[Salivary cortisol determination in the diagnosis of pediatric Cushing's disease].

Endogenous Cushing's syndrome in children is a very rare entity. Diagnosis is complex due to the wide variety of tests available. Recently, the utility of salivary cortisol has been proposed for the diagnosis of this entity, although greater evidence is required, especially in pediatric patients. We report a 14-year-old girl with Cushing's disease due to a pituitary microadenoma in whom salivary cortisol determination was very useful for the diagnosis.

Multipeaked polarons in soft potentials.

We consider a minimal coupled charge / excitation-lattice model capturing a competition between linear polaronic self-trapping and the self-focusing effects of a soft nonlinear on-site potential. The standard single-humped polaron ceases to exist above a critical value of the coupling strength, closely related to the inflection point in the nonlinear potential. For couplings beyond this critical value, we find that successive multihumped polaronic solutions correspond to the lowest-energy stationary states of the system, which may admit interesting quantum resonance behavior.

Post-coital administration of levonorgestrel does not interfere with post-fertilization events in the new-world monkey Cebus apella.

BACKGROUND: Experimental evidence to disprove the belief that emergency contraception with levonorgestrel (LNG) prevents pregnancy by interfering with post-fertilization events is lacking. Here we determined the effect of post-coital and pre-ovulatory administration of LNG on fertility and ovulation, respectively, in the Cebus monkey. METHODS: To determine the effect on fertility, LNG 0.75 mg or vehicle were administered orally or s.c. once or twice within the first 24 h after mating occurring very close to the time of ovulation. Females that became pregnant were aborted with mifepristone and re-entered the study after a resting cycle until each of 12 females had contributed, in a randomized order, two LNG and two vehicle-treated cycles. To determine the effect on ovulation, LNG 0.75 mg or vehicle were injected twice coinciding with follicles smaller or larger than 5 mm in diameter. Six females contributed five treated cycles each. RESULTS: The pregnancy rate was identical in vehicle- and LNG-treated cycles. LNG inhibited or delayed ovulation only when treatment coincided with a follicle <5 mm diameter. CONCLUSION: In Cebus monkeys, LNG can inhibit or delay ovulation but, once fertilization has taken place, it cannot prevent the establishment of pregnancy. These findings do not support the hypothesis that emergency contraception with LNG prevents pregnancy by interfering with post-fertilization events.

Nonlocal interaction effects on pattern formation in population dynamics.

We consider a model for population dynamics such as for the evolution of bacterial colonies which is of the Fisher type but where the competitive interaction among individuals is nonlocal, and show that spatial structures with interesting features emerge. These features depend on the nature of the competitive interaction as well as on its range, specifically on the presence or absence of tails in, and the central curvature of, the influence function of the interaction.

Hepatic haemodynamics: interrelationships between contrast enhancement and perfusion on CT and Doppler perfusion indices.

This study compares three techniques that evaluate hepatic haemodynamics for the detection of metastatic liver disease to determine the interrelationships between the techniques and to assess their equivalence. The three techniques studied were dedicated CT measurements of hepatic enhancement, CT measurements of perfusion and Doppler perfusion indices. 53 patients with proven malignancies of either breast or colon underwent a single location dynamic CT for measurement of hepatic perfusion and enhancement, whilst a subset of 12 patients underwent both CT perfusion and Doppler perfusion studies. Statistically significant correlations were found between CT arterial phase enhancement and CT arterial perfusion (r=0.612, p<0.001), and between both of these parameters and Doppler arterial flow (r=0.867, p<0.001 and r=0.842, p<0.001, respectively). Significant correlations were also found between both the ratio of CT arterial enhancement to peak enhancement and the CT arterial perfusion with the Doppler perfusion index (r=0.797, p=0.002 and r=0.725, p=0.008, respectively). Combined CT arterial and portal perfusion correlated with peak liver enhancement (r=0.614, p< 0.001), but Doppler measurements of portal flow did not correlate with any CT parameter. Increased arterial enhancement, perfusion or flow are valuable additional radiological signs for the presence of hepatic metastases that can be elicited by incorporating any one of these methods into existing imaging protocols.

Standardized perfusion value: universal CT contrast enhancement scale that correlates with FDG PET in lung nodules.

The standardized enhancement value and standardized perfusion value allow comparison between different methods for quantification of contrast enhancement during computed tomography (CT). Standard perfusion values calculated from CT measurements of perfusion within pulmonary nodules compared favorably with those derived from previously reported enhancement data and correlated with standardized uptake values obtained from positron emission tomographic images (r = 0.8, P <.01).

[Drug interactions in chronic prescription].

