Identification and quantitation of human urine proteins by capillary electrophoresis.

Capillary electrophoresis, as a fully automatable, powerful analytical technique, has the potential for use in clinical laboratories for separation and identification of protein types in normal and abnormal human urine specimens. Abnormal urine specimens containing previously identified proteins, anion-exchange resin treatment and addition of known components were used to identify and determine migration times of the peaks found in the electropherogram of human urine. The correlation coefficients between capillary electrophoresis and the currently used method of high-resolution agarose gel electrophoresis for Bence Jones protein and albumin were found to be 0.95 and 0.93, respectively.

Factors in childhood as predictors of asthma in adult life.

OBJECTIVE: To determine which factors measured in childhood predict asthma in adult life. DESIGN: Prospective study over 25 years of a birth cohort initially studied at the age of 7. SETTING: Tasmania, Australia. SUBJECTS: 1494 men and women surveyed in 1991-3 when aged 29 to 32 (75% of a random stratified sample from the 1968 Tasmanian asthma survey of children born in 1961 and at school in Tasmania). MAIN OUTCOME MEASURES: Self reported asthma or wheezy breathing in the previous 12 months (current asthma). RESULTS: Of the subjects with asthma or wheezy breathing by the age of 7, as reported by their parents 25.6% (190/741) reported current asthma as an adult compared with 10.8% (81/753) of subjects without parent reported childhood asthma (P < 0.001). Factors measured at the age of 7 that independently predicted current asthma as an adult were being female (odds ratio 1.57; 95% confidence interval 1.19 to 2.08); having a history of eczema (1.45; 1.04 to 2.03); having a low mild forced expiratory flow rate (interquartile odds ratio 1.40; 1.15 to 1.71); having a mother or father with a history of asthma (1.74 (1.23 to 2.47) and 1.68 (1.18 to 2.38) respectively); and having childhood asthma (1.59; 1.10 to 2.29) and, if so, having the first attack after the age of 2 (1.66; 1.17 to 2.36) or having had more than 10 attacks (1.70; 1.17 to 2.48). CONCLUSION: Children with asthma reported by their parents in 1968 were more likely than not to be free of symptoms as adults. The subjects who had more severe asthma (especially if it developed after the age of 2 and was associated with reduced expiratory flow), were female, or had parents who had asthma were at an increased risk of having asthma as an adult. These findings have implications for the treatment and prognosis of childhood asthma, targeting preventive and educational strategies and understanding the onset of asthma in adult life.

Treatment of bile leaks from the cystohepatic ducts after laparoscopic cholecystectomy.

The cystohepatic ducts represent accessory bile ducts of variable size which frequently travel within the gallbladder fossa or in the posterior wall of the gallbladder. These ducts can be injured during laparoscopic cholecystectomy and can result in bile collections if transected. Successful treatment by operative means or radiologically guided percutaneous drainage is possible, but endoscopic management has several advantages. We describe cases managed by endoscopic retrograde cholangiopancreatography (ERCP) with stent placement and discuss the advantages of this method. Also discussed is the anatomy of these accessory bile ducts, additional management options, and techniques for avoiding this injury during open or closed cholecystectomy.

The associations between childhood asthma and atopy, and parental asthma, hay fever and smoking.

The aim of this analysis was to examine the degree to which a life time prevalence of asthma in a 7-year-old child is statistically associated with atopic conditions of the child, and with parental asthma, hay fever and smoking. In 1968, 8585 children who were born in 1961 and who were attending school in Tasmania were surveyed. This comprised 99% of the eligible population. The prevalence of a history of asthma in the 7-year-olds was 16.2% (males 19.0%, females 13.2%). Multiple logistic regression analysis showed that a history of asthma in a 7-year-old was associated with the child being male (odds ratio [OR] 1.56; 99% confidence interval 1.30-1.86), having a history of hay fever (3.86; 3.12-4.78), eczema (2.04; 1.63-2.55), hives (1.34; 1.09-1.65) or allergy to foods or medicines (1.70; 1.26-2.30), the child's mother or father having a history of asthma (2.63; 2.08-3.31 or 2.52; 1.99-3.19, respectively), and the mother being a smoker (1.26; 1.05-1.51). Parental hay fever and paternal smoking were not independently associated with childhood asthma. The strength of association between childhood asthma and parental asthma was independent of the sex of either the parent or the child, and of atopic conditions in the child. In the 133 children for whom both parents were asthmatic, 65 (49%) had a history of asthma.(ABSTRACT TRUNCATED AT 250 WORDS)

The abnormality of normal comparison groups: the identification of psychosis proneness and substance abuse in putatively normal research subjects.

