistically significant fall in BP and BW was observed in all patients, and in 28 subject an significant rise in IOP was noted. Only one patient experienced a dramatic rise in IOP, and he was the only one suffering from angular glaucoma during the last two years. All this time he was continually treated for glaucoma. After dialysis his IOP was 45 mmHg. With the adequate medication, his IOP lowered to the point when he was able to bear iridectomy which was done with argon and YAG laser. After the surgery his IOP reached the normal values. His visual acuity was good (1.0), visual field was normal, gonioscopic findings showed a narrow angle (Shaffer I, with normal pigmentation), and the optic disk had no abnormalities. It is not yet clear what was the cause of IOP rise during dialysis. It might be that the increased production of aqueous humor, which has been reported in such cases, might be caused by imbalance between blood and aqueous osmolality. If dialysis causes decrease in blood osmolality and if, at some time, blood becomes hypotonic in relation to ocular fluid, then the regular osmotic forces transfer the fluid to the ocular fluid into the eye. This case suggests that all patients on dialysis should undergo a minute ocular anamnesis and examination, and if needed administered adequate medication during the treatment.