OBJECTIVE: Application of computerized program for detection of potential drug interactions (PDI) in chronic prescriptions in four primary care centers. To evaluate the clinical significance of PDI identified according to clinical criterions. DESIGN: An observational crossover study. SETTING: Clutat Vella health district (City of Barcelona). MEASUREMENTS AND RESULTS: Using information of Consejo General de Colegios Oficiales de Farmaceuticos databases and the chronic prescriptions database of the primary care centers, computerized drug-interaction system have been developed for detection of PDI in patients. A panel of primary care physicians and clinical pharmacists developed criteria that were used to evaluate the clinical significance of PDI. 9840 Cards of Authorized Prescription (CAP) were analyzed, 36108 medicaments and 42877 drugs. A total of 2140 patients were involved for a total of 3406 PDI, 21.75% of patients with CAP. Clinical signification for the panel was found in 40.07% of these 3406 PIF; 3.78% were suggest to avoid the association drugs. CONCLUSIONS: The incidence of PDI with clinical signification are lower than other studies of the literature; it suggest a appropriate knowledge of drug prescription. The application of computerized program make much more easy the detection of adverse drug interactions in chronic prescription.

Role of 17 beta-hydroxysteroid dehydrogenase in the modulation of nuclear estradiol receptor binding by progesterone in the rat anterior pituitary gland and the uterus.

Progesterone has been shown to decrease occupied pituitary and uterine nuclear estradiol receptor (E2R) binding in mature and immature estrogen-primed rats. Progesterone has also been shown to stimulate pituitary but not uterine 17 beta-hydroxysteroid dehydrogenase (17 beta-HSD) in the rat. The conversion of estradiol to its less active metabolite estrone by 17 beta-HSD and activation of phosphatase are among mechanisms considered to be involved in the reduction of E2R. To determine if 17 beta-HSD stimulation was a mechanism by which progesterone induced nuclear E2R decrease, the synthetic estrogen ethinylestradiol, which is not oxidized by 17 beta-HSD, was used instead of estradiol to prime adult ovariectomized rats. When ethinylestradiol-primed rats received 0.8, 2.0 or 4.0 mg/kg body wt of progesterone 2 h before sacrifice, the total and occupied nuclear E2R accumulation in the anterior pituitary by a subsequent ethinylestradiol injection 1 h later did not show any decrease. This response was different from that observed previously in estradiol-primed animals in which progesterone showed a multiphasic decrease of occupied form of nuclear E2R. However, in the uterus of ethinylestradiol-primed rats, a partial decrease of total and occupied nuclear E2R accumulation was observed in the presence of the three doses of progesterone used. The decrease of uterine nuclear E2R with the three progesterone doses was different from the dose-dependent effect of progesterone observed in the uterus of estradiol-primed rats. Affinity constants of the interaction between [3H]estradiol and the nuclear E2R were similar among groups treated with ethinylestradiol, estradiol and progesterone. These results demonstrate the involvement of 17 beta-HSD in the reduction of anterior pituitary gland E2R by progesterone in the estradiol-treated animals. Furthermore, the mechanism of decrease of E2R by progesterone in the uterus appears to be different from the pituitary gland.

Inhibitory effect of 5-alpha-dihydroprogesterone on nuclear estrogen receptor binding of the anterior pituitary and uterus in the rat.

Progesterone and its metabolite 5 alpha-dihydroprogesterone (5 alpha-DHP) have been shown to bring about gonadotropin release in the estrogen-primed ovariectomized rat. One of the actions of progesterone is the decrease in occupied estrogen receptors (E2Rs) in the anterior pituitary and uterus. This study attempted to determine if 5 alpha-DHP had a similar effect on pituitary and uterine E2Rs. Estrogen-primed ovariectomized mature female rats were injected with either vehicle, or 0.2, 0.8 or 2 mg/kg body weight (BW) of 5 alpha-DHP, 2 h before sacrifice and 1 h before the last estradiol injection (2 micrograms/rat). Nuclear E2Rs were determined by Scatchard analyses in the uterus and anterior pituitary. Total nuclear E2R levels of both tissues showed a 2-fold increase in the number of estradiol-binding sites after estradiol administration, as compared to control groups. In estradiol-primed rats, 5 alpha-DHP produced a significant decrease in total nuclear E2R levels in a tissue-specific manner. In the pituitary, there was a maximal and significant decrease in nuclear E2Rs with 0.2 and 2.0 mg/kg BW of 5 alpha-DHP as compared to estradiol alone; the intermediate dose of 0.8 mg/kg BW of 5 alpha-DHP induced a smaller nonsignificant change in nuclear E2Rs. In the uterus, 5 alpha-DHP showed a dose-dependent decrease in nuclear E2Rs. The 5 alpha-DHP effect in both tissues was due to a specific reduction in the occupied form of nuclear E2Rs levels. The unoccupied form of E2Rs was unaffected by 5 alpha-DHP administration. 5 alpha-DHP did not have any effect in the absence of estrogen priming.(ABSTRACT TRUNCATED AT 250 WORDS)