OBJECTIVE: Careful assessment of research subjects is important because the inclusion of subjects who manifest psychopathology and significant substance abuse in normal comparison groups will decrease statistical and experimental power. The current study evaluated the usefulness of an MMPI-derived algorithm in identifying tendencies toward psychosis and substance abuse in putatively normal research volunteers. METHOD: Ninety-eight adults who were recruited as normal comparison research subjects completed the MMPI, psychiatric interviews, questionnaires, and selected neuropsychological tests. The MMPI classified 81 presumed normal subjects into four subgroups: 1) not psychosis prone/substance abuse not likely, 2) not psychosis prone/substance abuse likely, 3) psychosis prone/substance abuse not likely, and 4) psychosis prone/substance abuse likely. RESULTS: The MMPI psychosis-prone and substance abuse factors identified significantly distressed and dysfunctional individuals with a relatively high degree of accuracy. CONCLUSIONS: It is becoming increasingly apparent that the cursory self-report screening of normal subjects may result in unacceptable levels of psychopathology in comparison groups. The current results also indicate that an adequate substance abuse evaluation is extremely important and that brief self-report information may be misleading. Empirically derived assessment tools, such as the MMPI, may prove useful in allowing researchers to more accurately define control parameters and group membership.

Wisconsin Card Sorting Test deficits in chronic paranoid schizophrenia. Evidence for a relatively discrete subgroup?

Some, although not all, researchers have reported dramatically increased numbers of perseverative responses on the Wisconsin Card Sorting Test (WCST) in schizophrenic patients compared to normal comparison subjects. The current study was designed to further explore the nature of possible WCST deficits in a group of paranoid schizophrenic patients compared to normal and psychiatric comparison subjects. In the current study, schizophrenic patients had significantly greater numbers of perserverative responses on the WCST than the comparison groups. The sample of patients with schizophrenia appeared to be characterized by a non-Gaussian distribution of perseverative responses on the WCST. WCST-impaired and WCST-nonimpaired schizophrenic subgroups were compared on cognitive and symptom measures, and increased perseverative responding was associated with negative symptoms, slowed reaction time, and more hospitalizations. While additional research is necessary to further investigate hypotheses of frontal versus generalized brain dysfunction in schizophrenic patients, WCST impairment seems to be present in a clinically meaningful subgroup of paranoid schizophrenic patients.

Accuracy of asthma death statistics in Australia.

Asthma mortality statistics issued by the Australian Bureau of Statistics (ABS) were compared with clinical data from a survey of asthma mortality. Deaths in Victoria from May 1986 to April 1987 containing 'asthma' in Parts 1 or 2 of the death certificate (N = 405) were reviewed. For each subject, the cause of death attributed by the Victorian Asthma Mortality Survey was compared with the ABS cause of death, by age and sex of the subject. Information on 393 of the 405 deaths investigated by the Victorian Asthma Mortality Survey was analysed. The ABS estimate of the total number of asthma deaths in Victoria was 47 per cent higher than the estimate of the Victorian Asthma Mortality Survey. In subjects under 50 years of age the two estimates were within 10 per cent. The difference between the estimates increased with age at death for persons over 50 years old and was equivalent for males and females. If the assessment by the Victorian Asthma Mortality Survey of the number of deaths due to asthma is accepted as accurate, then the ABS estimate of asthma deaths was reliable for those under 50 years of age. In those who died at an older age, the ABS significantly overestimated the number of deaths due to asthma in Victoria.

Effect of prolactin on alpha-aminoisobutyric acid uptake in mouse mammary gland explants.

Studies were carried out to characterize the effect of prolactin (PRL) on amino acid uptake into mammary gland explants derived from 12- to 14-day pregnant mice. PRL approximately doubles the rate of uptake of alpha-aminoisobutyrate (AIB), arginine, and hydroxyproline but has no effect on the intracellular accumulation of unincorporated leucine. PRL increases the apparent maximum velocity of AIB uptake from 0.16 to 0.43 nmol.l intracellular water-1.min-1. No effect of PRL, however, on the apparent Michaelis constant (0.75 mM) of AIB uptake was observed. The onset of the PRL stimulation of AIB uptake occurs 4-5 h after PRL addition to the cultured mammary tissues. The lowest stimulatory concentration of PRL on AIB uptake is 5-10 ng/ml, and a maximum response is achieved with 75 ng/ml. The action of PRL on AIB uptake requires both ongoing RNA and protein synthesis, since puromycin (50 micrograms/ml), cycloheximide (5 micrograms/ml), and actinomycin D (1 microgram/ml) abolished the PRL response.

Use of antiasthmatic drugs in Australia.

