Oral lesions in elderly population: a biopsy survey using 2250 histopathological records.

OBJECTIVE: To perform an oral biopsy survey focused on the incidence of oral lesions in Brazilian elderly patients. METHODS: A total of 17 329 oral biopsy records were analysed and divided into two age groups: elderly patients, > or =60 years old; and non-elderly patients, <60 years old. Information about sex, race, age and histopathological diagnosis (categorised in non-neoplastic and neoplastic lesions) was collected. Differences of diagnosis incidence were tested by comparison between two proportions (binomial test). RESULTS: The incidence of epithelial malignant neoplasms and pre-malignant lesions in the elderly group was higher than non-elderly group, as well as autoimmune diseases and salivary gland tumours. The three most prevalent lesions in the elderly group were inflammatory fibrous hyperplasia, squamous cell carcinoma, and fibroma. CONCLUSION: The distribution of oral diseases using biopsies allows greater accuracy in data about oral health of elderly patients, especially when considering malignant and pre-malignant lesions.

[Effect of the discontinuity of the zygomatic arch on facial growth in young rats].

The results of experimental studies on the zygomatic arch during the growth period, through excisions or fractures, are controversial. In this study, the effect of a discontinuity of the zygomatic arch on facial growth was experimentally evaluated, without tissual damage or deviations. One-month-old rats were submitted to a surgical procedure in order to obtain a one-milimeter-wide discontinuity of the right zygomatic arch. The left side served as a control. After three months, the rats were sacrificed, their skull and hemimandibles were dissected, and radiographs were obtained. Cephalometric measurements were carried out in order to assess the dimensions of the infratemporal fossa, maxilla and mandible, by means of a computer system. There was no significant difference between the experimental and control sides as to the measurements pertaining to the maxilla and mandible. Significant difference was observed in the anteroposterior length of the infratemporal fossa. It was concluded that the consequences of a discontinuity of the zygomatic arch on facial growth are restricted to the structure itself.

Teaching oral surgery to undergraduate students: a pilot study using a Web-based practical course.

The Internet has been used in oral surgery teaching mainly to deliver learning material across the World Wide Web and to make use of online interactivity resources in everyday surgical practice, such as by e-mails, discussion groups, and chats. The aim of this paper is to describe and evaluate a Web-based practical course on oral surgery principles, which was applied to undergraduate students. This course was investigated as a distance learning simulation in which the student would be performing surgical activities at home, and the teacher and the school environment would be absent. A Web site was created containing the course material. For this study, the students participated in the Web-based course in a multimedia lab equipped with computers and Internet, internal sound system and TV circuits. In the event of significant mistakes by students, the TV circuit could be used to show the correct procedure for all the participants at the same time. Microcameras were used to monitor the student's actions during the Internet use. Students' impressions were determined by a questionnaire. Computer manipulation with ease and antiergonomic postures were observed. We concluded that distance learning courses with practical modules must be considered as a special type of educational modality, with reference to the relationship between the student and the computer.

Immunohistochemical study of the orthokeratinized odontogenic cyst: a comparison with the odontogenic keratocyst.

OBJECTIVE: Orthokeratinized odontogenic cyst (OOC) is a developmental cyst that occurs in the maxilla and the mandible and is defined by the World Health Organization as the uncommon orthokeratinized type of odontogenic keratocyst (OKC). However, studies have shown that OOC has peculiar clinicopathologic aspects and biologic behavior when compared with other developmental odontogenic cysts, especially OKCs. Therefore, in this study, the immunohistochemical profile of the OOC was delineated and compared with that of the OKC. STUDY DESIGN: Twelve cases of OOC were submitted to a panel of antibodies composed of cytokeratins (10, 13, and 14) and extracellular matrix proteins: fibronectin, types I and III collagen, and tenascin. For comparative means, 12 cases of OKC also were submitted to the same panel of antibodies. RESULTS: The results obtained showed that OOCs expressed cytokeratin 10 and showed variable expression of cytokeratins 13 and 14. Fibronectin and collagen types I and III also were expressed in OOC in a fibrillar aspect. OKC showed only the superficial keratin layer positive to cytokeratin 10 and the basal and suprabasal layers with variable expression of cytokeratin 14, and cytokeratin 13 was present in the upper epithelial layers. The extracellular matrix proteins showed a nonfibrillar expression. Tenascin was immunoexpressed only in OKC. CONCLUSION: The immunohistochemical profile of the studied cysts clearly showed that OOC presents a well-formed cystic enveloping, whereas the OKC profile is compatible with a more aggressive biologic behavior.

Pediatric oral lesions: a 15-year review from São Paulo, Brazil.

There are few studies on pediatric oral pathologies in the literature. This study presents data from a review of 2,356 biopsies of young patients (birth to 14 years) received over 15 years (1985-2000) in the Oral Pathology Service at the University of Sao Paulo, Brazil. Information about patients (sex, age, race) and histopathological diagnosis was retrieved. Diagnosis data of 2,356 biopsies were classified into 20 groups. There was no significant difference between male (50.0%) and female (49.0%) patients. White is the predominant race (69.0%), and patients ages were concentrated between 9 and 14 years old (70%). Mucocele was the most frequent (13.5%), followed by dentigerous cyst (6.5%) and fibrous hyperplasia (5.4%). Papilloma and Langerhans cells histiocytosis were the most common non-odontogenic benign and malignant tumors, respectively. In the group of odontogenic tumors, odontoma was the most frequent, and ameloblastoma had a significant incidence (27 cases). These data are important in order to detect differences in geographic areas, diagnosis line tendencies and for clinicians to perform judgment to evaluate of the pediatric patients before the biopsy and management of pediatric oral lesions.
