Preparing CNSs for prescriptive authority: Winona State University Model Course.

Medical-surgical clinical nurse specialists (CNSs) have always been involved in treating disease-based and nondisease-based health problems. Recently, expanding state nurse practice acts have allowed medical-surgical CNSs to add pharmacologic agents to their arsenal of advanced practice interventions to best manage their patients' complex healthcare needs. This article discusses the history of prescriptive authority for medical-surgical CNSs, describes a new university program for preparing CNSs for prescriptive authority, summarizes interview responses of 4 students who have completed the university program, and highlights implications for practice and education. The university program includes coursework and clinical training and integrates distance-education strategies to reach students with diverse geographic and scheduling challenges. Themes from the 4 student interviews are positive and support CNSs having prescriptive authority. Facilitators and barriers to actualizing CNS prescriptive authority, implications for educational programs, and issues surrounding distance-education and clinical training are highlighted.

Testing the generalizability of the ISO model for nursing diagnoses.

The purpose of this study was to explore whether the ISO reference terminology model for nursing diagnoses could be generalized to the MDS data set that, like nursing terminologies standardizes expressions of the concepts within and relevant to the domain of nursing practice. We first constructed paraphrased expressions of the rubrics from the data set. Next we dissected those expressions into the reference model domains of focus and judgment, recorded any qualifiers required for either domain, and semantic links required to represent associative relations. Our findings demonstrate that the ISO model for nursing diagnoses is generalizable to the MDS data set, however expansions to the model are required if the model is to be used to represent objects rather than terms.

The consultation role of a nurse educator.

Consultation is one component of the nurse educator's role. This role is rapidly expanding as health care changes. Nurse educators are being called on to provide support for the educational process in areas outside their clinical specialty. This article describes the educational consultation process in a large medical center where nurse educators have expanded their roles to ambulatory care areas. Key principles guiding the educational consultation process are delineated as well as possible uses. A five-stage process is described to aid the nurse educator in implementing the educational consultation process. In addition, some of the barriers that may be encountered during a consultation are identified.