Bronchodilator prescription and sales data for Australia from 1975 to 1986 were analysed. Inhaled beta-agonists were the most commonly prescribed antiasthmatic agents in 1986, followed by sustained-release theophylline and inhaled corticosteroids. Marked increases in prescription sales of these three drug groups over the period were noted. We estimate that non-prescription sales of salbutamol inhalers increased by 410% between 1980 and 1986 (P less than 0.01). Prescribing rates by general practitioners for beta-agonists, sustained-release theophylline and inhaled corticosteroids increased from 1979 to 1986. Prescribing rates of beta-agonists were highest for persons aged 1-11 years and 55 years and over, and increased markedly for children aged 1-11 years over the period. For sustained-release theophylline, prescribing rates were highest for persons 55 years and over. The difficulties in postulating possible relationships between prescribing trends and asthma mortality are discussed.

Physiological evidence of exaggerated startle response in a subgroup of Vietnam veterans with combat-related PTSD.

One of the diagnostic criteria for posttraumatic stress disorder (PTSD) is an exaggerated startle response; however, this phenomenon has not been verified empirically. The authors compared 20 Vietnam combat veterans with PTSD and 18 combat veterans without PTSD on the eyeblink reflex electromyographic response of the startle reaction. Subjects in both groups who failed to show an eyeblink response to the startle stimuli were eliminated from further analyses. Among the remaining subjects, the 13 with PTSD had a significantly greater startle response amplitude than the 12 control subjects at intermediate intensities of acoustic stimuli. The relationship between startle responsivity and both negative and positive symptoms was also investigated.

Pacemaker follow-up: its role in the detection and correction of pacemaker system malfunction.

The goal of pacemaker follow-up is not only to detect battery depletion but also to detect all malfunctions of the pacing system and, when possible, to correct such problems using programming. During one year, we discovered 61 such malfunctions in a clinic of 1065 patients (5.7%). These were more frequent in the first year (7.7%) than in the third to fifth years of follow-up (range 3.1-4.8%). The incidence rose again in the sixth and subsequent years (7-7.7%). Despite a significant occurrence of malfunctions (5.2%) among multiprogrammable pacemakers, the necessity for operative intervention for their correction was low (1.2%). Sensing problems were the most common (57%) and the most likely to be corrected by reprogramming (85%); problems involving loss of capture were less likely to be corrected by programming (38.5%). Battery depletion accounted for only 18% of malfunctions, occurring earliest in the forty-third month of follow-up. Pulse generator longevity of those devices reaching end of battery life during the study period was 68.6 +/- 16.7 months (mean + SD). We conclude that specialized pacemaker follow-up continues to be necessary despite improved pulse generator reliability and longevity. Indeed, with reprogramming, it presently plays an even more important role than in the past. Follow-up should be oriented not only to the detection of battery depletion but also toward a comprehensive surveillance of pacemaker system function.

Trends in Australian mortality of asthma, 1979-1985.

Trends in Australian mortality rates that have been ascribed to asthma and to non-tuberculous, non-malignant respiratory diseases were examined for the period 1979-1985. Mortality rates of asthma and respiratory diseases increased substantially from 1979 to 1985, and in the case of asthma, the crude death rate in 1985 was higher than at the peak of the "epidemic" of the 1960s. The increase in mortality of asthma was most pronounced in those of greater than 60 years of age (from 11.5 per 100,000 such population in 1979 to 22.9 per 100,000 such population in 1985). Asthma mortality rates in five- to 34-year-old persons increased, although there were marked yearly fluctuations, and deaths of asthma in this age-group comprised only a small percentage (13% in 1985) of all deaths of asthma. Reasons that are considered for the rising death rate of asthma include changes in diagnostic fashion; an increase in the incidence or a change in the natural history of asthma; and an increased case fatality rate. With the presently-available data, it is not possible to distinguish among these possible explanations.

Evaluation of hemoglobin A2 quantitation assay and hemoglobin variant screening by capillary electrophoresis.

To critically assess the method of capillary electrophoresis, we examined 97 clinical samples submitted for hemoglobin electrophoresis by both conventional methods (electrophoresis on cellulose acetate and citrate agar for detection of abnormal variants, ion-exchange chromatography for quantification of HbA2, alkali denaturation for quantification of HbF, supravital stains for detection of HbH) and CE. CE was performed using a 72 cm (50 cm to detector) x 50 microns i.d. fused-silica capillary with detection of absorbance at 200 nm. Of the 97 samples examined, 34 contained a hemoglobin variant. Migration time, the difference of the variant peak to that of HbA, was used in an attempt to identify the hemoglobin variant by CE. We found the method to be suitable for the quantification of HbA2, useful as a screening technique for the presence of hemoglobin variants, but unsuitable for the quantification of HbF.
