A nonlinear biomechanical model for evaluation of cerebrospinal fluid shunt systems.

In view of complications arising from physical properties of cerebrospinal fluid shunts, a biomechanical model of hydrocephalus was set up to study in vivo parameters that may influence their function. These include: intracranial pressure, compliance and pulses, intrathoracic, intra-abdominal, and subcutaneous pressures, and the effects of siphonage and repeated valve flushing. Each of these factors was studied separately upon shunt implantation in the model. Results of testing of a sample low-pressure valve with antisiphon device conformed with consumer information in regard to valve opening pressure and pressure flow measurements. No customer information, however, was supplied concerning the deleterious effects of direct subcutaneous pressure, variable degrees of siphonage, and repeated valve flushing that were demonstrated by the model. Such results indicate that shunts should be similarly tested prior to marketing and implantation in patients.

During evolution of the surgical procedure to implant ventriculojugular (VJ) shunts against the direction of blood flow for the treatment of hydrocephalus, various modifications were made in the shunting catheter and in the implantation technique with the aim of improving the results. Shunt implantation was difficult in infants and young children with compressible craniums and the modifications failed to improve the results. Accordingly, they were not considered suitable for this method of cerebrospinal fluid (CSF) shunting. Our present policy is to implant VJ shunts in hydrocephalic patients with rigid craniums and ventriculoperitoneal (VP) shunts in infants and young children to tide them over a period until they become suitable for VJ shunting. During the past 5 years, 110 hydrocephalic patients with rigid craniums were treated by implanting VJ shunts without need for further modifications. The shunt was revised in one patient during the early postoperative period due to CSF leakage under the scalp. There were no complications related to CSF overdrainage in any of the cases.

Cerebrospinal Fluid Shunts

A study on the prevalence of tuberculosis in a rural community of Bangladesh.

The population of two villages of Chagalnaiya Upazila was selected for a study on the prevalence of pulmonary tuberculosis through a two stage screening process. Initial screening was conducted on 3406 persons which is 78% of the total population (3920) of the two villages. The initial screening revealed 182 persons with the history of cough lasting 4 weeks or more, chest pain, continuous fever and haemoptysis. Sputum examinations for Acid Fast Bacilli (AFB) of tuberculosis detected 21 sputum positive cases giving a prevalence rate of 6.2 (per 1000 population) higher than the estimated national average of 5.0 per 1000. Most of the cases were not receiving treatment. Some social and environmental factors of suspected and detected cases were also studied.
