The management of a multigenerational nursing team with differing qualifications: a qualitative study.

Retiring age is no longer fixed at 39 years of working service by Italian law, so nurses with more than 40 years of service continue to work in the wards, creating a distinct older generation. Moreover, with the closure of nursing colleges and the introduction of university degree courses for nurses, nursing education has changed significantly. This article reports on a study which explored the experiences of chief nurses in the daily management of nursing teams composed of multigenerational nurses who differed in age and qualifications. Husserls' phenomenologic approach was used, interviewing at length a purposeful sample of 10 chief nurses. Five main areas of concern emerged: (a) The generation gaps between peers and team leaders; (b) nomadism versus being settled; (c) conflicting methods of working which result in nothing being accomplished: managing the "flat paradox"; (d) different nursing values, ideals, and visions; and (e) existing generation gaps with the younger staff. The number of younger nurses will increase with time and, as a consequence, the relevance of intergenerational differences must be seriously debated, not only difficulties with age differences but also those relating to nursing education.

[Epidemiology of caring problems and drug surveillance in the elderly: a cooperative development of research protocol].

Nurses care for elderly patients at home and in nursing homes but there is lack of data on the caring problems encountered, their causes and the solutions adopted. The aim of the study is to produce the epidemiology of such problems and, at the same time, focus nurses' attention on their causes and avoidability. The process of definition of caring problems (the focus of the study) and the discussion that brought to the draft protocol are described. The final protocol (presented with the data collection forms) is articulated in two phases: a prevalence survey through a random sampling of points estimates in index days over 6 months; a prospective registry over the same period of time of all causes perceived as clinically relevant by the nursing personnel, who will document the suspected risk factors and causes (including specifically drug therapy), and the interventions and measures which are activated.

[A double blind randomised clinical trial to assess the efficacy of the treatments of the superficial pressure sores].

In spite of the progresses of knowledge and care, pressure sores continue to be a clinically relevant problem. A double blind randomised controlled trial was organised to assess the efficacy of triticum vulgaris (Fitostimoline) vs placebo in the re-epithelisation of superficial pressure sores. Patients with stage NPUAP II or superficial pressure sores, with an expected survival of more than 3 months and eligible for a follow-up up to 8 weeks were included, over a period of 2 years in 46 clinical sites. The protocol was approved by local ethical committees and informed consent was obtained before randomisation. Medications were performed by nurses if the patient was hospitalised and by nurses or properly instructed caregivers at home. Weekly follow-up controls were assumed by nurses. Out of the 294 randomised patients 270 were included in the analyses. The two groups are comparable for the main characteristics except for Norton Scale mean values, less severe in the group assigned to active treatment (10.1+/-3.7 vs 8.9+/-3.2). The mean follow-up was of 3.8 and 4.2 weeks with a mean duration of 26+/-18 and 29+/-18 days for the experimental group and controls respectively. Seventy-six patients in the treatment group and controls (58.0 and 54.7) had their lesions re-epithelized. Adjusting results for age, initial Norton and Push scores there are no differences between treated and controls (OR 0.99 95% IC 0.60-1.67). This multicentre study, sponsored by a research group of nurses, failed to support the hypothesis that triticum vulgaris, the active component of the product Fitostimoline, given on top of recommended treatment, provides a specific therapeutic advantage in terms of frequency and timing of re-epithelization in superficial pressure sores.

[Why a debate on nursing management?].

The aim of this monographic issue of Assistenza Infermieristica e Ricerca is to open a discussion on nursing management, its contents and contribution to the development of the nursing profession but also its scientific and evidence based bases, and specifically: to highlight the open questions that deserve more extended and further research; to compare problems and solutions adopted in practice, with the results of the main studies available to identify main trends and effective organizative nursing models to suggest the implementation of changes to improve working conditions and patients outcomes. The published contributions were produced by the students of the Specialistic Degree Course for Nurses and Midwifes of Verona University who selected the relevant topics, searched and critically appraised the literature. This monographic issue, that will be published in two parts (number 1 and 2 2006) is divided in three main areas: The first will deal with a fundamental question: whether a nursing management exists and when and how it should be taught. The second, selects a series of topics of relevant interest for nurses and summarises the main evidences available. Each contribution presents: the definition of the problem (i.e. what is the nursing shortage; its contributing factors, actual and future trends and possible solutions available); examples of relevant studies; a critical debate, focusing on hot questions, unresolved problems, issues that deserve further research. suggestions for further studies. Relevant articles and Mesh terms for independent literature search are suggested. In the third part, methodological contributions will be presented, with a critical appraisal of protocols of organizative research studies and an analysis of strengths and limitations of the studies on the organization of care.

[Aims and core contents of basic and post basic courses of nursing management].

The second part of the issue, dedicated to the nursing management offers a discussion on some of the relevant areas of debate for the organization of nursing care. A reflection on the core contents and main aims of basic and post basic (master, advanced level, specialistic degree) nursing courses is proposed in the first contribution. Then a selection of topics is presented, focusing on the main research contributions available in the literature and on the ongoing debate for the following areas: the role of communication at the inter-shift handover and the new model of bedside reporting; the problems and challenges of doctors nurses collaboration in the health care team; the problems and expectations of new nurses joining the health care team and the process of development of practical skills; the hot debate on nursing roles and level of practice and the difficulties in differentiating between elementary, specialist and advanced nursing practice; the new emerging roles: the characteristics and achievements of Nurse Consultant; the new challenges of coordinators of the health care teams with the management of the multigenerational nursing team. For each topic a definition of the problem is presented, an example of relevant study, a critical debate and suggestions for further studies and areas of research. A list of the main journals that deal with nursing management is proposed with comments on the main characteristics of each journal. A study protocol that aims at describing the outcomes of the different styles and models of organization of medical wards on patients and nurses is presented as an example of organizative research; an experience of, and a model for, participation of the nurses to health policy planning is proposed. Nurses were, asked to identify health care problems encountered in their daily practice, their causes and propose solutions at basic and policy level. The session of International Observatory explores the main problems and difficulties of the research on the organization of care, focusing more on opportunities than on the challenges and drawbacks of the available literature.
